3 Foreword Chair of the Area Planning Board NW APB Sbstance Misse Annal Report I am pleased to present the first annal report on behalf of the North Wales Area Planning Board (APB) for the 2014/15 financial year. It has been a year where mch work has been ndertaken and represents real progress for both the strategic and delivery partnerships. Strong fondations have been established for ftre APB operation and the achievement of improved services and poplation otcomes. A fndamental baseline has been set, based on an initial assessment of poplation need. Following this, mch work has been done to establish an effective platform for the commissioning of sbstance misse services across North Wales, together with effective performance management and accontability strctres. It will be important that we contine to bild pon this work going forward, strengthening or collaboration and ensring strong partnership governance and assrance. The delivery grop nder the leadership of Temporary Assistant Chief Constable Simon Shaw has grown in both size and statre and is providing an effective engagement strctre for taking forward its work programme. The grop provides a positive model for collaborative working across stattory and third sector partners and I wold like to acknowledge the contribtion of or volntary sector organisations in or progress this year. A clear strength is the wealth of service ser involvement in North Wales and I wold like to thank each and every individal involved in the continos strive for qality of sbstance misse provision across North Wales. It is down to those individals commitment and determination that enables service sers to have a voice not jst in relation to their own treatment bt in shaping services for the ftre. This year we have also started to frther strengthen or approach to prevention and early intervention, notably in relation to the challenges presented from alcohol misse. Going forward it will also be essential for s to actively engage with and listen to or local commnities, sing their knowledge and assets to shape or services, together, to improve health and well-being. I wold also like to thank the sbstantive APB commissioning and development team, who have been formed dring this period and provide evidence as to how we can provide spport, collaboratively across pblic services, in spport of a common prpose. Finally, I recognise that we have set pon a long jorney bt 14/15 has marked some real milestones for or APB partnership and has set a sbstantial degree of momentm. I have no dobt that there will be ftre challenges bt as a partnership we have a strong ambition and will endeavor to make best se of the opportnities available, working together to redce the devastating impact of sbstance misse. 4 Introdction - NW APB Regional Commissioning & Development Manager Vicky Jones NW APB Sbstance Misse Annal Report North Wales Area Planning Board is the partnership body responsible for spporting the planning, commissioning and performance management of sbstance misse services at a Regional level. North Wales Area Planning Board covers the Betsi Cadwaladr University Health Board and North Wales Police area, which incldes the six local athority areas of Conwy, Denbighshire, Flintshire, Gwynedd, Wrexham and Ynys Mon. Tackling sbstance misse activities in the region are nderpinned by the Welsh Government s 10 year strategy, Working Together to Redce Harm and associated Delivery Plan for 2013 to 2016 which otlines the priority areas for action across Wales. It has meant an end to transitional arrangements and mch work has been ndertaken to progress nderpinning principles to achieve work towards NWAPB s vision: Working together we will seek to make North Wales a safer and healthier place to live, work and visit by preventing and redcing the impact that sbstance misse has in or commnities and by promoting recovery and providing the best service possible to help those in need The following report otlines key progress and achievements dring the period The for priority action areas covered in the strategy are: Preventing harm. Spport for sbstance missers to improve their health and aid and maintain recovery. Spporting and protecting families. Tackling availability and protecting individals and commnities via enforcement activity. Effectively tackling the misse of drgs, alcohol and other sbstances is a key priority for organisations, providers, individals and commnities across the North Wales Region and for Wales as a whole. 14/15 has been a year where the APB has become flly established and fnctional as a regional entity. 2

5 APB STRATEGIC PROGRESS NW APB Sbstance Misse Annal Report Governance Arrangements The North Wales Area Planning Board for Sbstance Misse is reqired to have a robst set of corporate governance strctres in place in order to meet its obligations nder the National Core Standards for Sbstance Misse. Dring the 14/15 period a corporate governance framework has been developed and agreed. De to the partnership natre of the APB this has been a complex ndertaken, bt is of key importance to ensre that the APB can effectively operate and do bsiness. The Corporate Governance Framework provides a clear Memorandm of Understanding and Terms of reference; it also otlines comprehensive reporting and accontability arrangements (both local and regional) and a financial risk sharing agreement between the APB stattory partners. Alongside the Corporate Governance Framework a scheme of delegation has been formlated to ensre that decisions on fnding can be expedited efficiently. The sbstantive APB Commissioning and Development Team were formally established on 2nd Febrary 15 and are of key importance in ensring clear governance and progressing the work of the APB. Commissioning Strategy and Strategic Direction The Commissioning Strategy is an overarching docment setting ot strategic priorities and intended direction of travel for the commissioning of services, to meet the need of the identified poplation. Within 14/15 the NWAPB Commissioning Strategy has been developed. As part of this work a large scale sbstance misse priority setting engagement and consltation was ndertaken across the region. This involved commissioners, service providers and service sers and has informed the APBs strategic direction for the next 3 to 5 year period. For APB themes were identified: 3

Improve how we Work Together: by promoting a philosophy of joined p working and seamless provision, effective commnication and convergence of plans and planning processes. 2. Improve Eqity of services appropriate to identified needs: to ensre Provision of access to appropriate level of service irrespective of who yo are or where yo live, availability of treatment options across the recovery jorney regardless of sbstance being sed and an Increased focs on alcohol 3. To Promote a Sstainable Recovery Jorney: to ensre treatment and spport services are working towards recovery, to increase aftercare options and bild recovery capital and commnities 4. Adopt an approach that considers the whole family: to promote a holistic approach considering the needs of the whole family and significant others 5. Provide for the needs of Yong People: Increased focs pon prevention and early intervention to prevent the yong people of today becoming problematic sbstance missers of the ftre. 6. Redce the Impact of sbstance Misse on Commnities: inclding impact of sbstance misse related offending and crime. The former represent the nderpinning principles of APB work. In addition to the above, an extensive mapping exercise of the existing commissioned Sbstance Misse Action Fnd (SMAF) services against national service models and, of key importance against APB identified priorities, has been ndertaken. This has involved the rationalisation and organisation of all SMAF fnded projects into clear areas, reflective of national models and regional priorities and developed a coherent regional pictre. 7 NW APB Sbstance Misse Annal Report Whilst this gives a broad overview of profile of expenditre it shold be noted that there are many cross ctting elements so it s relatively simplistic. It is hoped that this represents a benchmark for development over time. The exercise has also led to the development of two diagrammatic representations of the SMAF and Health board fnded adlt and children and yong people s services across North Wales. All of this provides the basis for development of APB commissioning over the next 3 to 5 year period and is a strong fondation for progress. Figre 2 Adlt Mapping 5

Performance information is sed by the APB to ensre that services are meeting the needs of services sers and as an assessment of the extent to which key otcomes are being achieved. Since September 14, the APB Commissioning and Development team, with service providers, have invested a considerable amont of time to resolve some apparent performance isses against Welsh Government KPI s. Althogh, this work is on-going, most recent data demonstrates a marked improvement in reporting and performance and is mch more reflective of the activity within services and provides a higher level of confidence in the figres. The smmary headlines for 14/15 are as follows: KPI1 Do Not Attend Post Assessment - measres the level of how well a service has engaged with a service ser and is abot retention rates. 14/15 Post assessment rate is at 19.5% this has reminded stable in comparison to the 13/14 figre of 19.6% and is rated as GREEN. KPI2 Waiting Times from Referral to Treatment - measres the timeliness of access to treatment and spport. The percentage of individals accessing treatment from referral to assessment within 20 working days is 85%, as compared to 80% in 13/14, and is rated as GREEN. 6

9 NW APB Sbstance Misse Annal Report KPI3 Redction in Sbstance Misse - measres whether sbstance misse has redced as a reslt of treatment and spport The percentage of Treatment Otcome Profiles (TOP s) that recorded a decrease in problematic sbstance se is 66.5% as compared to 62.2% in 13/14, and is classified as AMBER. KPI4 Improvement in Qality of Life - measres whether qality of life has improved as a reslt of treatment The percentage of TOP s that recorded an improved qality of life is 74.5% and is classified as GREEN, this is a significant improvement on the 13/14 rate of 48.7% KPI5 Planned Case Closres - measres sccessfl treatment completions The percentage of cases closed as treatment complete is 72% and is classified as AMBER, however, is a significant improvement on 13/14 performance of 58.2 %. In addition to KPI s, each APB is reqired to provide a self assessment against the Welsh Government (WG) dashboard performance information. There are 3 parts to the assessment as follows: 1. APB Governance Arrangements 2. Progress against WG Strategy delivery plan 3. Implementation of Core Standards The reqired performance reporting is provided on a qarterly basis the most recent having been sbmitted in Janary. The following table provides a comparative smmary of performance from May 14 to Jan 15. The table shows very positive performance and significant improvement over the year with the majority of areas being assessed as Green stats. The Welsh Government Review carried ot dring this period also provided positive feedback. 10 NW APB Sbstance Misse Annal Report Finance and Contracts At the end of the financial year the APB are pleased to report 4,924,557 expenditre against Sbstance misse Action Fnd (SMAF) Revene Grant of 4,924,576. Despite considerable slippage having been accred de to delayed start of new services/ projects, nder spend has been identified early and reinvested in short term initiatives which has added considerable vale. Consistent Processes have been established for the allocation of slippage monies and Capital proposals, these have worked very effectively dring the period and will contine to be tilised. A nmber of sbstantial Capital projects have been managed and all achieved planned, fll or phased, completion before the end of the financial year. These inclde sbstantial work to the Dewi Sant Centre in Rhyl to increase temporary spported accommodation and remodelling of Rhyl sbstance misse service accommodation to better meet needs of the client grop and increase capacity. A fll Estates strategy is nderway to ensre Capital monies can be directed effectively and based on clear need in ftre. This will be completed early in 15/16 period. With regard North Wales Contracts an extensive rationalisation has taken place which ensres increased regional alignment. The APB contines to review commissioned services to ensre the eqitable delivery of, and access to, services across North Wales in line with the commissioning strategy. 8

11 APB DELIVERY PROGRESS NW APB Sbstance Misse Annal Report The prpose of the North Wales Area Planning Board Delivery Grop is to oversee and manage the delivery of the APB work programme; inclding, bt not limited to, the assessment of need; planning, design and development of services; commissioning; performance management; qality assrance, and governance. The grop brings together all those who have the capacity to contribte to, or advise on, the delivery of these fnctions. The delivery grop is crrently made p of 3 sb grops, Children and Yong People sb grop, Service provider form and the North Wales Service User form, which is independent of the APB. APB Delivery Grop Overview from Chair seamless set of services for North Wales and provide a form for information and good practice exchange. Service ser participation has had mch greater focs and we are working effectively with the North Service User form to make a difference to how services operate on the grond. It is of vital importance that we listen to or service sers to ensre they are satisfied with the provision they receive and that service ser satisfaction is measred in the regional performance management framework that we are crrently developing. 2014/15 has marked a progressive year for the APB delivery grop, admittedly we have come from a standing start bt the new APB commissioning and development team have gathered some real momentm to or work and provide a dedicated resorce to drive the delivery grop work programme. I wold like to thank the delivery grop members and sb grop members for their contined spport and engagement as this has provided a strong partnership to ensre we can tackle or demanding agenda. The delivery grop contines to grow and has evolved and matred into what I believe is an effective mechanism for tackling some key isses. These isses are not jst in relation to the work programme we have set bt inclde emergent isses sch as Novel Psychoactive Sbstances (NPS), leading to dynamic joined p action. The delivery grop is now in a place where it can create a We have had great sccess in pshing the recovery agenda as this marks one of the key priorities as an APB. We are developing Mtal Aid facilitation programmes to ensre people have access to Mtal Aid grops sch as Alcoholics Anonymos (AA), Narcotics Anonymos (NA) and SMART Recovery as we know that there is a large and growing evidence base to show they work in not only helping people into recovery bt helping them sstain their recovery which is crcial. We are working hard to break down the barriers and stigma associated with sbstance misse as people can and do recover and we need to make this more visible. 2014/15 has been a challenging year bt one which has seen s make real progress in delivering against the APB strategy and making a difference. 12 Children and Yong People s Sb-Grop Overview from Chair NW APB Sbstance Misse Annal Report It is seen as a positive step that Children and Yong People have been recognised as a key grop on the North Wales Area Planning Board. Significant work has been ndertaken over the last 12 months to coordinate services involved with Children and Yong People and sbstance misse across the region, clminating in a sccessfl conference which has enabled s to look towards a more consistent and standardised approach to service delivery regardless of provider. Following on from this is the development of a best practice form to bild on both regional and national work sch as the otcome measrement tool for CYP. Positive momentm has been created amongst service providers working together in this field which can only benefit children and yong people in the ftre. Lesley Bayley Chair of the North Wales Children and Yong Peoples Sb Grop Child and Adolescent Mental Health Services/ Betsi Cadwaladr University Health Board Service Provider Form Overview from Chair Following the introdction of the North Wales Area Planning Board, and the creation of it s Sb Grops, the newly formed Providers Form was establish in In the early days the form operated consistently, working alongside APB to help shape and define the Vision and Priorities for effective Sbstance Misse Treatment in North Wales. Sb grop members were actively conslted and contribted to a series of consltation activities, that dring 2014/15 clminated in a strong Area Planning Board vision for effect services. Spported by a strong set of defined area priorities and a spport strctre (the newly formed Regional Sbstance Misse Commissioning Team) this has enabled the Providers Form to consolidate and clarify its role as a valable sb grop to the APB for the coming years and new Welsh Government Strategy for the coming years. The Providers Form is now a strong grop of nominated individals representative of senior and operational managers, across a broad range of providers in North Wales. Ranging from third sector providers working directly with individals whose lives are affected by drgs or alcohol, to those with a specialist niche market in both volntary, pbic and commercial sectors. This mix ensres that the form is able to properly represent a range of affected grops; spporting not only those affected by Sbstance Use, bt those with a plethora of related isses that affect the health, wellbeing and qality of life for everyone across North Wales. In early 2015/16, the grop set ot to: To advise and gide to APB; ensring that it s priorities are representative if the experiences faced in North Wales Services on the grond! To help shape services and ensre that the vision of the APB is embedded in the qality and delivery of front line operations and that Welsh Government Gidance is translated properly into delivery; inclding the delivery of Harm Redction and Recovery across all aspects of provision To inflence, edcate and spport wider stakeholders abot the work of the APB and local services; ensring maximm by-in to the priorities and identifying new and innovative opportnity for collaboration across a range of sectors and stakeholders This has been a constant jorney for the grop, bt we are now ready to spport the APB and qality delivery into 2016 and beyond! The contribtions, shared debate, discssion and innovation has added real vale to the work of all providers; ensring that we are all collectively able to make a difference in North Wales and maximised each other s strengths. This can only be a strong step forward, enabling s to collectively deliver a more cohesive, integrated range of services that enable s to effect real change in the lives of individals and commnities across North Wales. 13 Service User Form Overview from Chair NW APB Sbstance Misse Annal Report North Wales Service User Form is respected as a professional organisation, collecting the views of service sers, and acting as a condit for information with the Area Planning Board. Membership and meeting attendance contines to grow, with a broad range of representation. Otreach Meetings in Anglesey have enabled s to engage and develop or links with local agencies and representatives. A new logo has been introdced, a newsletter and Facebook page have been pblished and a website is planned. Individal conty based directories have been prodced - available throgh the Dan 24/7 website - identifying available service ser spport services across the region. Form members recently spported the Pen y Fan Walk, a frther Celebration Event is planned for Agst in Bangor, and active spport will be shown for recovery based events dring September. A new Six Conties Achieving Recovery Federation (SCARF) has been established. Brian Lewis Chair North Wales Service User Form (NWSUF) In addition to the above, plans for a fatal and nonfatal poisonings review grop have commenced in line with WG gidance, this shold become fnctional in early 15/16 and will be able to share good practice and improvement areas to frther prevent drg related deaths. In smmary, the delivery strctres have fnctioned well and led to increased engagement and joint working which is hoped to benefit service sers across North Wales. Commnications An inagral APB newsletter was compiled and circlated widely, feedback on this has been very positive and will act as the main pblic facing commnication tool. Liaison has also taken place with one local MP to discss the best way to provide briefings abot work happening in their areas, this shold also spport pblic engagement. on recovery. Service sers in recovery told their stories to LPS SMT which aided a greater nderstanding of the isses of sbstance misse and how those in recovery can be mobilised to help others follow the same path. Alcohol Harm/ Demand Redction An ambitios North Wales programme to redce the harm associated with alcohol, driving new thinking, learning and change was spported by North Wales Regional Leadership Board on 11th Jly 2014 on the recommendation of the Cross Sector Chief Exectives Form. The project soght to form a Leadership Academy to consider the otcomes that shold be set for an alcohol misse redction programme and how those otcomes cold be achieved. The APB Chair and Vice Chair are leading this programme of work with Project Sponsor, Iwan Davies, Chief exective of Conwy Concil. The following are key milestones achieved within 14/15 period: Pblic sector representatives at Director or Head of Service level from across North Wales were invited to join the academy and contribte to the debate. An Inagral workshop was held on 7th November which broght together a broad spectrm of participants to begin the process. Representation and attendance of participants from key organisation/ sectors has been very positive. The range of organisations represented incldes, Police, Fire Service, Amblance Service, Health Board (clinical and non clinical), Academic Instittions and Local Athority representatives from across North Wales covering Hman Resorces, Edcation, Social Care, Pblic Protection etc The initial workshop focsed on scoping the way forward moving away from traditional models of working to achieve the scale of change reqired. Each participant from the workshop agreed to raise the profile of the work within their own organisations. As part of a wider commnications plan, a presentation was made by APB leads to Local Policing Services Senior Management Team (LPS SMT) to increase commnication and information exchange abot APB bsiness with a specific focs A frther workshop was held on 30th Janary 15 again facilitated by the above project sponsors, this also introdced the grop to work that has been ndertaken in the North West which has been particlarly impactive in 11

CONCERTED CROSS PARTNERSHIP ENFORCEMENT EFFORTS Andrew Jones Chief Spt Jeremy Vaghan Redce Availability and Affordability in a consistent and coherent way across the region. A frther overarching area of work was otlined as the need to IDENTIFY MEASURABLE GOALS to nderpin the sccess of the project and work to be ndertaken. The Project Lead identified is Dr Rob Poole, University of Wales, Bangor. This work is set to contine into 15/16 and beyond. Service User Involvement The NWAPB believes that service sers shold and will be meaningflly involved in the development, delivery and evalation of sbstance misse policy and practice at every level across North Wales. Service sers have niqe experiences, skills and abilities that enable them to provide expert advice in this field. North Wales APB commissioned a review of existing service ser provision and involvement across North Wales, to benchmark service ser involvement in North Wales against best practice and to develop a charter for service ser involvement as part of a robst involvement infrastrctre. The aim of the work programme is to ensre that service sers are meaningflly involved in the development, delivery and evalation of sbstance misse policy and practice at every level across North Wales with confidence. The methodology employed was wide ranging consltation with service sers, providers and commissioners from across North Wales via a qestionnaire, a nmber of focs grops and a co prodction event. 12

15 NW APB Sbstance Misse Annal Report The recommendations (drawn from the consltation) were as follows: Development of a Service User Charter. APB to consider commissioning an overarching service se involvement service to develop and monitor service ser involvement and implement the proposed service ser charter across North Wales. APB ensre that service ser involvement has a regional platform and framework within which to be planned, commissioned and monitored. This will inclde appropriate representation at the Exec, Delivery and sb grops. North Wales Service User Form shold be regarded as a central featre of the service ser involvement strctre. It shold be properly constitted and develop terms of reference which shold ensre regional membership and representation. It shold be adeqately fnded and staffed. The development and maintenance of service ser spport grops across the region, appropriate to the needs of each area, providing feed into NWSUF. This wold need to ensre shared leadership and work ethic, encoraging more people to become actively involved. A regional service ser training and spport strategy. These recommendations need to be assessed in relation to the cost and logistical implications of ensring that mainstream services are available to Welsh and English speakers. All of these recommendations have been considered in detail and action plans are well nderway to implement. Recovery and Mtal Aid Anonymos (AA), Narcotics Anonymos (NA) and SMART recovery, opening p options and choice. There is a strong National evidence base for Mtal aid and local facilitation projects have been particlarly effective in spporting people in to sstained recovery with over 50% of those engaging with the programmes sccessflly entering recovery. Work will contine into the 15/16 period. Other Service Developments Increased Alcohol services are now available in District General Hospitals to ensre people presenting or admitted to hospital as a reslt of alcohol related incidents and illness can be effectively identified and followed p as appropriate. This shold assist earlier intervention with alcohol sers and aims to redce the demand that alcohol places pon pblic services. It also targets a wide section of alcohol sers whom otherwise may not be identified and treated. Similarly increased specialist provision for Children and Yong People admitted or presenting to hospital as a reslt of sbstance misse and self harming behaviors which aims to identify and spport vlnerable yong people whom otherwise may not access services and remain nspported. Workforce Development/ Training Established relationships with service providers and service ser representatives across North Wales ensres that workforce development and training needs can be identified and effectively met. This year de to the emerging se of Novel Psychoactive sbstances (NPS), a series of training events were commissioned for practitioners and service sers to spport an effective response to NPS. Feedback from the training has been very positive and it is hoped that training for commissioners can be offered in addition. Ensring sstainable recovery jorneys for sbstance missers is identified as a key priority for North Wales APB. Dring this period a nmber of recovery events have been held to promote recovery and in particlar the key role of peer spport. Several Mtal Aid facilitation projects have been rn over the year ensring that people can prse active recovery throgh Mtal aid grops sch as Alcoholics 13

16 Looking Forward to 15/16 NW APB Sbstance Misse Annal Report Strategically: Work contines to ensre the APB is cited within the integrated regional and local planning mechanisms originating from the Safer Commnities Board. To prse the joined p commissioning of sbstance misse services, given the different fnding streams available to ensre dplication is avoided and economies of scale can be made. Maintain the progress from 14/15 with KPI performance. Development of an APB performance management framework to ensre that the APB are setting clear performance measres to assess themselves against in addition to the national KPI s Mapping and gap analysis are set to contine into the new financial year and a contract review methodology will be formlated to ensre that each commissioned services can be assessed to ensre it remains relevant.
