We are very excited to host this meting and look fward to having you join us. The meeting will be held at the the Panamericano Buenos Aires Hotel. *NEW* The day will begin at 9:00 on the mning of Thursday May with presentations and discussions regarding the CHIPS Trial. We will have the pleasure of hearing from our guest speaker Dr Edgardo Abalos, Centro Rosarino de Estudios Perinatales (CREP), specialist in antihypertensive treatment in pregnancy. The meeting will begin at 9:00 a.m. and will conclude at approximately 4:00 in the afternoon. On Thursday evening, dinner has been planned f all collabats. Then, on Friday May 15 we will focus on discussions about the Twin Birth Study and conclude the meeting at approximately 2:00pm. During the meetings we look fward to providing detailed infmation on each of the studies. In addition, updates on the current status of the trials, suggestions on the ganisation and conduct of the studies at your site, and helpful hints f successful recruitment and data collection. We would be pleased f you to join us on both days, even if you currently only participate in one study. If you want to receive the TBS CHIPS Infmation Package befe the meeting, please let us know and we can help you get started with the studies. What you need to do now: 1. Read the following infmation. 2. Decide who will be attending the meeting. 3. Complete the attached attendance fms to let us know if you are able to come. If me than one person is attending, please send a separate fm f each person. 4. Return the attendance fm by fax as soon as possible. 5. 2 PLANNING YOUR TRAVEL F reimbursement, please remember to save your receipts so you can be reimbursed after you become active. Details of how to request your reimbursement can be found on page 5. If you are an active TBS CHIPS centre: By active, we mean that you have sent us both your local Research Ethics Board Approval Certificateand your signed Clinical Study Agreement (CSA). We will reimburse travel expenses f two collabats per active centre. We will cover your hotel stay during the meeting and meals that are provided as part of the meeting. If you have local funds that can cover travel expenses f additional study staff, we would be pleased to pay f their hotel stay during the meeting and meals that are provided as part of the meeting, so that others involved in these studies can attend. If you have not yet joined TBS CHIPS and are an interested centre: We will cover your hotel stay during the meeting and meals that are provided as part of the meeting. Once we have received your Research Ethics Board Approval Certificate and signed Clinical Study Agreement (CSA), we will reimburse travel expenses f two collabats per centre. Travel: F this meeting, we ask that you arrange your own travel. Air: Train/Bus: Car: We will reimburse an economy airfare booked 14 days in advance (in der to obtain the most reasonable fare) and transptation from the airpt to the hotel (and return). We will reimburse an economy train/bus fare to and from the hotel. We will cover mileage at the standard Sunnybrook Health Sciences Centre rate of 43 cents/km, as well as parking at the hotel. *Please note: if you make other travel arrangements, the equivalent ption of your ticket will be reimbursed. Accommodation: We will book your rooms at the Panamericano Hotel, and pay f a standard room and taxes f up to 3 nights. Additional nights, charges f room service, mini bar, movies, etc will be at your own expense. Please use the attached reply fm to tell us who will be attending. 5 Reimbursement Infmation When you submit your travel reimbursement please note the following: Travel reimbursement can only be made when ORIGINAL receipts and boarding passes are submitted. If you have an electronic ticket, submit the documentation receipt of the transaction along with all ORIGINAL boarding passes. Include a copy of the invoice from your travel agent if the price is not clearly indicated on the other documents. F transptation fares to/from an airpt, please submit the iginal receipts given by the driver taxi company. If you are travelling by car, we will cover mileage at the standard Sunnybrook Health Sciences Centre rate of 43 cents/km, as well as parking at the hotel. We can only cover costs f accommodation at the hotel (i.e. room & taxes only). Any other items charged to the room are your responsibility.
