Consider the following: What is the program, initiative, or activity? Who (i.e. local health department, local and national partners, etc.) is involved in developing, supporting, or leading the program, initiative or activity? Where is it taking place? Why is this relevant to your local community/mrc unit? (Maximum 300 words): * River City Ministry of Pulaski County (RCM) has provided charitable medical, dental, and vision clinic services to thousands of the homeless and impoverished in Pulaski County for many years. Once RCM's MRC unit became an extension of our charitable clinic, we began to brainstorm ideas based on recommendations from information gathered through the MRC on how to improve our clinic and MRC unit. As RCM seeks to treat the whole person (body, mind, and spirit), RCM desires to work with MRC volunteers and partners in the community to increase health equity for those being served at our clinics. The homeless and poor population don't typically receive routine medical care, rather medical care that only occurs when a major medical need presents. Due to the nature of the transient lifestyle, RCM has identified a struggle in getting the homeless population to show up and keep appointments. As a result, many individuals don't have the same information about health maintenance, disease prevention, and other important health topics that are normally presented as routine medical care is given. We have identified that the most effective way to increase health equity in our community is by bringing educational information and specialized service days to our ministry. After talking to health professionals, patients, and other professionals providing similar services to the poor, we have identified the following four main health components where we want to see an increase in health equity for our target community: oral health, healthy eating, HIV testing and education, and behavioral health initiatives. It is our desire to put out a quarterly newsletter that highlights one of the listed components each quarter. We plan to utilize MRC volunteers, community partners such as the local health department, and local college medical professional students (mainly dietetic and dental students) to provide additional education and resources. Describe the goals of your project. What are the measurable objectives for your project? (Maximum 300 words): * The overarching goal of our project is to increase the health equity in our community. The two main objectives of our project are centered around providing information about specific health concerns on a quarterly basis including HIV education/prevention, healthy eating education/resources, oral health education, and behavioral health initiatives. River City Ministry will publish a quarterly newsletter focusing on one health component each quarter. In addition, RCM will utilize MRC volunteers, community partners, and MRC staff to provide onsite counseling regarding the above topics. To coincide with each of the four health initiatives, RCM plans to hold a specialized service day focused on each health component that will be publicized and open to the community. The specialized service day will provide medical services including HIV testing, dental screenings and care, healthy eating counseling, and access to behavioral health professionals. It is our hope that the specialized service days can turn into long-term care.
4 Who is the target audience for your project? How are the MRC Volunteers involved? (Maximum 200 words): * The target audience for this project are those already being seen through our charitable clinics as well as the unreached homeless and impoverished adult population in our community. MRC Volunteers are crucial to the operation of this project. Many health care MRC volunteers provide knowledge for the quarterly newsletter along with specialized services for the service days. Describe the benefits and impacts your program, initiative, or activity will have on your community and/or participants. What are the expected measurable outcomes? (Maximum 200 words): * The main premise for our project focuses on health equity. In our scope of services provided and our targeted population, many homeless and impoverished individuals lack quality, consistent healthcare in conjunction with health education information. It is our goal that with this program, we can start increasing health equity for members in our community in all areas. We also observe that many homeless and poor individuals lack the stability to keep and maintain medical appointments. We hope that by providing education through a newsletter and walk-in specialized service days, that new relationships can form with individuals in the community and bring about change in the individuals served. As people are treated and health equity increases, we hope to see a positive change in people's lives. Describe what makes your program, initiative, or activity innovative. (Maximum 200 words) : * Access to good healthcare is one of major the hot topic items surging around our country. As we seek to continue providing charitable services through our clinic, we want to continue building community and improving the lives of those we serve. Providing quarterly newsletters and having specific days set aside can serve as building blocks to the bigger picture of our mission. As our country grapples with how to unite the people, we seek to do that by providing education, building community by opening up our services and seeking to treat the whole person. Work Plan: Implementation plans: Describe how the project will be carried out and how the MRC Volunteers are engaged in the process. You should list any specific steps you will take. (Maximum 200 words) : * River City Ministry has identified a project manager who will serve as the anchor for this project. The project manager will contact different MRC volunteer health professionals (dentists, doctors, pharmacists, and behavioral health professionals) to gain information to publish the quarterly newsletter. The project manager will be responsible for publicizing and organizing the specialized service days. This will involve working with community partners and MRC volunteers willing to work onsite and provide services based on the health component for that quarter. The project manager will also be responsible for coordinating advertising for the service day and outreach to the community. List your project timeline. (Maximum 200 words): * 1. Quarter 1: January - March 2017 will focus on oral health care. 2. Quarter 2: April - June 2017 will focus on healthy eating. 3. Quarter 3: July - September 2017 will focus on HIV education and testing. Quarter 4: October - December 2017 will focus on behavioral health initiatives. What are your sustainability plans for your project? (Maximum 200 words): * This is the first time that RCM has undertaken a project of this scope to increase health equity in our community. We hope that this year will only lead to greater partnerships with the community and professionals to bring about more health education and services to increase efficacy in the lives of those we serve. After the completion of this first initiative, we plan to evaluate and identify areas of improvement for future reference. We plan to continue publishing a quarterly newsletter that will identify health concerns/initiatives for our community and work with partners on how to provide education and services to bring education and change in the identified areas. Budget: Your project budget should include detailed line items of your estimated program costs up to $13,000. Administrative costs should be minimized to the extent possible. NACCHO s MRC Award funding may not be used to purchase promotional items/giveaways or food and beverages.

Professional Service Fees: $844.33/month to provide for the payment of the project coordinator to organize, compose, and publish the quarterly newsletter; to plan specialized service days by reaching out to the target population to publicize, reach out to community partners for professional assistance, recruiting MRC volunteers, and completing tasks needed in order to make the service days a success. River City Ministry has many MRC volunteers that are medically licensed and trained to help with the health components of this project. However, River City Ministry is in great need of a project coordinator that can put in a good amount of work to make this project a success. 2. HIV testing $2,868 - River City Ministry will need to buy the HIV testing kits in order to provide the specialized service day focused on HIV education and prevention. Evaluation: Describe how you will obtain and report data, as well as measure your project's success and community impact. Consider: What evaluation tools will you be implementing to measure success? How will success be defined for this project? ( Maximum 200 words): * River City Ministry will track how many newsletters are given out each quarter and how many individuals attend service days. After the year is complete, RCM will also review how many new medical patients are being seen through our charitable clinics. While numbers will definitely play a role in the success of this project, we will also be evaluating interest and effectiveness by surveying clients receiving services to gauge the community's response to our efforts.
