LOCKOUT DEVICES Explanatin When an islatin f any mbile plant r equipment is required befre safe access t a machine can be prvided the fllwing must be undertaken: Machine islatin requirements must be addressed n task specific JSA All persns t btain persnal islatin lcks, danger tags and lck hasps frm the maintenance ffice. Issuing f persnal islatin lcks and danger tags must be registered n the Persnal Islatin lck register. Out f service tags must be attached t all islatin pints befre persnal islatin lcks & tags can be placed n islatin pints. Supervisrs t versee all islatins and attach ut f service tags t islatin pints. Each Individual wrking under islatin must place their wn islatin pint All keys must be remved frm islatin lcks nce placed n the islatin plant. Only the persn wh placed a persnal islatin lck may remve the lck. Any persn in any dubt as t what needs t be islated r hw it is t be islated must stp immediately and cntact their supervisr. Lckut Devices that can be used- persnal islatin Lck & Danger Tag, Lck Hasp Unsuitable r ineffective lck ut devices- failure t use apprval lck ut devices may result in the lckut being ineffective and expse individuals t energized equipment that may result in equipment that may result in equipment damage r persnal injury. 2. LIGHT VEHICLE POSITIVE ISOLATION POINTS When required t islate and carry ut maintenance r repair activities n light vehicles, the methds f islatin shall be carried ut: Main Islating Switch Fitted Remve the key frm the ignitin switch and return t maintenance ffice

Light vehicle main islatr where fitted N main islating Device Fitted Remve the key frm ignitin switch and return t maintenance ffice Discnnect the earth lead frm the negative terminal f the battery.

Attach a persnal islatin lck and danger tag nt the hasp n the battery lead clamp.

Supervisr t attach yellw ut f Service tag t the hasp n the islatin pint Attach a persnal islatin lck and danger tag t the main islatr. Light vehicle main islatr where fitted N main islating Device Fitted Remve the key frm ignitin switch and return t maintenance ffice Discnnect the earth lead frm the negative terminal f the battery. Supervisr attaches a yellw ut f Service tag t hasp nt the battery terminal clamp. Discnnect and lck ut earth lead ff light vehicle battery. Attach a persnal islatin lck and danger tag nt the hasp n the battery lead clamp. Energized equipment- failure t islate and lckut main islatr may result in unauthrized peratin f vehicle resulting equipment damage r persnal injury. 3. MOBILE PLANT & EQUIPMENT POSITIVE ISOLATION POINTS When required t islate and carry ut maintenance r repair activities n mbile plant r equipment and where main battery islatrs have been fitted, the fllwing shall ccur. Battery islating switch fitted Remve the key frm the ignitin switch and return t the maintenance ffice Lcate the main battery islatr and turn it t the ff psitin Supervisr t attach a yellw ut f service tag t hasp n the islatin pint Psitive Islatin Pint n Excavatr Attach a persnal islatin lck and danger tag t the main islatr Remve persnal islatin lck key - 2 -

MACHINE ISOLATIONS WHERE NO POSITIVE LOCKOUT POINT IS FITTED Where machines have nt been fitted with main battery islatrs, alternate means f islatin and persnal prtectin must be used. The prcess that needs t be fllwed in these instances is as fllws: Remve the ignitin key frm the ignitin switch Place the ignitin key thrugh the jaws f the lck hasp and attach the hasp t the base f the indicatr lever n the steering wheel clumn Supervisr t attach the yellw ut f Service tag t the hasp n the islatin pint Attach persnal islatin lcks and danger tags nt the hasp Persnal islatin pint n Isuzu truck steering clumn Remve persnal islatin lck key Energized equipment- failure t islate and lckut main islatr may result in unauthrized peratin f vehicle resulting equipment damage r persnal injury. 5. REMOVING ISOLATIONS Once all wrk has been cmpleted n the islated piece f plant and machinery r it is the end f the shift, the fllwing shuld take place: Wrk activities cmpleted Ntify supervisr f cmpleted wrks Supervisr t asses machine t ensure it is safe t return t service Supervisr apprves cmpletin f wrks and authrizes islatin t be remved

Supervisr remves the yellw ut f service tag frm the hasp and then remves the hasp frm the islatin pint. Supervisr t authrize light vehicle, plant r equipment t be re-energized and returned t service Out f service tags must the last tag remved by Supervisin frm the islatin pint t ensure all persns have remved their persnal islatin lck and the machine is fit T return t service. End f shift- Wrk Activities Nt Cmpleted Outging maintenance persnal t ntify their shift supervisr f incmplete wrks utging maintenance persnnel t remve their persnal islatin lck and danger tag frm islatin pint utging shift supervisr t ntify incming shift supervisr via written handver reprt and verbal handver Incming maintenance persnnel t place their wn persnal islatin lcks and danger tags n the main islatin pint. CHECKLISTS TO ASSIST WITH THIS WORK ACTIVITY PRE CHECKS Ensure that all islatin and lckut equipment is available; Identify islatin and lckut methd; Understand defect management prcesses and the use f ut f service tags TOOLS & MATERIALS REQUIRED Pre- Start Inspectin Checklist; Out f services tags available Crrect PPE - 4 -
