And with a little help from Facey s Patient Education team, you can take managing your health to a new level. We offer classes on a wide variety of subjects, ranging from general nutrition and stress management to more illness-specific issues like diabetes, high blood pressure, and cholesterol. Facey gives you the tools to live smarter. Signing up is easy. If you see a program in this guide that interests you, please call us toll-free at to learn about upcoming class dates and to register. We also post the current schedule of classes on our web site at if you prefer. Not all classes are created equal. Most of our education classes are open to the public and are offered free of charge. Several classes are offered in Spanish. We have marked our classes with icons to make it easier for you to see which programs are available to you:! 4 diabetes management & prevention All classes facilitated by a, Pre-Diabetes Recent findings indicate that healthy eating, moderate exercise, and a 5 7 % weight loss could delay or possibly prevent Type 2 diabetes in people who are at risk. This class provides you with the tools to make lifestyle changes including improving diet, increasing physical activity, achieving weight loss, and reducing stress., Canyon Country Languages: English and Spanish Diabetes Education Program A two-session class which will help you maintain better blood sugar control and learn how to live healthier with Type 2 Diabetes. You will learn lifestyle and nutrition strategies for managing your diabetes, about medications and how to avoid or delay diabetes-associated complications., Canyon Country Languages: English and Spanish Diabetes Support Group An open support group for you and your loved ones to share your thoughts, be heard and receive on-going support for living with diabetes from a Diabetes Educator, Licensed Marriage and Family Therapist and each other. During a one-to-one appointment with a registered dietitian, certified diabetes educator you will receive an individualized gestational diabetes nutrition care plan and diabetes management education including managing blood sugars, learning how to test blood sugars and strategies to prevent diabetes related complications. Please talk to your medical provider for a referral. A co-pay may apply. Languages: English, Spanish, Armenian, Farsi Individualized Appointments with a and/or a f! 6 weight management All classes facilitated by a Healthy Weight Management A comprehensive program to help you learn healthy eating and physical activity strategies and tips for overcoming weight loss challenges. Topics include food choices, menu planning, portion sizes, dining out, cooking techniques, interpreting food labels, healthy habits and budgetfriendly grocery shopping., Canyon Country Languages: English, Spanish Healthy Kids It s never too early to teach your children how to lead healthy lives. This class is designed to teach children and their families about balanced nutrition and healthy activity, helping them to become healthy kids. Topics include healthy food choices, portion control and label reading, tips for making exercise fun and family communication skills. Languages: English Individualized Appointments with a f! During a one-to-one appointment with a registered dietitian, you will receive an individualized nutrition care plan with lifestyle recommendations based on your medical and personal history, food and lifestyle preferences and health goals. 7 bariatric surgery education & support All classes facilitated by a Bariatric Education Pre-Surgery Preparation f! A comprehensive 6 month program to prepare you for weight-loss surgery. Topics include emotional and physical preparation, types of surgeries, risks and outcomes, and long-term lifestyle and dietary changes. Completion is required before surgical consultation is approved., Valencia Specialty and Women s Center Languages: English, Spanish Bariatric Surgery Support Group An open support group for you and your loved ones to share your thoughts, be heard and receive on-going support for life before and after bariatric surgery. This is a safe forum where you can discuss the impact of weight loss surgery on your life and learn how to focus on success. Languages: English 7
8 prenatal & postpartum wellness All classes facilitated by a, Certified Lactation Educator, Registered Nurse and Licensed Marriage and Family Therapist Prenatal & Postpartum Wellness Program A comprehensive program that will prepare you for baby to arrive. In each class you will learn important strategies to ensure you and your baby are healthy and safe. Topics include the importance of prenatal care, recognizing labor, breast-feeding, proper nutrition for pregnancy and postpartum wellness. This is a four-part series you can join in at any session. 9 lifestyle wellness Healthy Heart: Blood Pressure & Cholesterol Our program helps you and your loved ones identify and reduce risk factors for heart disease, recognize signs and symptoms of heart attack or stroke, and apply strategies to control blood pressure and cholesterol. Languages: English, Spanish Healthier Living f This 6 week program is for anyone living with a chronic condition. This is an evidence-based program that focuses on selfmanagement techniques. You will learn how to set goals, make weekly action plans, eat healthier, be active, manage difficult emotions, problem solve, sleep better, improving communication and manage stress., Valencia Specialty and Women s Center Languages: English, Spanish Exercise Join us for a guided chair exercise routine to improve flexibility, balance, and strength. 10 lifestyle wellness (continued) Fall Prevention In this class you will learn to identify the causes of balance problems and how to reduce your risk of falls by learning specific exercises to improve balance. Once you receive a brief fall risk assessment by a physical therapist, we will discuss ways to keep you safer at home. Languages: English Advance Directives In this class you will learn how to complete the California Advance Health Care Directive form. We ll also discuss various important documents, including: Durable Power of Attorney for Health Care, Living Will, and Physician Orders for Life-Sustaining Treatment (POLST)., Canyon Country Languages: English, Spanish Individualized Smoking Cessation Program f! Learn strategies to stop smoking including medications, setting a quit date, and utilizing available resources to become free from your tobacco addiction.
