Narrative Description of Research: Schizophrenia is a major public health problem that affects 1% of the general population and has devastating effects on the psychological and financial resources of the patient, family, and larger community. Unfortunately there is still no clear understanding of the pathology although recent research has made it increasingly clear that biological factors may play an important role. One promising area of research has focused on the detection of abnormalities in the brains of patients afflicted with schizophrenia. Here, both post-mortem and recent magnetic resonance (MR) structural imaging studies suggest that temporal lobe frontal lobe and limbic system abnormalities, may be implicated in the pathophysiology of schizophrenia. Many believe that these abnormalities, both structural (as characterized by structural magnetic resonance and post-mortem studies) and functional (as characterized by functional MRI and PET studies), are due to the disruptions in connectivity between frontal and temporal lobes. This theory, referred to as disconnection theory of schizophrenia, has lately gained a great deal of attention among reserachers. Until recently there were no studies to demonstrate direct evidence for the existence of an anatomical disconnection between different brain regions in schizophrenia. The goal of this investigator s research program has been to apply new imaging techniques to the study of schizophrenia in order to determine and to localize white matter brain abnormalities in this disorder which likely underlie the symptoms and disordered behavior observed in patients diagnosed with schizophrenia. Accordingly, over the past three years, this investigator has been involved in trying to define and to localize abnormalities within fronto-temporal connections in the brain of schizophrenics, using Diffusion Tensor Imaging and Magnetization Transfer Imaging. These techniques have now been used by the investigator and his colleagues to quantify: (1) the uncinate fasciculus; (2) the cingulate fasciculus; (3) the corpus callosum; and, (4) the internal capsule. Results demonstrate that schizophrenic patients, compared to normal controls, show reductions in diffusion anisotropy, a measure of coherence and organization of the fiber tracts, in fronto-temporal connections. Additionally, reduced anisotropy within one of the investigated areas (uncinate fasciculus) in patients with schizophrenia, was correlated significantly with lower scores on neuropsychological measures of learning that involved mnemonic operations of encoding and retrieval, but not with measures of executive functions of problem solving and planning. These findings suggest the importance of investigating white matter tracts in vivo in schizophrenia and they support the hypothesis of a disruption in normal connectivity between fronto-temporal brain regions. Moreover, these findings suggest that disruptions in left UF connectivity of frontal and temporal regions in schizophrenia may be associated with verbal memory impairments. Another line of investigator s research is to implement new, more automated methods for analyzing the brain, including voxel based techniques, that may replace the laborious tracing of smaller regions of the brain. Thus one of the next projects of investigator was to apply volumetric automatic voxel based analysis to brains of unmedicated patients with first episode schizophrenia. This focus is important as it allows for the comparison
4 between this automated procedure with the manual region of interest tracing analysis, as well as making it possible to discern whether, and where brain abnormalities are evident early in the course of the illness, prior to confounding variables such as a chronic illness and long term effects of neuroleptics. This method of analysis is also being extended to white matter, and both orientation and organization of the fiber tracts (as measured by DTI), as well as myelin component of the fiber tracts (as measured by MTR) are being quantified on a voxel by voxel basis by the investigator in yet another project. Work using functional Magnetic Resonance Imaging (fmri) has also begun in an attempt to understand further cognitive processes underlying semantic encoding and retrieval, as these processes are frequently linked to the function of both frontal (inferior frontal) and temporal (superior temporal) brain regions, and if abnormal in schizophrenia, they would directly point to frontal-temporal disconnections and lead to a better understanding of the pathophysiology of schizophrenia.
