6 The Ticking Clock Hypothesis Type 2 DM is associated with microvascular and macrovascular complications. Duration of DM and severity of glycemia are primarily associated with microvascular disease. Metabolic disturbances during the prediabetic period may contribute to macrovascular disease. Macrovascular complications: the clock starts ticking years before the onset of clinical diabetes.
