2 YOUTH INTERVENTIONIST TEAM BERT PATANIA Supervisor Bert has been with Youth Services for 18 years. He has his M.S.Ed & CADC. ROB SIMMONS Interventionist Rob has been with Youth Services for 10 years. He has his MSW and MPP. Rob s last day with the Township was on February 23, 2016 MELISSA POTRAWSKI Interventionist Melissa has been with Youth Services for 6 years. She has her LPC. Melissa s last day with the Township was on October 21,2016 TIANNA HILL Interventionist Tianna had her Masters in ROB SIMMONS Social Work. Interventionist Tianna s last day with the Township was on November 18,2016

3 OVERVIEW Intensive Case Management Model which provides intense intervention through the following means: ID and Manage immediate crisis Assess environment and risk factors Implement, maintain, follow up, reassess case plan. The strategies and tactics that Interventionists and Youth Services deploy are what the research base recommends as best practices.

11 REASONS FOR REFERRAL Clients often referred for multiple reasons including: History of Client Gang Affiliation 5 History of Family Gang Affiliation 5 History of Violence 27 History of Client Substance Abuse 10 History of Family Substance Abuse 13 Mental Health Note: There has been an increasing trend of youth with co-occurring mental health disorders with Substance Abuse masking other mental health issues primarily, Trauma.

AMANDA MUELLER Interventionist She holds a Master of Social Work from Loyola University Chicago and has worked recently with the Salvation Army, Chicago, as a Substance Abuse Counselor; the Boys Hope Girls Hope home, Evanston, as a Residential Counselor; and the Youth Connection Leadership Academy, Chicago. Interventionists are being trained in Cargo Peace Circle facilitation.
