4 CHAIRMAN S INTRODUCTION As Chair of MQI I am pleased to welcome our Aual Report for Despite sigs of ecoomic recovery i the coutry, at MQI we cotiue to witess sigificat icreases i homelessess, log term addictio ad premature deaths amogst our cliet base. The sad ad tragic death of Joatha Corrie was a catalyst for substatial state ad public egagemet with our sector. At MQI we were pleased to exted our Ope Access service to ear 24/7 through the opeig of our Night Café, providig refuge for those homeless who, for whatever reaso, caot access hostels. The fact that the Café has operated at full capacity from commecemet cofirms the ecessity for such a service. However, the statistics aroud fatalities amogst drug users are a atioal disgrace. I the eight year period from 2004 to 2011 there were a total of 4,606 deaths by drug poisoig ad deaths amogst drug users. I 2011 aloe the umber of deaths by poisoig, which were recorded, rose to 365. At MQI we cotiue to raise awareess of the awful cosequeces of overdose through atioal ad iteratioal cofereces. We will cotiue to lobby for a cocerted strategic state respose to this crisis. MQI has, for the past few years, champioed easy access for drug users ad their families to Naloxoe ad we welcome the pilot project which will provide Naloxoe free of charge o a take home basis. Naloxoe is a atidote used to reverse the effects of opioid drugs like heroi, morphie ad methadoe if someoe overdoses. Last year we committed to achievig the highest stadards of trasparecy ad efficiecy at MQI. I this cotext, I am pleased to cofirm our accouts are preseted to SORP stadard (see ote i Aual accouts) ad we are makig sigificat progress towards sigig up to the Sector s Code of Goverace. Our doors have cotiued to respod with icredible geerosity to MQI. To all our doors statutory, corporate ad persoal we say a heartfelt thak you. 5 LOOKING BACK: 2014 As he passed i MQI s doorway, I almost forgot to say hello. All I could thik about were his shoes. They had holes i the toes ad his feet poked out through threadbare socks. He must ve bee walkig forever. But here at Riverbak Cetre he had fially foud somewhere to rest. Ad I greeted him with the same welcome we exted to everyoe at our doors. This ma was just aother of the evergrowig list that preseted at our cetres i It was a busy ad challegig year for our drugs ad homeless services. There was much talk i the public domai about the ew shoots of recovery i the ecoomy ad Irelad turig the corer from recessio. But this was a log way from the experieces of our cliets. I fact, it is the cumulative effect of the previous five ad more years of austerity ad cut-backs i services that are ow maifestig i what ca oly be characterised as a housig ad homeless crisis. The crisis has emerged i the absece of ay sigificat ew social housig uits beig built. Ad the collapse of the private retal market due to risig rets has exacerbated this. The result is that emergecy homeless accommodatio has become bottle-ecked, leadig to more people beig stuck o the streets ad rough sleepig. The cotiued icrease i families becomig homeless is aother huge cocer. While families with childre are rightly prioritised i access to emergecy accommodatio, the kock-o effect is that sigle people are left behid. Especially those with issues such as metal health problems ad addictio. It is these people that we try to focus o assistig, ad over the course of 2014 we saw almost 4,000 idividuals i our homeless day services. Providig a healthy meal for homeless people is a importat aspect of our service ad i 2014 we provided over 79,500 meals. We also provide crisis support ad i 2014 we recorded 5,623 supportive itervetios with users of our homeless service. The strog lik betwee homelessess, ill health ad the harsh reality of life o the streets is clearly evideced by the icreased uptake i our primary health care services. Our GP, cousellig, ursig ad detal services recordig a total of 5,329 health care itervetios durig the year, a icrease of 19% compared to The drugs situatio remais a very serious ad live issue. I 2014 demad for our drug treatmet services, both i Dubli ad aroud the coutry, remaied cosistetly high. Durig 2014 we provided 24,266 eedle exchage itervetios, which is a 6% icrease o I total 3,179 idividuals accessed the eedle exchage, of which 527 were ew first time preseters to the service. Similarly, our residetial detox ad rehabilitatio programmes also remaied i high demad with a sigificat icrease i referrals to all of these services. Our medically supervised detox service at St. Fracis Farm had 64 admissios, 50 male ad 14 female, over the course of the year. 69% successfully completed the detox regime. The detox is a atioal service ad the admissios were draw from ietee differet couties across the coutry. Both of our residetial rehabilitatio programmes, High Park ad St. Fracis Farm, were equally busy durig 2014 with 43 ad 46 admissios respectively. Agai, these admissios were draw from more tha te couties across the coutry. I reflectig o the year it is also hugely importat to recogise the positive gais ad advaces that have bee made. Notwithstadig the declie i statutory icome over the last umber of years, we have maaged to maitai our core services ad ideed develop ew iitiatives to address emergig eeds. This is due to the hugely geerous support of our doors: cocered idividuals, compaies ad grat makig trusts. They share our visio of a better society where problem drug use is miimised ad homelessess elimiated. The issues of drugs ad homelessess ca be addressed. This is demostrated to us agai ad agai by the may service users who, whe they receive the support they eed, ca ad do move o successfully with their lives. Ufortuately 2014 eded with the tragic Lord Mayor of Dubli, Mr Christy Burke, ad Toy Geoghega MQI s CEO at the lauch of last year s Aual Review. death of a homeless ma sleepig rough o the streets ear the gates of Dáil Eirea. This tragic occurrece caused huge public outrage ad the Govermet respoded by committig additioal resources ad commissioig ew emergecy homeless beds. This demostrates that resources ca be foud ad progress made whe there is real political leadership ad commitmet to address the matter. The curret drugs problem ad the crisis i homelessess have bee bor out of the ecoomic crisis, ad the lack of ivestmet i drug services ad public ad social housig. As a society we caot afford to make the same mistakes agai. The issues of drugs ad homelessess are ot isurmoutable ad ca be addressed. 8 ADDRESSING CRISIS OPEN ACCESS SERVICES OPEN ACCESS HOMELESS SERVICES The ew Riverbak buildig o Merchats Quay cotiues to be a resoudig success. With the ehaced space we are able to cater for the icreasig umbers of people comig to our homeless services with complex eeds. Over the course of the year we provided more tha 79,636 meals for homeless people across our day ad eveig services. We cotiue to offer meals at our Day Service ad at our Exteded Day (Eveig) Service, which is operated i cojuctio with Focus Irelad. THE DROP-IN SERVICE FOR HOMELESS PEOPLE (FÁILTIÚ) The aim of the Drop-i Service at Riverbak is to provide a oe stop shop offerig a rage of iterliked services. These services miimise harm associated with life o the streets ad offer clear pathways towards settlemet ad reitegratio. I 2014, our Drop-i Service was ope Moday to Friday from 7:30am to 4:30pm ad o Suday from 7:15am to 1:30pm. The services we provide for homeless people from Riverbak are as follows: NEW IN 2014 Cliet Care ad Case Maagemet CRM System: The cloud based CRM system implemetatio project was completed i December With effect from Jauary 2015, MQI ow use a state of the art cliet maagemet system. This database allows us to provide the optimum support to cliets i lie with the HSE s NDRIC (Natioal Drugs Rehabilitatio Implemetatio Committee) protocols o iter-agecy workig. I lie with iteratioal research evidece o Homeless Services betwee 2010 ad 2014 Meals Exteded Day Serivces Support Itervetios Primary Health Care recovery, this system eables us to maximize outcomes ad esure cotiuity of care for service users. Iformatio, Advice ad Crisis Support Service: The Drop-i Service icludes assessmet of each service users eeds. It offers advice ad iformatio o health, social welfare, emergecy accommodatio, log-term housig ad other issues. I 2014, we recorded 5,623 supportive itervetios with service users at Riverbak. Itervetios iclude key workig sessios, referrals to emergecy accommodatio, social work services, medical services ad social welfare services. It also icludes help i cotactig frieds or family, access to drug treatmet ad support i a variety of other matters. The service supports cliets who have bee sleepig rough o the streets ad esures that they fid ad maitai a place to live. Project workers help homeless people from a variety of settigs - rough sleepig, hostels, B&B s, short-term arragemets with frieds/ families ad trasitioal ad supported accommodatio. Our staff aims to assist those who are homeless to lik i with short term crisis accommodatio, with the evetual goal of helpig cliets ito more stable log term housig ad idepedet livig. 9 Meals Service: The Day Service at Riverbak cetre offers homeless people two meals a day -breakfast ad luch. A total of 63,279 meals were provided by the Day Service i The Exteded Day Service, operatig from 5:30 to 8:30pm, provided a additioal 16,357 eveig meals. Suday remaied our busiest day with more tha 350 people attedig for breakfast or luch each Suday. MY YEAR AS MQI MENTAL HEALTH NURSE: DEREK LOOKS BACK Late i 2013 MQI supporters rallied to hire a 100% door fuded metal health urse the first positio of its kid i Irelad to brig hope to people who are aloe o the streets ad sufferig from metal illess. Oe year i, Derek looks back... PRIMARY HEALTHCARE SERVICES MQI is workig i partership with the HSE, the Dubli Regioal Homeless Executive ad SafetyNet to provide a primary health care service for homeless people. The service has Geeral Practitioers, a Detist, a Nurse ad a Cousellor. I total there were 5,329 health care itervetios durig the year (a icrease of 19% compared to 2013). Nursig: There were a total of 1,804 ursig itervetios i 2014, or 150 per moth (a icrease of 10% compared to 2013). The urse s drop-i cliic provides a full rage of primary health care services such as woud care maagemet, blood testig, sexual health, medicatio maagemet ad geder specific health issues. Metal health itervetios rage from acutely suicidal cliets to those who have lost touch with other state ad agecy metal health services. Referrals to tertiary services such as A&E ad other hospital services are a large part of the ursig role. The atiowide SafetyNet Primary Health Care Service, of which the MQI medical uit is a part of, is still developig ad provides for cosistecy ad cotiuity of care betwee those homeless services ivolved. Cousellig: We provided 811 cousellig sessios i 2014 (a icrease of 3% compared to 2013). Homelessess is ofte associated with severe stresses ad difficulties i life, ad it s ot surprisig that may people fid it hard to cope. The Cousellig Service for homeless people No State fudig existed for the pilot programme. But amog Irelad s homeless there was a cryig eed for help aroud metal illess. So whe MQI supporters leared of a icredible gift to cover half of a ew metal health urse positio, they came through with flyig colours to provide the rest. Just six days ito 2014, Derek MQI s first metal health urse joied the staff. With early a decade of experiece i acute metal health i both hospital ad commuity settigs, Derek hit the groud ruig at MQI s Riverbak homeless cetre. It s a good thig too: he s see cliets sice the day he started. Multifaceted problem The uderlyig problem, he explais, is multifaceted. People ca fall ito homelessess because of their metal health. Or the drugs they use ca make them more susceptible to metal illess i the first place. To make matters worse they lack professioal supports, i that they are barred access to may places. With me here it s hoped the umber of people who to go to hospital will go dow, ad that will ease pressure o HSE beds. I do educatio for our staff as well, so they kow the warig sigs. A silet stigma It is MQI s ope door that makes the differece, Derek says. I ve see people here who have just vaished from other Eve with the govermet cutbacks ad people more reliat o services tha ever, I m still shocked by the sheer umbers of homeless people that are comig through our doors ad by how willig they are to egage with me. Derek, MQI s first metal health urse services. I m seeig people who are suicidal or have plas to self-harm. They have lost their family, their house. They could have bee goig through this six moths. Other tha staff here obody has spoke to them. Quite a few people break dow the first time I ask, How are you? It s such a relief to kow it s ot hopeless, that somebody is listeig that maybe there is somethig they ca do. Because of our doors that support is ow here. 10 ADDRESSING CRISIS OPEN ACCESS SERVICES, cotiued works at two levels. Firstly, providig a brief crisis cousellig service targeted at service users i distress. Ad secodly, offerig medium to log-term cousellig relatig to issues such as relatioships, bereavemet ad drug use. I additio we provide group support for homeless service users attedig our day programmes. GP Service: Our GP service dealt with 1,985 cosultatios i 2014, or 165 per moth (a icrease of 33% compared to 2013). The service is used by cliets with a variety of acute ad chroic illesses. I 2014, 1,008 uique cliets used the GP service (a icrease of 28% compared to 2013). NEW IN 2014 Psychiatric urse: Recet research shows that as may as 81% of homeless people have a metal health diagosis. I Jauary 2014, Derek Parker, a psychiatric urse, was added to our existig metal health team. The iitial fudig for three years was provided by geerous volutary doatios from MQI s supporters. Derek was quick to establish strog liks with the HSE, foreig embassies ad other homeless ad addictio agecies. Sice commecig our metal health crisis support service i Jauary 2014, 168 cliets have bee assessed by the psychiatric urse due to cocers about their metal health. 141 of the 168 cliets see i 2014 were Irish atioals, with 124 of these from Dubli. 27 foreig atioals used this service i Supportig foreig atioals with a metal health diagosis to access treatmet ad medicatio has bee a area of great success for the team this year. The metal health crisis support ow offered at MQI has resulted i fewer MQI staff member David Sherry greets cliets at the Riverbak Day Cetre referrals beig made to A&E with those that are beig referred beig give psychiatric treatmet. 86 cliets usig the service i 2014 required sigificat crisis itervetio. Of these, 16 were referred for hospital assessmet with 5 cliets requirig admissio. Of the 168 cliets, 93 achieved a positive outcome as a result of their egagemet, 56 disegaged for various reasos icludig stabilizatio of their metal health crisis. Establishig closer workig relatioships betwee the Ope Access service ad maistream psychiatric services has provided sigificat beefits to all ivolved. The Metal Health Service ow has a iitial poit of cotact which is highly beeficial ad provides more effective moitorig, cotiuity of care ad commuicatio. The Metal Health Service has also provided substatial support to staff at Ope Access as well as at Merchats Quay s other services. Staff traiig is a itegral part of the role which has assisted i staff makig more appropriate referrals as well as maagig lower level problems i a more effective maer. Havig this support ad iformatio has icreased staff cofidece ad aided i them assistig cliets to achieve more positive outcomes. EXTENDED DAY SERVICE This service was established i partership with Focus Irelad to meet the eveig time eeds of homeless people ad rough sleepers, ad cotiued throughout Betwee 80 ad 120 people used the service each ight. The Exteded Day Service addresses a sigificat gap i services whereby oe were available to homeless people i the eveig time from 5:30pm to 8:30pm. The service provided 16,357 eveig meals i 2014, alog with crisis support, iformatio, advice ad assistace arragig overight accommodatio. NEW IN 2014 THE NIGHT CAFÉ I December 2014, there was public outcry at the death of a homeless ma ear the Dáil, which led to the coveig of a Homeless Summit by Miisters Ala Kelly TD ad Paudie Coffey TD. As a result of this the govermet commissioed MQI to setup a ew Night Café to support homeless people who are rough sleepig. The MQI Night Café service opeed i Jauary 2015 ad will operate betwee 11pm ad 8am, seve ights per week, caterig for 50 people each ight. The Night Café service is differet from a coffee shop people ca t just walk i the door. Istead, they will be referred to the Night Café by callig the homeless Freephoe or by egagig with the Housig First Outreach Team operatig o the streets at ight. Over the past umber of years MQI received fudig from Pobal, uder the Homeless Measure, for a New Commuities Support Worker to facilitate support for people from the ew EU states who have become homeless i Irelad. Several of our Project workers are fluet i a umber of Easter Europea laguages ad provided a rage of supports icludig advice ad iformatio, referral ad advocacy for these service users. I 2014, MQI provided oe to oe support to a estimated 350 such service users the greatest umber were from Easter Europea coutries (the majority of whom were Polish). For this reaso we are ofte the first place to which drug users tur for help. I 2014 MQI set up a ew fuctio called the Cotacts Service as part of the Ope Access Service to specifically address this eed. The service provides oe to oe support maily with accommodatio but also i relatio to treatmet, traiig, medical, welfare ad legal issues. The Cotacts Service also provides cotiuity of care sometimes it ca take weeks or moths workig with multiple support agecies i order to attai outcomes for cliets. Part of the challege i workig with homeless people or drug users as a support agecy is that cliets do ot preset for appoitmets due to chaotic Educatio/Employmet I.D. The report highlighted the relatively ew pheomeo of Users of Performace ad Image Ehacig Drugs (steroid users) presetig at Irish drug treatmet services. Users of Performace ad Image Ehacig Drugs (PIEDs) are a ew group of ijectig drug users who are at serious risk across a rage of physical, psychological ad behavioural areas. The focus of the preset report was o explorig this emergig tred ad examiig ways i which to address the associated risk. The Research Report worked with a sample of 89 idividuals who were usig Performace ad Image Ehacig Drugs ad were attedig Merchats Quay Irelads Dubli based eedle exchage programme. The report revealed the followig: The sample was all male with a average age of 24 years ad with a age rage of 18 to 40 years. 50% had ever tested for HIV or Hepatitis C ad of those who had tested 10% were Hep C positive. The level of other drug use was high with use i the past moth showig 62% used alcohol, 40% used caabis, 23% used bezodiazepies ad 17% used cocaie. Users reported a rage of egative side effects icludig icreased aggressio (38.2%), sudde mood chages (31.5%), axiety (19.1%), ad depressio (18%). We also ru a Family Support Group which meets every week providig a forum where parets, ad other close relatives ad frieds of drug users are offered support ad advice o a rage of issues. The Family Support Group is liked to the Natioal Family Support Network which offers a opportuity to raise issues at a atioal level. There s truth i that. After my detox ad rehab at St. Fracis Farm I was drug free for the first time i my life sice I was a teeager. Everythig was just raw. Ad I was still homeless without MQI s aftercare I would have had to go back to a hostel. MQI: Was it that way after rehab for you as well, Oisí? Oisí: It was. Had o my heart I would be back to usig drugs if I d goe home. It s whe the drugs are goe out of you that it really starts. MQI operates two short-term trasitioal housig services, oe i Ballymout, Dubli, developed with the support of oe of the orgaisatio s doors, the other i Leixlip, Co Kildare, developed i partership with Respod Housig Associatio. Across both houses we provide a total capacity of 9 supported accommodatio uits. Of the 18 cliets who departed the service durig the year 67% completed the programme. COMMUNITY EMPLOYMENT (CE) SERVICES MQI works i partership with the Departmet of Social Protectio (DSP) to provide hads o traiig for prospective drugs workers, cliets ad post treatmet service users. This is doe through a umber of Commuity Employmet (CE) projects based i our services. CE projects provide participats with skills that eable them to access permaet employmet. I may cases our CE projects have ofte provided service users with their first experiece of paid employmet. I 2014 a total of 144 persos participated i CE placemets at Merchats Quay Irelad. Of those, 42 completed their CE placemet at Merchats Quay durig the course of the year. Of these, 8 secured permaet employmet, 3 retured to educatio ad 3 trasferred oto aother scheme. The service is focused o providig crisis support ad offerig progressio pathways for cliets. The service operates i partership with DSP, HSE, VEC, GP s ad other iter-agecy parters i the midlads regio. As part of the harm reductio ad rehabilitatio process, the Ope Door team have bee workig closely with a group of 144 idividuals, may of whom have bee attedig o a regular basis throughout 2014.This group makes use of the facilities withi the cetre such as food, traiig, computers, washig ad laudry as well as the oe-to-oe cousellig sessios with the project workers. The Ope Door team develop therapeutic relatioships with cliets though a focused o-judgemetal approach to their work. Total expediture for the year was 7.77m compared to 2013 ( 7.10m). Total icome for the year was 9.36m compared to 2013 ( 8.11m). This icrease is largely due to the cotiued geerosity of our doors supportig our visio. Over the last umber of years MQI have had to deal with reductios i state fudig at a time whe demads o its resources are icreasig. I respose to this MQI have bee workig hard to ot oly icrease its volutary fudig but to also cotai costs where possible whilst tryig to ot impact its frot lie ad core services. Some of the measures udertake i 2014 are: Review of o-goig service cotracts ad expediture to achieve cost savigs where applicable Strategies i place to icrease the umber of DSP participats, voluteers ad graduates i the orgaisatio Utilisatio of outscourig where appropriate ad cost effective Parterships with busiess ad other charities provided efficiecies i the provisio of cliet services I 2014, 82% of all costs were directly spet o the core service workers dealig with cliets ad the core objectives of the Compay; these iclude cousellors, project workers, outreach workers, urses, caterig staff, food ad homeless resources services. Allocatig sufficiet expediture to staff, admiistratio ad maagemet is essetial to esure that all resources, icludig doatios, are used to greatest effect for public beefit ad that the highest stadards are maitaied at all times. MQI is gratefully supported by various differet state fuders icludig Midlads Regioal Drugs Task Force, HSE, Probatio Service, Dubli City Coucil, Dubli Regioal Homeless Executive, South Ier City Drugs Task Force, The Family Support Agecy, Departmet of Social Protectio, Pobal, South East Regioal Drugs Task Force ad the Irish Priso Service. OUTLOOK FOR 2015 MQI is committed to its cliets ad the improvemet o how fuds are spet as well as maximisig the value for these 82% Services fuds. We will cotiue to try ad develop ew icome streams i lie with emergig cliet eed. Whilst it is a challege to pla or develop ew services with the difficult ecoomic climate, MQI will cotiue to work with state fuders, doors ad staff to try ad esure its frot lie ad core services are maitaied whilst also tryig to deal with ay emergig eeds withi our cliet group. FUNDRAISING The support of those who doate ad support MQI is vital i the cotiuity of services to the poorest ad most margialised i our society.
