Statements Mission To eliminate Alzheimer's disease through the advancement of research; to provide and enhance care and support for all affected; and to reduce the risk of dementia through the promotion of brain health. Background In 1979, Jerome H. Stone and representatives from several family support groups met with the National Institute on Aging to explore the value of a national, independent, nonprofit organization to complement federal efforts surrounding Alzheimer's disease. That meeting resulted in the April 10, 1980, formation of the Alzheimer's Association with Mr. Stone as founding president. Today, the Association reaches millions of people affected by Alzheimer s across the globe through our national office and more than 75 local chapters. As the largest donor-supported, voluntary health organization for Alzheimer s, the Association is a catalyst for advancements in Alzheimer's research and care. Impact Alzheimer's Association Key Accomplishments FY09 FY15 350,000 advocates and 120 Ambassadors successfully worked with leaders in Congress and with the White House to create and enact the National Alzheimer s Project Act. Worked closely with the top leadership of the Social Security Administration to achieve the inclusion of Alzheimer's and certain other dementias in the Compassionate Allowances initiative. Successfully pushed for the inclusion of cognitive impairment detection as mandatory part of the Medicare Annual Wellness visit. Fielded nearly 700,000 calls at the nation s only 24/365 toll free Alzheimer s Helpline Created and released the first revised Alzheimer s diagnostic criteria in 27 years in partnership with the National Institute on Aging. Developed recommendations to support clinical trials requested and implemented by the Food and Drug Administration. Rebranded and grew our signature event, Walk to End Alzheimer s with a 36% increase in participation over three years and 10% growth in revenue. Published and promoted key resources about Alzheimer s disease and its impact, including our annual Facts and Figures, Changing the Trajectory of Alzheimer s: A National Imperative, Generation Alzheimer s, HBO s The Alzheimer s Project, and The Shriver Report: A Woman s Nation takes on Alzheimer s Developed and successfully launched the Know the 10 Signs: Early Detection Matters campaign. Funded nearly 250 peer-reviewed grants to advance Alzheimer s research. Helped more than 13,000 Trial Match participants begin profiles to sign up as volunteers for clinical trials in order to help speed research Hosted and grew the Alzheimer's Association International Conference, converting the gathering into an annual event that draws thousands of participants from around the world. Selection twice by The Nonprofit Times as one of the Best Nonprofits to Work For, winning the designation, Best Large Nonprofit to Work For in Piloted first successful rural outreach effort in Lawton, Oklahoma in which culminated in the establishment of an Lawton satellite office and staff member to serve Commanche County and surrounding communities. 2nd satellite office established in Enid, OK providing programs and services to west central Oklahoma. 2

But until that day comes, we will continue to build public awareness, offer hope to families, provide support and advocacy for all those affected, and advance the research necessary to discover treatment and a cure. 4 Program Success Monitoring Examples of Program Success Success is monitored by senior management and the Medical and Scientific Advisory Council. The year 2007 entered the history books as a period of tremendous advances in Alzheimer research. These advances occurred on all fronts, encompassing areas as diverse as genetic risk factors for Alzheimer's, imaging studies to identify Alzheimer's in the living brain and blood tests that may one day be used to diagnose Alzheimer's. 4

5 Care Connections Description Care Connections includes the direct assistance programs we provide to families affected by Alzheimer's and educational programs for the community, family and professional caregivers. These programs include Individualized Family Care Consultations, Town Halls, The Basics of Alzheimer's Disease, Living with Alzheimer's, Savvy Caregiver, Caregiver Essentials, Foundations of Dementia, Activity Based Alzheimer's Care (ABAC), First Responder Training, Professional In-service Training, Lunch and Learn sessions, 24/7 Helpline, Support Group Facilitator Training, Drawing on Memories, Early Access to Support and Education (EASE), Medic-Alert, referrals and resource information and an annual Education Conference. First Responders will be able to quickly identify and effectively help persons with dementia thus maintaing safety both for the person with the disease and the general public, persons with dementia will be connected to others who are going through the same process and professional caregivers will be able to provide better care for diagnosed individuals. Ultimately the communities we serve will be knowledgeable about Alzheimer's, families will be better able to communicate with and care for their loved one with Alzheimer's, keep them socially active as long as possible and make it easier for familiesble to keep their loved one in the home as long as possible. Improve the quality of life for people affected by Alzheimer's disease through dementia-specific education, fostering of communication methods and caregiving strategies, dementia-specific training of professional caregivers and connection to local resources. Surveys are conducted for every educational program and results are regularly reviewed. Feedback from families we have helped enable us to hone and enhance our programs and services to better meet the needs of the growing epidemic that is Alzheimer's disease. The success, depth and personalization of our direct assistance and educational programs is difficult to describe in our own words.
