6 BTI IMPLANTS Narrow Universal Universal Plus Wide The Interna connection by BTI has the following main features: Exclusive four-lobe anti-rotation design. Entrance cone to facilitate the insertion of prosthodontic components. Designed to withstand high torques without the connection suffering deformations. The most advanced design technologies have been used to calculate the stress distribution. Guaranteed bone stability around the implant thanks to the study of the load stress distribution. The dimensions maximise material in critical areas and thus increase resistance to breakage and fatigue. The dimensions of the connection are always the same for all implants, regardless of the platform. The BTI Externa connection is characterised by: Having a hexagon with different side and height dimensions depending on the implant platform. This design is a result of the indications of the implants, therefore, the Wide platform implants will have a larger hexagon than the universal platform implants, as they are indicated for giving prosthetic solutions to those areas of the mouth with greater biomechanical requirements. Children: Permanent implants are not intended for use in children who have not concluded their growth and development stages. Women who are pregnant or breastfeeding: There are no additional preventive measures other than those generally accepted in oral implantology techniques. However, these particular situations should be considered by the clinician before placing the implants. Re-use and/or re-sterilisation of implants may cause risk of infection and/or cross contamination. 6

7 TECHNICAL SPECIFICATIONS BTI implants TRANSPORTER Transporter with depth marks and with the reproduction of the connection on the same, to control its positioning. INTERNA CONNECTION EXTERNA CONNECTION Two connections. Three different platform diameters with the widest range of prosthodontic components on the market. 1 o 2 o 3 o TWO SURFACES optima The optima surface is adapted to the different tissues in contact with the implant in order to promote and accelerate osseointegration. NECK 1 st The optima surface up to the platform is adapted to favour the retention of marginal tissues and reduce the risk of periimplantitis. CRESTS 2 nd The optima surface of the threads presents a porosity that allows bone anchoring to its exterior, favouring the initial stability and improving osseointegration. VALLEYS 3 rd The optima surface of the valleys presents an ideal average roughness suitable for guiding the bone growth between the threads at the same time as maintaining the mechanical properties of the implant. unicca (Surface optima + Ca) The unicca surface is a chemical modification with calcium ions, thus providing a number of advantages: ADHESIVE AND PLATELET-ACTIVATING Reduces regeneration times. OSTEOGENIC Induces the formation of bone tissue. PROCOAGULANT Provides immediate stability. ANTIBACTERIAL Significantly minimises the risk of periimplantitis. ELECTROPOSITIVE, CLEAN AND ACTIVE Maintains the superhydrophilic properties. TRIPLE ROUGHNESS Optimises the adaptation to the different tissues and improves osseointegration. APEX There are two possible apexes for BTI implants: 1. An apex with exceptional advance/drive capacity and a small apical surface that prevents compression at this level, providing control of the direction and the location of the implant in narrow alveolar crests. It also provides a greater osseointegration surface. This is the standard apex. 2. An apex with a lower advance/drive capacity and large apical surface adapted to achieve primary stability without compression in anatomical areas where significant injury to structures is possible such as to the maxillar sinus and dental nerve. 16 EXPANDER IMPLANTS TRANSITIONAL The expander implants by BTI are the perfect alternative and complement for conventional bone expansions. They are transitional implants that are inserted in the alveolar bone crest when there is not enough width to fit a definitive TINY implant. They are, therefore, an alternative to block bone grafts for the reconstruction of the atrophic alveolar process. They have a specific surface treatment that increases bone apposition, but different to that carried out on definitive implants, making their explantation easier. + More information about the expander implants in the brochure BTI solutions for atrophic maxillae. 16

18 CLINICAL-PROSTHETIC COMPONENTS Narrow platform Universal Platform Wide platform This chapter presents all of the attachments for making both cemented and screw-retained provisional and/or final prostheses. It will set out the technical specifications of each of them and provide a basic explanation of how to perform the prosthetic work. 19 NEW SURFACE TREATMENT TI-GOLDEN Abutments BTI prosthetic attachments are evolving to improve the quality of the prosthetic work. Ti-Golden is a coating that improves the aesthetics and the biological interface by decreasing the accumulation of the bacterial plaque. ADVANTAGES Golden finish: The golden colour achieves a significantly aesthetic improvement by transmitting a greater warmth to the gingival tissues. 20 HEALING ABUTMENT INTERNA AND EXTERNA The slotted healing abutments are transepithelials made from commercially pure titanium with a smooth external surface finish that enables perfect healing of the soft tissues. They can be cut with thick diamond drills and polished with fine grain drills and rubber if it is necessary to customise the outline. (E.g. in case of pressure in the space of the papilla). The Interna connection abutments are supplied sterile in heat sealed blister packs. Thus they can be used in the patient's mouth straight from the package, without needing to be sterilised in the clinic. The Externa connection abutments do not come in blister pack. The new expanded healing abutments have a wider geometry in the supragingival part. Available for Interna connection, universal platform with expanded diameter from 4.1mm to 6 mm (total height 5 and 6 mm) and wide platform with expanded diameter from 5.5 mm to 7 mm (total height 5 and 6 mm). As main advantage, this expanded platform protects the soft tissue during the osseointegration period. Both abutments are retained directly into the implant (with a torque of 20 Ncm) in the first surgery for single stage implants, or when the second surgery is performed, in cases of two-stage implants. * BTI recommends using the narrowest abutment, provided this is possible, to gain gingival width, so the papilla will have greater irrigation and consequently, less recession. Re-use and/or re-sterilisation of these single-use products may cause risk of infection and/or cross contamination. 20

22 IMPRESSION COPINGS AND OCCLUSAL REGISTRATION INDICATORS INTERNA AND EXTERNA IMPRESSION COPINGS This part is used to make impressions directly to the implant or to a transepithelial abutment. Individual fenestrated trays or disposable perforated plastic trays are used for the indirect technique and closed trays are used for the direct technique. They are manufactured from commercially pure titanium and consist of two parts, the body that has the shape of the connection and the screw that must be tightened manually or with a large hexagon screwdriver. In the indirect technique, this screw must be loosened once the impression material has set and before removing the tray from the patient's mouth. The copings for the direct technique are made of steel AISI303. OCCLUSAL REGISTRATION INDICATORS These are used for occlusal registration in edentulous patients in cases of immediate load. They are screwed directly onto transepithelial abutments with the DIRO screwdriver before making impressions. 22

These new products make the transfer from the Universal platform (diameter 4.1 mm in the implant) to the Wide platform (diameter 5.5 mm prosthetic structure). They maintain the two-part structure (sleeve and screw), and the surface with Ti Golden treatment and Ti Black screw. The Multi-Im transepithelials Interna Connection are available in 4 heights: 2, 3, 4 and 5. The transepithelial Unit Interna connection and Multi-Im Externa connection are available in 3 heights: 2, 3 and 4 mm. Insertion into the implant at 35 Ncm torque. INDICATIONS They are indicated for rear areas of the mandible which need a wider prosthetic emergence than that offered by a Universal platform. They facilitate the stability of the soft tissues, in rear areas of the jaw, in the second surgery or in cases of immediate load. Having positioned an implant from the Universal platform, they distribute the biomechanical load better in these rear areas of the mandible. 62 LOCATOR ABUTMENT INTERNA AND EXTERNA Abutment for overdentures, in one single piece that must be screw-retained into the implant at a torque of 30 Ncm. We will select the abutment that is exactly at the same height as the tissue or the next size up. This size will enable the 1.5 mm of the fixing head to emerge over the level of the gum that surrounds the implant. PROPERTIES Auto-aligning, as they provide easy positioning and correct seating of the patient's overdentures; in addition they eliminate additional wear and the loss of bonding due to incorrect seating. Angle corrections of up to 40º, in divergent implants, using the same abutment, therefore it is not necessary to place angled pillars. The lowest vertical height of the market, an advantage especially when the interocclusal space is limited. The total height of fixing is 2.5 mm for an Interna connection implant. 66 TITANMAGNETICS ABUTMENTS INTERNA AND EXTERNA The Titanmagnetics abutments are retention elements for overdentures. The system consists of two magnets: The first is screw-retained into the implant and is presented for Tiny implants and for the Universal platform, for both Interna and Externa connections. The second is incorporated onto the prosthesis. In addition it has a position hose, which is placed on the screw-retained magnet and this enables the second magnet to be seated perfectly upon it, accurately indicating where the prosthesis must be placed. It also works as the insert for the transport and application of torque to the magnet that is fixed to the implant. This maximum recommended torque is 20 Ncm. ADVANTAGES The fixing with magnets is dynamic and simple for the patient to put in and remove; especially for much older patients, thus facilitating hygiene. It is indicated for solutions for basic and economical prosthetic work. 66

74 SURGICAL DRILLS INTERNA AND EXTERNA The design of the BTI drills enables you to work at low revolutions (50-75 rpm) without irrigation, which means you can recover all the bone when drilling and use it as an autograft. It is also possible to work with the parameters of conventional drilling, at high revolutions if required. Drilling at low revolutions provides excellent control of the drilling depth and perfect visualisation of the depth marking, reducing the chance of causing damage to significant structures. It also helps to control the direction of drilling on the vertical. With the new drilling technique, as it is more accurate, the neo-alveolus created will house the implant with greater initial stability. At the same time, from a biological point of view, the socket, as it has been made without irrigation, will be in the best condition for revascularisation and the subsequent osseointegration of the implant. The depth marks are at: Short drills 3, 5, 8.5, 10 and 13 mm Long drills 3, 5, 8.5, 10, 13, 15 and 18 mm ADVANTAGES The initial drill has a very active tip and with excellent penetration capacity in the cortical bone. It achieves precise drilling and an exact location of the starting point for the drilling, particularly in narrow alveolar crests, and better directional control. It also enables lateral drilling in case we want to modify the location of the neo-alveolus by half a millimetre. It is the ideal drill to use in immediate post-extraction implants, as it allows us to drill accurately into the wall of the post-extraction alveolus. This drill must always be used with abundant irrigation, either with saline solution or with pyrogen-free sterile water, and at revolutions between 800 and 1,000 rpm. The next drill is 1.8 mm diameter, with an exclusively apical cut. Its special helicoidal design is very good for retaining the bone particles that are cut. This drill is used almost exclusively in narrow alveolar crests. The rest of the drills with larger diameters are used progressively, depending on the diameter of the implant to be placed and on the bone quality. They can all be used without irrigation as long as you work at low revolutions. 74

85 DYNAMOMETRIC SURGICAL WRENCH Narrow Universal Universal Plus Wide INTERNA AND EXTERNA LLMQ Surgical ratchet wrench with torques 20, 30, 40, 50, 60 and 70 Ncm enables you to know the exact insertion torque of the implant in the maxillary bone. In order to decide which torque to apply, BTI's scientific directorate recommends detecting the bone density with density identification tools, such as the implant simulation and diagnosis software based on CT scans, BTI Scan 3. For suitable fitting of the implant with a controlled torque, the surgical motor must be set at Ncm and the implant inserted. Once the preset torque has been reached in the motor (20-25 Ncm), continue the manual insertion with the wrench, setting the desired torque depending on the bone quality and the calibre of the implant; thus, in narrow implants or in cases of wide implants poor quality bone (Type IV-V), the wrench must be positioned at 40 Ncm; otherwise, with wide implants placed in bone type I, II or III, the torque selected shall be 50 Ncm. INSTRUCTIONS FOR USE Rotate the torque range regulator in both directions to achieve the desired torque; the torque is read using the markings on the handle of the wrench: 20, 30, 40, 50, 60, 70 Ncm. Once the torque has been set, fit the adapter of the ratchet wrench and proceed to the insertion of the implant. When the preset torque has been reached, the wrench will turn without applying torque (with a characteristic sound). 85

They must be inserted into one of the bodies of the screwdriver supplied. The set can be used as a manual screwdriver. Turning the handle produces the rotation of the interchangeable screwdriver tips. It can be used for inserting or removing screws or healing abutments. The components of the handpiece are reinforced so that the application of torque does not deteriorate the splines; its special design enables it to be perfectly cleaned, lubricated and sterilised like any surgical handpiece. It enables the application of up to 80 Ncm of torque, so we must monitor the force when tightening screws; otherwise, they shall be an instrument of choice for unscrewing tightened parts with a high torque. 100 EXPLANTATION KIT This innovative system enables the safe and traumatic extraction of implants with both internal and external connections of practically any brand of dental implants with prosthetic connection. The extractors are inserted in the connection of the implant and adapted, using an extender, to the special wrench included in the kit. Thus the implants are detached safely by torsion, as bone-implant union is broken. In the event 200 Ncm is reached, the wrench forms an angle and prevents progress. If necessary, it is repositioned using a cylindrical part. INDICATIONS The system is indicated for the extraction of osseointegrated implants when they are no longer functional or are unnecessary in a new prosthetic situation. 110 MICROIMPLANTS FOR ORTHODONTICS Kit: KIO BTI offers microimplants for orthodontics manufactured from Grade 5 Titanium alloy with different diameters and lengths, designed to be the fixed anchor for the different orthodontic movements, used as a complement to natural teeth for this purpose. They are used temporarily while they are needed as an anchor, and are later removed. They are inserted transmucosally on adhered gum. CHARACTERISTICS OF THE HEAD The head of these implants has two crosswise grooves that house the corresponding screwdriver during insertion. Similarly there is a hexagonal area so they can be manipulated with the system connector. CHARACTERISTICS OF THE THREAD The external thread has a self-tapping profile with a sharp tip, so drilling is not performed prior to the insertion of the implant. The thread profile is anti-expulsion, in order to withstand the tractions to which it is subjected. It has a wide thread pitch to avoid breaking the bone. The threads are 0.5 mm deep. Children: They are indicated for this population group for temporary use. Women who are pregnant or breastfeeding: There are no additional preventive measures other than those generally accepted in oral implantology techniques. However, this particular situation should be considered by the clinician before placing the implants. BTI offers its users in the field of oral surgery single-use kits with the necessary equipment for the habitual surgical treatment. With these single-use kits we increase the biosafety of the treatment, reducing the working time and avoiding the sterilization of the materials for use. They also comply with the quality standards required by the European health authorities. Children, and pregnant and lactating women: There are no additional preventive measures other than those generally considered for Endoret (PRGF ) technology. However, these particular situations should be considered by the clinician before its application. 118 BTI Scan 3 This section presents the software that BTI has developed to facilitate implant surgery. Children: Permanent implants are not intended for use in children who have not concluded their growth and development stages. Women who are pregnant or breastfeeding: There are no additional precautions than those generally considered in radiology and oral implantology techniques, nevertheless, these states must taken into account by the clinician, before subjecting the patient to a radiology examination (CT) and subsequent use of BTI Scan 3 for diagnosing and planing implant treatment. 118

After the radiologist performs the scan of the patient, a set of slices are obtained that are sent to the dentist on a CD-ROM, in order to view the case in three dimensions and make the corresponding study using a computer with the software BTI Scan 3. 120 OPERATION STEPS The patient arrives at your clinic, and after an examination, they will be diagnosed with an absence of teeth or the need to replace existing ones. With a scanner a series of axial sections are obtained which the radiologist stores on a CD-Rom that will be sent immediately to the dentist. BTI Scan 3 reformats the axial sections to show three types of section per screen: axial, panoramic and sectional. BTI Scan 3 allows you to carry out a much quicker, effective and precise analysis of the CT scan, helping to adequately plan the surgery and its long-term predictability. BTI Scan 3 generates panoramic images that show the mandibular foramen, making possible to draw its trajectory, (which can be seen in the panoramic section and in the sectional views) and observe any dental pathology the patient may have. BTI Scan 3 allows you to simulate the placement of the implants, being able to define the length, diameter, angulation and distance between implants and teeth with greater ease. It offers your patients confidence. The 3D simulation and virtualisation of BTI Scan 3 will allow them to feel much safer with the operation. The dentist can print out a final report of the planning, adding their own notes and recommendations for the surgery. With BTI Scan 3 the chances of success with surgery increase considerably. 120

122 CLEANING AND STERILIZATION All the prosthetic components and surgical instruments are supplied clean and hermetically packaged, but only those where it is marked on the label are sterilised. The device that comes from the factory sterile is subject to the following process: 1. Cleaning and degreasing. 2. Cleaning in clean room and hermetic packaging. 3. Sterilization by gamma rays. For the other devices that are not supplied sterile, all components intended for use in the mouth must be sterilised in autoclave before first use. (See procedure for sterilisation in autoclave) If the instruments are reusable, they must be reprocessed after every use. Reprocessing consists of three stages: 1. Manual or automatic cleaning. 2. Manual or automatic disinfection. 3. Sterilisation in autoclave.
