2 ABOUT THE SYMPOSIUM The Sickle Cell Disease Patient & Family Educational Symposium (known throughout the sickle cell community as the Warrior Convention) was founded in 2013 by Sickle Cell Warriors, Inc. as an annual gathering of sickle cell patients and caregivers. The Convention is designed, created and hosted entirely by the sickle cell patient and family community. Now in its fifth year, the meeting has grown in size and scope, featuring our most robust agenda to date. Each year the meeting is hosted by a partnership of sickle cell community-based organizations, patients and advisors. We are pleased to announce the 2018 symposium will be hosted by the Sickle Cell Foundation of Tennessee and the Sickle Cell Champions Association (SCCA), in partnership with Hope for SCD and the International Association of Sickle Cell Nurses and Professional Associates (IASCNAPA). With a focus on Warrior Mental Health, and tracks designed specifically for teens and young adults, children and caregivers, and warrior men, the 2018 Convention will bring together our largest representation of Warriors and Caregivers since we began in The agenda is designed by the sickle cell community with topics identified by patients, caregivers and healthcare supporters, ranging from Parenting Issues from Birth to Adult and The Science of Sickle Cell to Sickle Cell Mental Health and Sickle Cell and College Life. Registration opens Wednesday, July 18th at 10 AM. We begin festivities at 4 PM with an opening reception and icebreakers. Early risers are invited to join us each morning at 7 AM for morning yoga and workout. The General Plenary session starts at 10 AM on each day, followed by multi-track breakout sessions, workshops, roundtable discussions and panels. Lunch will be provided each day accompanied by a main general session on a wide variety of topics of interest to the sickle cell warrior community. We are excited to offer several special evening sessions. Join us on Wednesday night for Warrior Game Night & Sickle Cell Trivia. On Thursday we will host Warriors After Dark Men-Only and Warriors after Dark Women-Only (age 17+/15-16 with parent permission). This session is included in full registration and is divided by gender for adults only. We will discuss a variety of adult-themed topics that are important to our warrior community. Friday afternoon has been left completely open to enable participants to enjoy the sights and tourist destinations of Memphis. and then grab your ticket for the Warriors of Wakanda Awards Gala/Prom on Saturday night.

3 About the consortium The 5 th Annual Sickle Cell Patient & Family Educational Symposium is powered by the Sickle Cell Community Consortium. The Consortium is a collaborative of sickle cell patients, caregivers, and community-based organizations (CBOs), united to ensure that the patient/caregiver voice is at the center of sickle cell research, education, awareness, advocacy, legislation and policy. We are patient-powered and patient-centered. All projects, programs, platforms and initiatives, including the annual Symposium, are conceptualized, created and executed directly by the patient/caregiver community. WHY SUPPORT? The 5th Annual Sickle Cell Patient & Family Educational Symposium is the only national patient-designed, organized and hosted meeting of its type created specifically to address needs of sickle cell patients and caregivers. The Symposium brings together patient warriors, caregivers, advocates, healthcare providers and supporters to empower, encourage and educate each other as we define what it is to live SICKLE STRONG. There are multiple opportunities to present to the entire group, interact one-on-one with patients and caregivers and create contacts with community-based organizations and opinion leaders of the sickle cell community. This year, we are delighted to include Warrior Game Night & Sickle Cell Trivia, Paint & Sip Unwind, the Warriors of Wakanda Prom, and the Young Warriors Junior Prom. Vendor booths and exhibit tables are available at deeply discounted rates for community sponsors and small businesses. Sponsorship packages include advertisement opportunities, social media advertisement, prominent logo display, and numerous other perks. Your support will enable us to educate our community, provide scholarships to patients and caregivers in need, support the social, medical and mental health needs of the sickle cell warrior community. 5 WARRIORS IN WAKANDA AWARDS GALA/PROM On Saturday, July 21 st the Sickle Cell Community Consortium is pleased to present the Warriors in Wakanda Awards Gala and Prom. This formal event was created by request of the sickle cell community. The Annual Awards Gala is a collaborative effort between the sickle cell patient, caregiver and CBO community to recognize individuals who have made a difference for those living with sickle cell. This elegant event was purposed as the "Oscars" of the sickle cell community. Each year multiple groups, individuals and community-based organizations join forces to present their awards to deserving patients, caregivers, healthcare providers and research scientists in the community. This is an opportunity for those that missed their prom due to illness to enjoy the experience and for others to experience prom all over again. Often those living with this disease are forced to miss their prom because of illness and fatigue. This is our chance to enjoy prom while honoring champions in the sickle cell community. Unique sponsorship and participation opportunities are available for both. From start to finish, the Symposium was designed to provide support, encouragement and education on all areas of the Warriors life. Exhibitors are invited to purchase a booth in the Warrior Expo Hall to display promotional items and collateral for attendees to take home. Submitted abstracts should describe: 1. New and emerging research, 2. Community education, advocacy, outreach or awareness projects primarily involving sickle cell disease, 3. Descriptive summary of a sickle cell organization, or 4. Literature review of a topic deemed of interest to the patient/caregiver community. Proof of 501(c) status must be provided. Nonprofits are also encouraged to submit an abstract for display. Partner Community-Based Organizations (CBOs) $75 Credentialed, Sponsored, Community and Junior partners of the Sickle Cell Community Consortium may access this special rate. Warrior/Independent Patient $45 This category type is available for all sickle cell warriors and caregivers. Join us and display your goods or services.

10 EXHIBIT GUIDELINES: Purchase of an exhibit booth does not replace registration. All exhibitors planning on attending the Convention must also register. The sale of pharmaceutical products, food, alcohol, tobacco, firearms, or any illegal products are strictly forbidden. Approval and inclusion of exhibit/vendor does not imply endorsement by the Sickle Cell Community Consortium or Consortium partners. A seller s fee of $10 is applied to all exhibitors planning to sell any product or service. An additional $45 fee will be applied to the sale of supplements, oils, vitamins, or herbal/alternative treatments and therapies. Vitamins, nutritional supplements, oils, etc. are not regulated and are not endorsed by the Sickle Cell Community Consortium. Any exhibitor/vendor with the intent to sell such supplements must display a disclosure statement (provided upon arrival) in plain sight of all potential customers. Acceptance of exhibitors is at the sole and absolute discretion of the Consortium. All exhibitors/vendors are subject to the conditions and requirements contained within these guidelines. All exhibits are 10x10 and include a table and two chairs. Exhibitors requiring larger space, or other special request, must contact Alexis Wardlow no later than Friday, June 29th at awardlow@sicklecellconsortium.org. Exhibitor assumes all responsibility and liability for losses, damages, injuries and all claims arising out of the exhibits activities and will indemnify, defend and hold harmless the Consortium and all partners, the hotel, it's agents, staff, interns and volunteers for any and all such losses, damages and claims. The exhibitor assumes responsibility for securing all appropriate licenses for the sale of merchandise and is solely responsible for the collection of all applicable state and local taxes. Exhibitor agrees to pay the rental space in full within 10 business days of approval. The Consortium may revoke acceptance at any time. If cancel by the Consortium applicant will receive a full refund. Exhibitor must cancel prior to Friday, June 29th to receive a 50 % refund. All cancellation after Friday, June 30th will not be refunded and will result in the loss of exhibitor fee. Exhibits cannot be transferred to another for any reason. SPACE IS LIMITED.
