1 City of rocky Mount Wellness incentive Program Why does the City of rocky Mount offer a Wellness Program? Across the nation, health care costs continue to rise which creates a burden f both employees and the ganization. Through the creation of the wellness program, we are hoping to create a me health conscious wk environment where employees are me concerned about their personal health care. By providing free biometrics screenings, health risk assessments, and health coaching sessions, we hope to create an environment where our fellow employees are healthier and happier. A healthier wkfce will assist in reducing overall health care costs f our employees by saving employees money on premiums and out of pocket expenses, and at the same time, reduce the health care expense f the City. Is the program mandaty? The Wellness Initiative is completely voluntary. However, by choosing not to participate you are losing the premium savings being offered by the City in connection with the wellness program. Why should I participate in the Wellness Program? Why not? By participating in the program, you will learn me about your personal health allowing you to take control of any health concerns that may be identified through the biometric and health risk assessment (HRA) screening process. The benefit of participating in the Wellness program provides employees the opptunity to receive premium savings. You can earn premium savings in the following manner: 1) Complete the annual Biometric screening 2) Complete the annual health risk Assessment (hra) The Incentive: Is an Incentive rate structure PPO Rates Base Incentive Only $ $34.94 $ $23.40 and Children $ $ $ $ and Spouse $ $ $ $ and Family $ $ $ $ Page 36
2 HSA Rates Base Incentive Only $ $11.54 $ $0 and Children $ $ $ $ and Spouse $ $ $ $ and Family $ $ $ $ If you are currently receiving the incentive rate, the open enrollment system will assume you will continue at the new incentive rates and vice versa. PPCN will notify the Benefits staff if you meet the standards not. The infmation we receive is very generic. We do not received specific medical infmation. however we will know if john Doe qualified f the incentive rate the base rate. What is being measured in the Wellness program and the biometrics screenings? risk fact Waist Circumference BMI Blood Pressure Cholesterol ratio Glucose/hemoglobin A1c Tobacco The biometric screen and the wellness program attempts to determine the health of an employee. The biometric screening consists of a blood draw, blood pressure readings, and your body measurements. The blood draw is used to determine your cholesterol levels and your A1c levels. The body measurements include your height, weight, and waist circumference. All of these facts are then measured against medical nms to determine if you may have a health risk. Additionally, an affidavit is completed about your use of tobacco related products. It has been determined that many significant diseases have a direct crelation with the use of tobacco products. What will health coaching involve? A health coaching session will be available f those who have an interest in a one-on-one open conversation to discuss your biometric screening. Page 37
3 Who will have access to my biometric results personal health infmation? Nash Health Care will be perfming the biometric screenings and the HRA. The wellness coach is employed by a private firm (PPCN) who is currently a third party provider to the City. The third party provider and wellness coach are the only individuals who will see have access to your personal health infmation biometric results. No one employed with the City of Rocky Mount will have access to the results any other personal medical infmation discussed during the private coaching sessions. All medical infmation will be secured by the Nash Health Care and PPCN and is not accessible to anyone internally. how do I earn the Wellness Incentives? New hires since last screening those who declined in the Spring 2014: By voluntarily participating in the Wellness program in 2015, an employee can earn the Incentive Rate structure f Current participants: To qualify f the Incentive Rates f , the employee must participate in the biometrics and meet 3 out of the 5 moderate control categies meet the Alternative Method. risk fact Waist Circumference Moderate Control Abd Circ < 40 Male 35 female Body Mass Index (BMI) < 30 to qualify Or improve by 5% Blood Pressure < 140/90 mmhg to qualify Or lose 10/5 mmhg Cholesterol ratio 5.1 to qualify Or improve by 10% Glucose (fasting) <100 hemoglobin A1c < 7.0 to qualify Tobacco Tobacco free to qualify Sources: National heart, Lung & Blood Institute, American heart Association & American Diabetes Association NEW - Spring 2015 Screenings: s who meet Waist Circumference/BMI meet the Alternative Method by improving 5% will receive an incentive of $250 in their HSA HRA (Health Reimbursement Account). additionally, employees who do not meet the a1c standard and wish to earn the $250 Waist/BMi incentive, need to enroll and participate in the HealthMaprx program. 4 What if I cannot meet the Wellness Program requirements f medical reasons? If your primary care provider feels that achieving the standards the alternative standards f the incentive rate reward under this program is unreasonably difficult due to a medical condition medically inadvisable f you, please call our representative at PPCN, Ron DeVizia at (919) You will be required to present documentation from your primary care provider. 5 NEW BIoMETrIC SCrEENING CATEGIES EffECTIvE APrIL 2016 Why are we tightening up the standards? When we started the biometric screenings our standards were set very lenient with the intentions of tightening them up after a period of time. This will be our fourth screening and based on our claims and the slight changes noted about, we feel this is the right time.
