Pre-requisites: DHY 105, DHY 106, DHY 107, DHY 108 COURSE DESCRIPTION This course provides the student with the fundamentals of pain management used in dentistry. A variety of topical and local anesthetic delivery devices will be discussed and utilized. Neuroanatomy, neurophysiology and pharmacology of local anesthetics, vasoconstrictors and topical anesthetics will be covered. A thorough patient assessment including stress reduction protocol is utilized. Techniques of local anesthesia administration including infiltration, field and nerve blocks will be demonstrated and return demonstrations will be performed until competency is achieved. This includes injections for maxillary, mandibular, palatal and supplemental pain control. The expanded role of the dental hygienist is to include administration of oral anesthetics for pain control necessitates a thorough exposure to the theory and practice of administration within the scope of dental hygiene education. This course will prepare the student to take the Permit L which licenses the student to administer local anesthesia in Massachusetts. Critical thinking skills, whole body patient management, local and systemic complications including emergency response to patient medical emergencies will be an integral part of the course. This course will introduce the student to utilizing pain management options in dentistry and provide basic techniques in choosing and administering anesthetic agents for pain control. COURSE OBJECTIVES 1. Discuss the clinical actions of topical and local anesthetics and vasoconstrictors in the body. 2. Review head and neck anatomy for landmarks in local anesthesia administration. 3. Identify the oral landmarks used for local anesthetic administration. 4. Understand the neurophysiology and pharmacology of local anesthetics and vasoconstrictors utilized in pain management. 5. Describe the mode of action for each anesthetic agent and the importance in choice of options. 6. Demonstrate safe handling in the preparation, assembly and disassembly of armamentarium for topical and local anesthetic use. 7. Evaluate patient s medical history for contraindication and patient drug selection for anesthetic agent application. 8. Prevent, recognize and manage local and systemic complications of local anesthesia administration. 9. Use proper infection control and needle stick prevention techniques. 10. Discuss record keeping and legal issues involved with the administration of local anesthesia. 11. Apply knowledge gained in pain control to the clinical care of patients. 12. Safely administer topical and local anesthesia pain control to clinical patients. Teaching Methods: Lecture: Power point, DVD s, CD s, mock Permit L test, anesthetic and epinephrine calculation problems, discussion, question and answer forum. Lab: Demonstrations by faculty and return demonstrations by hygienists in anesthetic techniques in the MWCC dental hygiene clinic. Participants will be patients for each other. Course Requirements: Grades on all weekly tests, examinations, and clinical exams must average a C+ (77) or above. All clinical requirements and expectations must be completed as set forth in portfolio. The students must pass both lecture and lab to pass the course. Student is responsible to make up all missed work and assignments. Attendance Policy: Attendance is expected at all lectures and labs. Absence may adversely affect the class and clinical participation component of the final grade. Individual conversations are very distracting and disrespectful to the instructor and classmates. Please be respectful of this or students talking among themselves will be asked to leave the classroom. Students should notify the secretary prior to class if they are unable to attend. Tardiness of greater than 10 minutes on quiz and exam days will prevent the student from taking the exam or quiz that day.

3 Students are responsible for all class materials and activities regardless of absence. All work and oral presentations are due on date assigned and any missed exam or quiz must be made up prior to the next class meeting. It is the responsibility of the student to make up all missed clinical sessions before the end of the semester. Grading: Tests/Quizzes (approximately 9): 50% Midterm: 25% Final: 25% Clinical Final: Pass/Fail, Re-do fail until student reaches competency Statement on Disabilities: If you have a disability, a medical or emotional issue that might impact on your class performance, please consult with me in the privacy of my office so that the college may provide the required and appropriate accommodations for you. If plagiarism is identified, a conference will be scheduled with the student and the instructor. The student may have one chance to re-write the assignment, based on the discretion of the instructor. Faculty will review written assignments for plagiarism on Blackboard through Safe-Assign. Students may seek help in the writing labs at the Gardner Campus, if needed, with written assignments.
