3 Introduction The Paediatric Physiotherapy service works with children from 0-19 years who present with general developmental delay, disorders of movement, disabilities or illness, which can be improved, controlled or alleviated by physiotherapeutic skills and/or the use of specialised equipment. Based at the Children s Centre, staff provide a community physiotherapy service for children living within the Sunderland area and also for inpatients on the Children s ward at Sunderland Royal Hospital. Paediatric Physiotherapy covers most of the multiple pathologies benefiting from physiotherapeutic intervention. Neurodevelopmental conditions, progressive neuromuscular disorders, musculoskeletal conditions and rheumatology, elective and acute orthopaedics, burns, plastic surgery, oncology, a wide range of respiratory conditions including intensive are and neonatal care. It is essential that the physiotherapist has specialist paediatric core skills in addition to skills particular to the different specialities encountered. The next page details the structure of our paediatric physiotherapy department including staff specialties. 3

5 Directions Directions Children s Centre By Train: Newcastle to Sunderland or Middlesbrough to Sunderland then bus By Bus: From park lane interchange bus station take numbers 3,4,23,24 or 133 and ask for The Barnes pub. Cross at traffic lights to the park and head up the hill (A690 Durham Road). The Children s Centre is 200 yds on the right. Follow road to the back of the centre. Physiotherapy stands alone at the back of the centre. By Car: A19 to A690 Sunderland. Follow signs to City Centre. At traffic lights at Lidl (on left) go straight ahead, down a hill, passing Bede College on left. The Children s Centre is immediately after College boundaries. Follow road through Car Park and head behind main building. This car park is now a pay and display. (Student permit available from main hospital site charge 1.50 per day) 5
6 Placement Procedures We would be grateful if you would contact our department prior to your placement to determine start time and venue. First day Please arrive at the Children s Centre, Durham Road, at time arranged prior to placement. Please bring to the placement your SWOT analysis and placement objectives. You will be given you placement timetable on your first day following your induction and introduction to the team. Changing Facilities: Changing room at Children s Centre Uniform: Staff room: Start time: Canteen: In-service: General Information: Banking: Hospital shop: Educational Facilities Library: Study Areas Usual physio uniform with either tunic or t-shirt. Please bring with you swimwear, T-shirt to wear in the pool and a towel. Yes First day 9am and then to be negotiated with Clinical tutor (between 8.30am and 9am) Lunch Finish time determined by start time (between pm) No. Please bring a packed lunch with you. Tea and Coffee available. Microwave. One afternoon per month with staff, but individually as and when needed. None on site. Cash point at Barnes Garage 100yds Newsagent 75yds away Small Departmental Library at Children s Centre. Access to hospital library at main hospital site. Only at Sunderland Royal Hospital (SRH) Reference Literature Packs: Only at SRH 6
7 The Pre-school Team Physiotherapists in the Pre-school team treat children from birth to 5 years. They offer advice to parents and carers on the handling, positioning and treatment of babies and young children who have experienced birth trauma, difficult beginnings and those who present with delayed development. Referrals into this part of the service usually come from Neonatologists, Consultant Paediatricians and GP s. The child is usually treated in their own home by the physiotherapist but treatment can also take place in Nurseries, schools and other centres. The therapist may provide advice about positioning and handling of a child on the Special Care Baby Unit. Based at the Children s Centre, the Child development Unit caters for pre-school children with long-term difficulties, unknown diagnosis, language delay, Autistic Spectrum disorder and behavioural problems. Children attending the Child Development Unit often require input from therapists from different disciplines. Speech and Language Therapists, Occupational Therapists and Physiotherapists will often work together with Specialist Nursery Nurses to meet the needs of this complex group. 7
8 The Mainstream Service Therapists in this team work with children who have physical and medical need, aged between 5 and 18 years attending mainstream schools and colleges of further education. To minimise disruption of the child s education, treatment often takes place within the school setting but children may also attend outpatient appointments at the Children s Centre or be treated in their own home. The physiotherapist collaborates closely with school staff offering advice about the child s physical and medical needs, their functional ability, the level and type of support they require to enable them to safely access the curriculum. The physiotherapist may train Educational Support Staff to safely and effectively deliver exercise and mobility programmes, how to position a child in specialist equipment and to put on and take off orthoses. Referrals to this part of the service may be from Consultant Paediatricians, Orthopaedic Consultants, Rheumatologists, GP s and School Doctors. 8
9 Special School Service Some children s physical and medical needs are so complex that they can only be met within a specialist setting. Therapists working in this team provide input to four non-residential special schools. Sunningdale School This school caters for children aged 4-11 years with Physical disability and PMLD (Profound and multiple learning difficulties). An assessment unit based within the school admits children from 2½ years to identify which educational placement will be best suited to their needs. Portland School Children aged 11 to 19 years with physical disabilities and PMLD attend this school. Barbara Priestman School Secondary aged children with physical disability or Autistic Spectrum Disorder attend this school. 9
10 Musculoskeletal and Rheumatology GP s or Consultants in Orthopaedics, Rheumatology and Community Paediatricians may refer children with conditions affecting the musculoskeletal system to this part of the service. Children often visit the Children s Centre as outpatients for treatment and attend the pool at Sunderland Royal Hospital for hydrotherapy. Inpatient Paediatrics Staff from our service treat patients on the three-paediatric wards at Sunderland Royal Hospital. F63 Children who have undergone orthopaedic surgery or have been admitted for injections of Botox. Half of this ward is also for ENT (Ear, Nose and Throat) surgery. F64 Children with respiratory difficulties and those with Cystic Fibrosis F65 Babies with respiratory or medical problems The physiotherapist also performs the diagnostic sweat test to determine whether or not a child has Cystic Fibrosis 10
11 Multi-disciplinary and Multi-agency Team Working To effectively meet the needs of the child s family, the Paediatric Physiotherapist works with other professionals as part of a multi-disciplinary and multi-agency team. 13 Paediatric Conditions The Paediatric Physiotherapy Service offers treatment for children with problems caused by neuromuscular, musculoskeletal, cardiovascular and respiratory conditions, congenital abnormalities and acquired disorders. Neurodevelopmental Cerebral Palsy Spina Bifida and Hydrocephalus Developmental Delay Neuromuscular Muscular Dystrophies (Duchenne, Becker, Congenital, Myotonias, Emery Dreyfus) Leucodystrophies Charcot Marie Tooth Disease Musculoskeletal and Rheumatology Perthes Osgood Schlatter s Disease Osteogenesis Imperfecta Arthrogryposis Multiplex Congenita Hypermobility Syndrome Scoliosis Talipes Equinovarus Juvenile Ideopathic Arthritis Severs Disease Rehabilitation following acute and elective orthopaedic surgery and trauma Respiratory Cystic Fibrosis Chest Infections Acquired Conditions Head Injuries Amputation Developmental Co-ordination Disorder Autistic Spectrum Disorder - Usually only if the child has associated problems which responds to therapy 13

15 Learning Opportunities The following Learning Opportunities are available: Assessment, treatment and management of children, who have a general development delay, disorder of movement, disability or illness, which can be improved, controlled or alleviated by physiotherapy. Experience working in a variety of community settings e.g. Patients homes, specialist and mainstream schools and nurseries, Child Development Unit and Children s Centre. Treatment of inpatients on the Paediatric wards at Sunderland Royal Hospital. Attendance at Specialist clinics (Orthopaedic, Paediatric, Rheumatology, Cystic Fibrosis, Orthotic, Wheelchair). Experience treating a range of conditions using hydrotherapy. Use of specialist equipment e.g. orthotics, wheelchairs and wheelchair supportive seating, standing frames, sleep systems and mobility aids Opportunity to acquire moving and handling skills Collaboration with some of the members of a wide multidisciplinary and multiagency team. Opportunities to acquire therapeutic handling and positional skills specific to this client group. Your clinical placement will be planned prior to your arrival. You will spend time with therapists working in the Pre-school, Mainstream and Special school services, on the Paediatric wards at Sunderland Royal Hospital and with children presenting with musculoskeletal disorders. Access to some of the learning opportunities outlined in this document will depend on the time of your placement and availability of what is on offer i.e. schools are closed during half term holidays, absenteeism of patients or staff will result in changes in timetable. 15
