1 HAND AND HAND WITH TOPS SOUTHERN NEVADA FALL RALLY SATURDAY, NOVEMBER 3, 2018 Dear TOPS Family, Summer is drawing to a close and it is time to start thinking about our TOPS Fall Rally in Southern Nevada. Be sure to mark your calendar for this event, you don t want to miss it. Location for Fall Rally is: GREEN VALLEY PRESBYTERIAN CHURCH 1798 WIGWAM PKWY HENDERSON, NV 9:00 am to 1:00 pm REGISTRATION FEE - $15.00 PER MEMBER Registration begins at 9:00 am and the Rally will start as soon as possible. We will end the Rally earlier this year without a lunch break, and you will be free to eat on your own. We will provide a light healthy snack midway through our Rally. If any chapter would like to volunteer to bring either cut up fruit or cut up vegetables, please contact me. We will only use this as a light snack, so I would like to keep the amount of food smaller to prevent waste. We will provide coffee, tea and water. We will also have a 50/50 opportunity drawing for those members who wish to participate. Tickets will be available to purchase prior to our Rally or at the break. The drawing will take place at the end of the Rally. I would like to ask each chapter to bring a contest to share with the other chapters. Share with other chapters a contest that was fun, inspiring and successful for your chapter. As I visit other chapters I hear about many of their contests that seem to be fun and challenging for all members. I look forward to hearing about your Chapter s favorite. If you have any comments and/or suggestions to make our Rally more successful, please let me know. 2 PARTICIPATION EVENTS FAMILIES IN TOPS Is your family a TOPS FAMILY? If so, plan to introduce yourselves to us. Please bring a 3 x 5 inch card indicating your chapter number and location, the members names and family relationship. You will present your card to the MC when it is your turn to be introduced. Please be sure the following information is printed on your card. Chapter TOPS # NV: Family Name: Number of Participants: Names and relationship: (Mother, Father, Husband, Wife, Son, Daughter, Sister, etc.) Groups, from our KOPS Society can also be introduced at this time. SRD FUND BASKET RAFFLE Each chapter attending the Fall Rally is asked to bring a basket valued at $30.00 or more to help raise funds for SRD (State Recognition Day). If your chapter is not attending the Fall Rally, please send a chapter check (made payable to Nevada TOPS Rally Fund) or a cash gift card in the amount of $ by the registration deadline. TOPS #NV, Will provide a basket or cash gift card valued at $30.00 or more to be raffled at Fall Rally OR Has enclosed Chapter Check # as our donation for the Nevada State Recognition Day 2

3 DOOR PRIZE RAFFLE Each chapter attending the Fall Rally is asked to provide one or two new door prizes with a value of at least $5.00. These door prizes will be raffled during the rally. Each member attending the Rally will receive one ticket for the door prize raffle. TOPS # NV, Will provide at least one door prize valued at $5.00 or more for the Nevada TOPS Rally fund Sorry, we will not be able to provide a door prize. 3

4 OFFICERS WALK THE TALK RECOGNITION 2018 FALL RALLY RECOGNITION All current active members who were chapter officers for the 2017/2018 year and attend Fall Rally are eligible to participate in this contest. We will be recognizing each officer with the most weight loss in their position. This will culminate in recognition of the chapter board with the most weight loss per TOPS member on the board THE RECOGNITION TIME FRAME IS FROM AUGUST 1, 2017 TO JULY 31, 2018 Use the first weigh-in on or after August 1, 2017 and the last weigh-in before or on July 31, The starting date for officers elected mid-term use the date their appointment became effective. Members that were not KOPS for the whole year are credited or charged with their loss or gain before they made or lost KOPS status. DEADLINE TO SUBMIT THIS FORM IS OCTOBER 19, 2018 IF FORM IS NOT SUBMITTED BY THE DEADLINE YOUR OFFICERS WILL NOT BE ELIGIBLE. Office Member s Name Attending Rally? 5 Chapter Angel Is there a Special Someone in your chapter who: Calls you when you are down? Cares when you are having a bad day? Guides you when you don't know how? Picks you up when you are in a rut? Writes when you need encouragement? Applauds you when you deserve some praise? Inspires you when you need it most? In other words, is there a Special Someone in your chapter who has made your TOPS world a whole lot brighter? Each chapter attending Rally is encouraged to name its own Chapter Angel. The Deadline date is 10/19/ Fill out this form and place in envelope with the following: 2. Vote by secret ballot. (Consider all members, not just those attending Rally) Write both first and last name on the ballots. Area Captain will count your ballots. Place all ballots in the envelope (Do not count or peek) and send them to the area captain. Criteria: At least one member from the chapter must attend Fall Rally to accept the award if the winner is not in attendance. Chapter Angel award is a onetime honor. 6 FALL RALLY WEIGHT LOSS CONTEST MUST BE POSTMARKED BY 10/26/2018 List all members with a weight loss starting with the first weigh recorded the week of August 20, 2018 and ending with the last weight recorded by October 26, I realize this only for a ten week period, but let s see how successful each of us can be during this time period.
