3 Headline and description of the planned project: Identification of relevant imaging biomarkers for retinal changes and therapy response in patients with diabetic macular edema Diabetic macular edema is one of the leading causes of visual impairment in the working-age population in developed countries. Anti-VEGF has been identified in the past years to be an effective drug for the treatment of DME but treatment strategies are heterogenous and no treatment scheme could be identified to be effective for every patient and every pathomorphological appearance. The next step in the era of anti-vegf DME treatment now must therefore be the individualization of treatment for every patient with respect to individual morphological changes in the retina. Optical coherence tomography is the major imaging modality used for evaluation of retinal diseases like diabetic macular edema. The novel OCT generations are able to provide 3D raster images of high resolution. This produces a level of detailed pathomorphological insight close to histopathology. Central retinal thickness had been the major criterion for retinal image evaluation in large clinical trials up to today. Scientists to date agree that a detailed and reproducible analysis of the microstructural parameters detectable with SD-OCT will provide the necessary realistic guidance for flexible patient monitoring and anti-vegf treatment. But clinical trials to date have not included SD-OCT technology into their treatment paradigms; moreover, morphologic features were not classified, nor were the specific alterations quantified and correlated with their functional impact. OPTIMA Ophthalmic Image Analysis aims at substantially advancing image analysis and computational modelling in retinal disease, based on clinical data at unprecedented scale. In our current analysis dataset we have established automatic methods on data from neovascular age-related macular degeneration and retinal vein occlusion. A large dataset on diabetic macular edema, including spectral-domain optical coherence tomography, will be available for a PhD student to complement our data and to extend our analyses on this very important disease. An individualized treatment guideline from imaging data for anti-vegf treatment could be established even for particular agents for individual subgroups by the PhD student. Image based morphologic evaluation in anti-vegf therapy of diabetic macular edema comprising 1. Automatic microstructural feature segmentation 2. Co-registration of multimodal 3D and 2D image data for point-to-point morphologic analyses 3. Population-wide disease models with monitoring of pathomorphological changes over time for identification of subpopulations in disease morphology and treatment response 4. Evaluation for prediction of visual acuity and anatomic outcomes 5. Development of individualized DME anti-vegf treatment guidelines with innovative PRNmanagement strategies Section XXV 3/5
