From the erly dys of the founders nd the ssocition s multi-professionl membership, psychitrists, psychologists, eductionl scientists, mentl helth nurses, mngers of support services for people with intellectul disbility were convinced tht stging biennil Europen congress presenting nd promoting dvnces in the re of mentl helth nd intellectul disbility might be substntil step towrds mjor chnge. Indeed, up to this point, mentl helth issues nd problem behviour in people with intellectul disbilities were commonly referred to s being inherent to the intellectul disbility itself, with low intellectul functioning being viewed s resultof dysfunctionl or bnorml brin sitution. In line with this ssumption nd the fct tht there ws no cure for the suggested brin dmge, it ws common prctice to tret mentl helth problems in this popultion without comprehensive mentl helth ssessment nd thus bstining from tretments bsed on dignostic criteri nd clinicl evidence. Now, 20 yers lter, with mjor dvncements in bsic nd pplied reserch, knowledge on mentl helth issues specific to people with intellectul disbilities hs rised substntilly. In the pst 20 yers, the EAMHID orgnised ten conferences, with the inugurl being held in Amsterdm in 1995 nd the 10th being hosted in Florence in Abstrct compiltions of these conferences re good proof tht EAMHID is offering unique pltform in Europe for exchnging dvnces in the field, disseminting evidence-bsed prctices, promoting competences nd skills within professionl groups nd encourging reserch. Obviously, much chnged during this period, with more nd more voices reching out with convincing evidence, using the network offered by EAMHID. However, limittions of these chievements become evident when evluting the success of the knowledge ccumulted in this period being trnsferred into prctice nd when ssessing the impct our knowledge ctully hs for the mentl well-being of individuls with intellectul disbilities.

This interply nd reciprocl effect between personl outcomes nd socil fctors s well s the biologicl fctors intercting with behviour nd experience re highlighted in the so-clled bio-psycho-socil frmework, widely recognised model. Consequently, this prdigm prominently determines WHO Europe s cycle for mentl well-being. Recently, most of our societies committed themselves to developing equl opportunities for people with disbilities, iming, mong other things, for socil inclusion of people with disbilities in ll res of life, including people with intellectul disbilities, presenting substntil move offering options for new mentl helth promotion strtegies especilly for people with intellectul disbilities. The lck of inclusion nd violence re known to be mjor risk fctors for mentl helth, while n ccessible nd cceptble mentl helth system is of gret importnce for effective support nd eduction, nd community ties re significnt protective fctors for good mentl helth. In ddition, recent evidence on dversities in the ffective-emotionl development of children nd young dults with intellectul disbilities s well s new findings with respect to biologicl fctors nd gene-to-behviour reltions, often reported with specific syndromes, re now offering us new perspectives for understnding the complex nd high vulnerbility for mentl helth problems in people with intellectul disbilities. Thus, with this conference in 2017, the first of the next series of ten, EAMHID strives towrds next mjor step for improving mentl helth for people with ID. In its cll for contributions, EAMHID offered trditionl formts for presenting genuine reserch like individul ppers or symposi, offering the collbortion of up to four scholrs, contributing to one overll topic. In ddition, co-productive workshops were offered within the trditionl congress trcks. We re convinced tht congress offering n inclusive setting nd tking dvntge of more inclusive strtegies will offer better frme for inspiring experiences. Once the medicl discourse hs reched its limits, mny ll-importnt questions remin unnswered for those struggling with complex nd lifelong problems. Wht do these evolutions entil for persons with mentl helth problems in ID? Our yerlong experience with these people in Belgium nd Luxemburg llows us to identify some problemtic phenomen viz. frgmenttion nd splitting tht impose (n) (ideology-)criticl ttitude towrds the dominnt discourse nd its guiding principles. Our experience imposes certin fundmentl reflections on lnguge hygiene (Broekmn, 1996): However importnt principles like inclusion nd societlistion my be, they re not self-evidence but chllenge,ndtheyshouldnotbewieldedsslogns or dogms. As guiding ethicl principles tke form, they need constnt reformultion on humn scle. We distinguish three domins in which the core problem of frgmenttion nd splitting occurs with ll its ensuing itrogenic effects: Legl nd dministrtive orgnistion of helth cre: double dignosis In our Western societies with ll their historic ntionl idiosyncrsies, the differentition between mentl helth cre nd disbility cre hs yielded seprte orgnistion of the ltter. It hs developed its own identity, different nd seprte from mentl helth cre. They ech fll under the jurisdiction of different ministries nd dministrtions, nd hve their seprte finncing nd regultions. They lso use expertise nd knowledge models from different fculties. They employ more or less numerous, differently trined stff. Both function ccording to their own (mutully exclusive?) discourse. The double dignosis in this context is very meningful rtefct. I do not use the term to refer to double dignosis strictu sensu; I m referring to those persons who resort under both domins t the sme time. It is not the clinicl but the business logic (who foots the bill?) which becomes the finl stumbling block through which these people fll between two orgnistionl stools. Double dignosis turns into n excuse for splitting nd exclusion. Lyered relities: colonistion of life fctulity Themeso-leveliswheretheinterfcendtrnsformtion must tke plce, from the objectivting mcro-logic to the level of ctul service, nmely in the life-fctul subjective nd intersubjective context of the rel individuls concerned. FMRP controls the trnsltion of hundreds of messges importnt for other disorders

21 EAMHID ABSTRACT BOOK 7 including schizophreni nd utism spectrum disorder (ASD). We know tht the mglur5 pthwy is upregulted in FXS nd the GABA pthwys re downregulted. The initil trils of mglur5 ntgonists did not show efficcy in dolescents nd dults with FXS in the behviourl outcome mesures tht were chosen. However, there is evidence tht younger ptients my do better nd the outcome mesures should include cognitive mesures in ddition to electrophysiologicl studies, mesures of hbitution to sensory stimuli, eye trcking studies, nd behviourl mesures. We hve found tht the event relted potentils (ERPs) prticulrly hbitution prdigms tht trget brin processing bnormlities with deficient GABA inhibitory input were prticulrly helpful in our controlled tril of minocycline in young children with FXS. Other new mesures include expressive lnguge smpling mesures developed by Dr Len Abbeduto nd collegues nd the use of the NIH toolbox dpted by Dr Dvid Hessl for children with FXS to ssess improvements in cognitive processes nd ttention. We hve lso lerned tht erly intervention with trgeted tretments cn significntly improve developmentl testing in young children with FXS s seen by the controlled tril of low dose sertrline in those ges 2 6yo. This controlled tril of mg of sertrline over 6 month period demonstrted significnt improvements in the Visul Perception nd Fine Motor subtests nd in the overll Composite T score in the Mullen Scles of Erly Lerning (MSEL) in those on sertrline vs plcebo. Those with FXS plus ASD lso demonstrted significnt improvement in the Expressive Lnguge subtest of the MSEL on sertrline vs plcebo. Erly intervention is lso studied now with the mglur5 ntgonist, AFQ056, combined with intensive lnguge intervention provided by videoconferencing to the fmily in their home for young children with FXS ges 3 6yo in multicenter tril funded by NIH. Mouse nd Drosophil models of FXS hve demonstrted the benefit of metformin s trgeted tretment nd encourging results re seen in open lbel studies with controlled tril in process. Other medictions tht hve shown efficcy in FXS include minocycline, rbclofen nd trofinetide nd these studies will be reviewed. Since mny pthwys re dysregulted in FXS it my tke more thn one tretment to mke overll cognitive nd behviourl improvements in FXS. The ddition to novel lerning prdigms for FXS re lso expnding nd such prdigms cn be combined with trgeted tretment. We hve shown efficcy in lnguge improvements with Prent Implemented Lnguge Intervention (PILI) tht is guided by speech nd lnguge pthologist combined with BCBA nd the intervention is given through skype twice week in the home. The PILI intervention hs lso been combined with controlled tril of lovsttin, nother trgeted tretment for FXS in n ongoing tril. The future looks bright for new trgeted tretments tht will benefit not only FXS but other neurodevelopmentl disorders including ASD tht hve common spects of moleculr dysregultion.
