The meetig was a great success thaks to the hard work put i by Greg Olavarria, MD, FAANS ad Jogi V. Pattisapu, MD, FAANS(L). There were early 400 attedees i a outstadig veue. As you kow, the Sectio has had to be more budget-mided with meetigs ad Thursday ight evets have ot bee scheduled for the past two years. It seems this has tured out to be a great opportuity to socialize with colleagues, so I thik this tred will cotiue. Curretly a professor of eurosurgery at the Uiversity of South Florida (USF), he has edited ie books o pediatric eurosurgery ad wrote the Hadbook of Pediatric Neurosurgery ad Neurology with Sarah J. Gaskill, MD, FAANS. He has i additio authored close to 100 articles. Dr. Marli traied at McGill Uiversity, the Uiversity of Miesota ad New York Uiversity. His most ifluetial metors have bee Theodore Brow Rasmusse, MD; Joseph Rasohoff, MD; ad Frederick J. Epstei, MD. He was the foudig chief executive officer at the Methodist Childre s Hospital of South Texas ad held that positio for five years while i active practice of pediatric eurosurgery. He is a seior member of the America Society for Pediatric Neurosurgery (ASPN). With almost 40 years of experiece i pediatric eurosurgery, his mai cliical iterests are hydrocephalus, epilepsy, spasticity ad tumors. The committee is curretly directig two major iitiatives: (1) the secod year of a successfully revamped Pediatric Sectio Grat ad (2) ogoig participatio i the Neurosurgical Research & Educatio Foudatio (NREF) grat program. Last year, the Sectio approved a sigificat chage i the format of the aual research award i respose to several years of limited applicatios. The Fellow/Juior Faculty Research Grat was iitiated, chagig the target populatio of the grat to fellows/ew faculty i order to recruit ad retai pediatric eurosurgeos egagig i research focused o problems uique to pediatric eurosurgery. The fudig level is $10,000/year, with discretio to award as a sigle grat or divide it ito two grats of $5,000. 5 AANS/CNS Sectio o Pediatric Neurosurgery Travelig Fellowships The AANS/CNS Sectio o Pediatric Neurosurgical Surgery offers the followig travelig fellowships: 1) The Iteratioal Travelig Fellowship is for eurosurgeos who, at the time of their applicatio, are either traiig i a residecy program outside the U.S. ad Caada or who have completed residecy traiig outside the U.S. ad Caada withi the past five years. The fellowship will cover the travelig ad livig expeses for a three-moth period to be spet observig the activities of a established pediatric eurosurgical service of the applicat s choosig i the U.S. or Caada. Up to two fellowships will be awarded aually o the basis of a competitive evaluatio by a committee of the Pediatric Sectio. The fellowship stiped is $7,500. 2) The Domestic Travelig Fellowship is for residets i accredited North America eurosurgical traiig programs. The fellowship is iteded to cover travelig ad livig expeses for oe moth for a residet who wishes additioal experiece i pediatric eurosurgery durig residecy. The oe-moth fellowship ca be spet at ay pediatric eurosurgery program withi North America to pursue a activity which broades the residet s exposure to pediatric eurosurgery, icludig observatio at a cliical or research ceter, participatio i a research project or ay other relevat activity. 6 Message from the Presidet of the AANS Pediatric eurosurgery has recetly passed the 25-year mark sice the iceptio of the America Board of Pediatric Neurological Surgery (ABPNS). Sice that time, over 200 idividuals have completed fellowships approved by the Accreditatio Coucil for Pediatric Neurosurgery Fellowships (ACPNF) ad have passed their pediatric subspecialty qualificatios, allowig them to hold a 10-year, time-limited ABPNS certificate. For the first decade followig the establishmet of the ABPNS, less tha 10 pediatric eurosurgery fellows per year completed ABPNS certificatio (Table 1). I the last decade, iterest i pediatric eurosurgery has bee o a upswig (Table 2). I 2016, 13 idividuals passed their ABPNS credetialig process ad aother 24 passed the recertificatio process. Likewise, the America Society of Pediatric Neurosurgeos (ASPN) ow has over 250 members. As such, the subspecialty of pediatric eurosurgery appears to be havig a reaissace. Lookig back over the quarter cetury sice the developmet of the ABPNS, we have see the survival rate of childre with the most commo brai tumors more tha double. Advaces i the treatmet of hydrocephalus have led to dramatic reductios i the umber of shuts ad shut revisios beig performed at most major pediatric facilities. Our shut ifectio rates are tredig steadily dowward. Advaces i euro-imagig ad electro-diagostics have led to earlier ad better treatmet of childre with epilepsy. Newer age-specific implats have led to better maagemet of pediatric spial disorders of all types. Spial deformity surgery i childre is rapidly becomig part of the armametarium of pediatric eurosurgery rather tha beig relegated to orthopaedics. I most tertiary pediatric hospitals with more tha oe pediatric eurosurgeo, we are ow seeig further subspecializatio withi the field. Pediatric eurosurgeos have also gaied stature i leadership positios withi orgaized eurosurgery. At preset, pediatric eurosurgeos hold leadership positios withi the AANS, the Cogress of Neurological Surgeos (CNS), the Society of Neurological Surgeos (SNS) ad the ABNS, just to ame a few. It seems to me the perfect occasio for us to ackowledge the presciet work of our predecessors who had the foresight to both orgaize ad execute the developmet of our subspecialty i such a maer that pediatric eurosurgery ow serves as a model for fellowship traiig ad sub specializatio withi eurosurgery. I his article o the Memoradum of Uderstadig (MOU) betwee the ABPNS ad ABNS, Joh Ragheb, MD, FAANS, details the aligmet of the two orgaizatios i a way that should cotiue to beefit pediatric eurosurgery. With the implemetatio of these agreemets, pediatric eurosurgeos will o loger hold a diploma from a rump board, but will be recogized by the America Board of Medical Specialties (ABMS) for the special qualificatios that we hold. 7 ABPNS ABNS Update The America Board of Pediatric Neurological Surgery (ABPNS) was established i 1996 by a dedicated group of pediatric eurosurgeos to idetify ad recogize subspecialty practice ad expertise i pediatric eurosurgery at a time whe orgaized eurosurgery refused to idetify subspecializatio. I order to establish ad codify the optimal traiig ad practice requiremets for pediatric eurosurgeos, 40 seior pediatric eurosurgeos from the U.S. ad Caada met i Chicago i the summer of By uaimous vote, they fouded the ABPNS, ad the origial 152 Diplomats were certified i Sice that time, a total of 283 eurosurgeos have bee certified, ad may of the origial group have recertified for the third time. The Board is recogized by hospital ad isurace credetialig bodies ad our Diplomates are active across eurosurgery. Times have chaged. The America Board of Neurological Surgery (ABNS) is ow committed to idetifyig subspecialties i eurosurgery ad recogizes that the model for subspecialty credetialig created by the ABPNS should be embraced. The Committee o Advaced Subspecialty Traiig (CAST) has also ackowledged that the Accreditatio Coucil for Pediatric Neurosurgical Fellowships (ACPNF) requiremets for ad mechaisms of moitorig of fellowships traiig will remai ad is the model for subspecialty fellowship traiig. I additio, both the ABNS ad CAST will defer to the ABPNS ad ACPNF regardig the credetialig of pediatric eurosurgeos credetialed by the Caadia Royal College. Therefore, i a effort to improve ad streamlie the certificatio ad cotiued certificatio processes, the ABPNS Board of Directors has siged a memoradum of uderstadig with the ABNS that will allow the two orgaizatios to work together. The stregth of the two orgaizatios will come together toward the shared goals of a efficiet, effective ad educatioal credetialig ad re-credetialig process. This trasitio will take place over the ext several years, as the processes for certificatio of pediatric eurosurgeos evolves. Although may of the details are still i developmet, the Board of Directors wated to provide a broad outlie of the plaed chages to our Diplomates ad over the ext several moths will cotiue to fill i the details of the process as it evolves. Periodic updates will be added to the ABPNS.org Board website. It will remai the place to look for details of the certificatio ad re-certificatio processes. The iitial pediatric eurosurgery subspecialty exam will be take withi oe year of completig a ACPNF accredited pediatric eurosurgery fellowship ad will be available to those that have completed accredited traiig i eurosurgery recogized by the ABNS or Caadia Royal College. The applicat will the submit case logs from their post-fellowship practice demostratig a pediatric eurosurgery-focused practice i compliace with ABNS/ ABPNS criteria ad, oce approved, will take a combied ABNS/ ABPNS oral examiatio. There will be a trasitio period over the ext several years, allowig those that have already graduated, but ot yet take the ABNS exam, to sit for the ABPNS writte exam. The exam will be give electroically at ABNS testig sites, but the cotet will still be developed by the ABPNS. The curret ABNS Maiteace of Certificatio (MOC) process ad the ABPNS re-certificatio process will evolve ito a uified system of cotiuig educatio that focuses o core cocepts ad advacemets that alter the fudametals of practice. The goal is to have a sigle process that is valuable to Diplomates ad focuses o cotiued learig i our ever-evolvig field ad is specific to the practice of pediatric eurosurgery. Those re-certifyig will still meet case log requiremets verifyig pediatric-focused practice. The educatioal material that will be the core of this ew process is still uder developmet ad therefore those recertifyig i 2017 will still be subject to the origial ABPNS exam process. I additio, i order to better alig the ABNS ad ABPNS categories of certificatio, the ABPNS ow recogizes those who have retired from our field as well as those who are cliically iactive. 8 Update from the Accreditatio Coucil for Pediatric Neurosurgery Fellowships The past year has see some major chages i the pediatric eurosurgery fellowship accreditatio process. The Accreditatio Coucil for Pediatric Neurosurgery Fellowships (ACPNF) has siged a memoradum of uderstadig (MOU) with the Society of Neurological Surgeos (SNS) Committee o Accreditatio of Subspecialty Accreditatio (CAST). Accordig to the terms agreed upo i the MOU, the ACPNF will cotiue to establish stadards for fellowship accreditatio ad determie if programs meet these stadards. The ACPNF will cotiue to admiister the pediatric eurosurgery fellowship match. The ACPNF will share iformatio with CAST o accredited programs. CAST will o loger make decisios o pediatric fellowship accreditatio idepedetly of the ACPNF ad will o loger offer accreditatio for pediatric programs except i coordiatio with the ACPNF. The ACPNF will facilitate CAST efforts to share pediatric eurosurgery fellowship data with the Accreditatio Coucil for Graduate Medical Educatio (ACGME). The orgaizatios have eacted bylaws chages to reflect the ew relatioship. More detail is available o the website for those who are iterested i learig more about these chages. For the last two cycles, we have required programs to submit case data for the hospital as well as for idividual fellows. O the basis of the data that has bee submitted, we aticipate movig forward with several ew case-volume requiremets i future accreditatio cycles. The ratioale for these requiremets was discussed i detail at the recet program director meetig that was held durig the America Society of Pediatric Neurosurgeos (ASPN) Aual Meetig i Jauary. These additioal metrics will esure the best learig eviromet for our traiees ad will be useful as we begi data sharig with the ACGME. There are ow 28 accredited fellowship programs. A complete ad up-to-date list of all curretly accredited programs may be foud here. I the last cycle, 29 idividuals etered the match. Of these, 18 were graduates of U.S. or Caadia residecy programs. After some discussio at the program director meetig, the decisio was made to hold the fellowship match i the week followig the Pediatric Sectio meetig. Examiatio of a cadidate s kowledge ad judgmet typically icludes queries regardig preoperative evaluatio, itraoperative techical ad judgmetal issues ad post-operative maagemet, icludig appropriate treatmet of complicatios. The Oral Examiatio of the America Board of Neurological Surgery (ABNS) has udergoe periodic review ad revisio over the years. The curret format icludes three, 45-miute examiatio sessios that focus o craial, spial ad other eurosurgical topics. Differet medical boards coduct their oral examiatio i differet ways ad some do ot have a oral examiatio at all. The ABNS seeks to maitai a oral examiatio process that is relevat, rigorous ad of value to both our specialty ad for public safety. Commecig with the Sprig 2017 examiatio, a ew format will be used. The ew format icludes three, 45-miute sessios: Oe focused o geeral eurosurgery topics Oe focused o a subspecialty topic that the applicat may choose (spie, tumor, vascular, trauma/critical care, fuctioal, pediatric or a secod geeral sessio) Oe focused o a evaluatio of the applicat s ow cases. At preset, each applicat submits 150 cosecutive cases as part of their credetialig process. From this pool, the ABNS will select 10 cases for possible discussio, five or six of which may be discussed. The purpose of this sessio is for the applicat to take the examiers through his or her case maagemet process with a focus o kowledge, maagemet ad safety. Cadidates who pass the oral examiatio become Certified Diplomates of the ABNS. Richard G.
