They exist not only in low income countries where food shortages and hunger persist, but also in countries where food is plentiful, largely due to imbalances in the diet. Under-consumption of nutrient-rich foods like fruits and vegetables, healthy oils, fish, lean meats and dairy products, creates subtle nutrient gaps. 3 Global trends drive demand Global trends are creating a positive outlook for dietary supplements and fortified foods 2 : Despite their regional differences, consumers share certain basic health concerns as well as positive attitudes toward the use of dietary supplements and fortified foods to fill dietary gaps. Rising awareness of vitamin supplements as a way to fill dietary gaps has contributed toward doubledigit growth in emerging markets: 70% of shoppers in China and India, 60% in Latin America and 33% in Western and Southern Europe agreed that it is important to take a vitamin or mineral supplement every day. 3 In mature multivitamin markets such as the U.S. and Western Europe, more health-aware, Internet-savvy seniors continue to drive a robust market for antiaging nutritional products. 2 4 Essentials for life Tips for healthy aging: Growing concern for health and wellness Greater awareness due to access to health and nutrition information via the Internet, mass communications and government education campaigns Greater numbers of older individuals seeking to delay the effects of aging Rising incomes and an expanding middle class in emerging markets Consumer movement to self-care High risk groups are the same around the world School-aged children and teenagers are at risk for multiple nutrient insufficiencies due to nutrient-poor diets, dieting for weight loss, rapid growth spurts, and unbalanced food patterns such as avoidance of dairy or meat products. Studies have found inadequate intakes of vitamins A, C, D, E, folate, calcium, iron, potassium, magnesium, phosphorus, zinc, copper and selenium among school-aged children and teenagers in Western, Southern, Central and Eastern Europe, and the Nordic countries Elderly people are often at risk because of decreased intestinal absorption, decreased ability to synthesize certain vitamins (such as vitamin D), use of multiple medications, or physical disability secondary to chronic disease. Even otherwise healthy elders may suffer nutrient deficits from decreased food intake. Risk nutrients that have been documented in the elderly include vitamins A, E, D, C, B3, B12, folate, riboflavin and thiamin; and the minerals potassium, calcium, iron, magnesium and zinc in regions as diverse as Europe, the US, Australia and the Philippines Women of childbearing age are often at risk for iron deficiency due to menstruation. Lifestyle factors such as slimming diets, unbalanced food intakes and sun avoidance can also compromise women s nutritional status. Women who may become pregnant should pay special attention to folic acid: it is critically needed during the first six weeks, before a woman may realize she is pregnant, to prevent neural tube defects in the embryo. Low intakes of folate as well as vitamin D, potassium, calcium, iron and PUFA have been found in women throughout the world. 11,18 Pregnancy imposes high nutritional demands on a woman s body. Ideally, every pregnant woman should be receiving prenatal care, however, studies have found that even middle to upper-income women and women participating in a prenatal clinic have dietary gaps for several nutrients. 19,20 When pregnancy is superimposed upon adolescence, poor micronutrient status may increase the risk for small-for-gestational-age infants. 21 Nutrition surveys repeatedly identify school-aged children and teenagers, the elderly, women of childbearing age and pregnant women as groups at highest risk for nutrient inadequacies Essentials for life 5
4 Essentials for life Risk nutrients create niche marketing opportunities Vitamin A Fighting the epidemic of vitamin A deficiency in developing countries has been a long-term public health priority of the World Health Organization and other humanitarian groups. Vitamin A deficiency is a leading cause of blindness in these areas and contributes to high rates of childhood mortality from infectious diseases. 22 Deficiency during pregnancy can cause serious birth defects. 23 β-carotene is a plant compound that the body can convert to vitamin A, and is a common source of this vitamin in the diet. 24,25 Vitamin B12 This is provided mainly by foods of animal origin, and may be lacking in poor diets. In younger adults and in socioeconomically disadvantaged populations, low vitamin B12 status usually corresponds to low intakes of these foods; in the elderly, poor absorption is more often the cause. Low or deficient vitamin B12 status has been found in 5 to 20% of older adults in the US and UK. In Latin America, Africa and India it is a serious problem affecting 40% to 80% of children and adults. 17 Vitamin C This is widely available in fruits and vegetables, fortified foods and dietary supplements, yet low vitamin C intakes and low blood levels have been found among one-third to one-half of adults in the US and Canada. Smoking is one risk factor that robs the body of vitamin C. 26,27 Vitamin D The sunshine vitamin is both obtained from the diet and formed in the skin during exposure to sunlight. Recently has it become clear that about half the world s people have inadequate vitamin D status, particularly the elderly, people with darker skin, and those who cover up to avoid sun exposure. 28 Vitamin D is listed as a critical nutrient for all fourteen countries evaluated in the European Nutrition and Health Report. 11 Levels are even low in countries with sunny climates such as Saudi Arabia, Africa and India, and in countries with vitamin D fortification of foods such as the United States and Canada. 29,30 Women, people with darker skin, the elderly and obese people appear to be most at risk. 31,32 Factors that increase risk for vitamin D deficiency include: 1. sun avoidance or heavy use of sunscreens 2. aging, as the skin loses its ability to make vitamin D 3. dark skin, because the pigment melanin is an effective sunscreen 4. obesity, which makes it difficult for the body to access stored vitamin D Vitamin E This is the fat-soluble antioxidant vitamin. It has a protective role in the body, defending cell membranes and lipoproteins from oxidative damage. Even though overt deficiency is rarely found, recent reports show that this vitamin is lacking in largescale nutritional surveys. In Germany, about 50% of adults, and in the US up to 90% of adults do not meet daily requirements for vitamin E. Supplements or fortification can fill this critical gap in otherwise well-nourished populations. 33,34 Folate (Folic acid) This supports DNA and RNA replication in new cells, protein metabolism and the health of red blood cells. Mild deficiency produces tiredness, irritability and appetite loss; severe deficiency leads to anemia that leaves a person weak and short of breath. Deficiency during pregnancy can cause premature birth, low birth weight, or birth defects. Folate inadequacy due to poor diet quality is a worldwide problem except in Canada and the United States, where fortification of cereal grains with folic acid has corrected most deficiencies. 35 Vitamin K Vitamin K plays an essential role in blood clotting and also supports bone health. 36 Reports around the world show that about half of adults do not meet dietary intake guidelines for this important vitamin Insufficient vitamin K intakes are found more often in younger adults, and the elderly over the age of 85,mainly due to their low consumption of green leafy vegetables. 39,40 Supplements currently do not contribute significantly to vitamin K levels, and Other vitamins There are six B vitamins in addition to folate and vitamin B12. Most of the B vitamins are involved in the release of energy from carbohydrates; thus, people with higher energy expenditures such as those active in sports may have higher requirements. Other B vitamins are involved in fat and protein metabolism. In countries that fortify grain products with thiamin, riboflavin and niacin, severe deficiencies are uncommon. However, use of oral contraceptives, alcohol intake, dieting and other lifestyle factors can cause marginal B vitamin deficiencies. 6 Essentials for life offer a chance for people to fill this dietary gap. 39 Essentials for life 7
5 ROPUFA /life sdha long chain omega-3 and 6 fatty acids There are two classes of PUFA: omega-6 and omega-3. Primary dietary sources of omega-6 PUFA are corn oil, soybean oil and safflower oil. Dietary sources of omega-3 PUFA include oily fish such as salmon, tuna and herring, as well as a few plant oils including canola and flaxseed. Although canola and flaxseed oils are rich in short chain omega-3 fatty acids (linolenic acid) these must be converted to the long chain omega-3 fatty acids, DHA and EPA, in the body and the conversion rate is quite poor. While both types of PUFA are needed, shortfalls in the diet tend to be in long chain omega-3 because of low fish consumption and an overabundance of omega-6 in the food supply. 41,42 Omega-3 PUFA (DHA and EPA) provide multiple health benefits support of heart health, mental health, immune function and fetal development, to name just a few. Consumers awareness of these benefits and of their own dietary gaps have made omega-3 dietary supplements and fortified foods best-selling categories; continued category growth is expected. 2 Minerals Minerals are essential for healthy bones, muscles, heart, brain and nerves, fluid balance and many aspects of metabolism. The minerals most likely to be low or deficient in large population segments are calcium and iron, and to a lesser extent, potassium, iodine, magnesium, selenium and zinc. Iron deficiency anemia is prevalent in many parts of the world affecting over 1 billion people worldwide, especially India, China and Africa. Iron deficiency anemia is considered a major public health problem and is usually compounded by other nutrient deficiencies such as nutritional anemia most vulnerable population groups are pregnant women, women of childbearing age and young children. 18 Adequate intake of other minerals is also important for all-round nutrition: zinc plays a role in immune function, and magnesium may reduce tiredness and fatigue. 43,44 Inadequate calcium intakes are prevalent worldwide, especially in children and teenagers, women and the elderly, potentially causing poor bone development in the young and osteoporosis in the old. Consumer education has spurred a strong market for dietary supplements and fortified foods containing calcium in many regions, with calcium supplements showing particularly strong growth in China. 2 8 Essentials for life Essentials for life 9
