2 We welcome you to the Ninth Annual Interdisciplinary and Interfaith Conference of the Collaborative for Palliative Care, Reframing Palliative Care: Education, Research and Advocacy. This year s full-day conference at Calvary Hospital features renowned medical ethicist Dr. Joseph J. Fins as keynoter, and prominent palliative care physicians Dr. Thomas Caprio and Dr. Cary Reid as morning and afternoon plenary speakers, respectively. We are also joined by leading health professionals in the palliative care field who hail from the Hudson Valley and New York City and offer diverse panels that span inter-professional and trans-disciplinary interests. The Collaborative for Palliative Care continues to build on its rich history and traditions since its founding in 2005, drawing on community assets and resources in educating professionals and community leaders about best practices and future directions in palliative care. Through this important work, we strive to support those who are living with serious and life-limiting illnesses, as well as their formal and informal caregivers, and to expand community-based palliative care. As you may know, the Collaborative for Palliative Care is entering a critical stage of its work in this 12th year of its history. Begun as a grassroots movement of community leaders interested in improving the lives of individuals with serious illness, the Collaborative now includes many health care organizations and health and aging network professionals, agencies, governmental and non-governmental organizations, and businesses in our region. As we expand the circle of stakeholders who are committed to achieving equitable access to palliative care services and improving the delivery of high quality palliative care through education, research and advocacy, we take this opportunity to thank the many dedicated individuals and organizations who have generously made this conference possible! Your efforts will help sustain the robust contributions of the Collaborative in building community capacities and assuring equitable access to high quality palliative care across diverse populations and care settings. Older adult patients with physical frailty, cognitive loss, and multiple chronic medical conditions can make it challenging to assess palliative care needs and develop person-centered plans of care. New York State is also grappling with high maternal death rates among black women and documented health disparities across the state including New York City. The magnitude of this public health problem and its deep social and economic determinants call for a community-based, palliative approach to care.
