1 Tuolumne County Senior Network May 12, 2011 Draft Meeting Notes UPCOMING EVENTS Next Senior Network meeting, 8:30am Thursday, June 9, Area 12 Agency on Aging, Standard Road, Sonora. Speaker: Deanna Maurer, Veterans History Project. OLD BUSINESS Minutes for 4/14/2011 approved Cathy Peacock: The Commission on Aging is seeing candidates for Volunteer of the Year. The nomination deadline is June 10. It s a simple form (attached below) and please include the county supervisor s district if you know it, so that the appropriate county supervisor can present the award. On Oct. 19 the Commission will hold a centenarian even for those 100 or older. They are also looking for folks 96 years old plus. Those 96 are considered freshmen, 97 is sophomore, 98 is a junior, and 99 is a senior. For either event, contact Cathy at NEW BUSINESS Joan Jackson: Calaveras Senior Network is interested in holding a joint Tuolumne/Calaveras Senior Network meeting to discuss ideas for adult health services. Joan will help set up a time and place for the meeting. Ira Uslander: Ira is impressed with the outpouring of interest that the closure of Adult Day Health has generated. They are looking at ideas for bringing it back in Tuolumne County in some form. SPEAKER Ron Hamilton of Tuolumne County Behavioral Health Ron described the wide range of services and programs currently offered by Behavioral Health despite the cuts in funding and staffing in recent years. His presentation is attached. Peter Carrillo thanked Ron for the amazing work and service that department offers residents of the county. ROUNDTABLE DISCUSSION Helen Sparks, Faith Community Nurse Coordinator, Sonora Regional Medical Center It has been business as usual. Beki Medina, intern, works with Ron Hamilton at Behavioral Health and Peer Counseling, graduate social work student She and her husband operate nonprofit We Care Sober Living Recovery Homes for people with addictions, hard-core cases. 2 Jim Sells, Catholic Charities HomeShare Project He recently took a mediation program which he would recommend for anyone working with teens or seniors. They are trying to decide how they might implement the information from the program. The Senior Conference on May 4 was well attended. Home Share Program: There are lots of people who need places to stay and may even need some caregiving. They don t have much money. He s looking for ideas to help with the home matching process Cynthia Halman, Sonora Regional Hospice They offer services to patients and families during a difficult time of life. They are currently looking at providing music therapy to help patients and their families with anxiety. She has been involved with work on the issues of senior suicide. There is a task force trying to set up interventions. Suicide prevention training will take place in July and October each training is two days. It s pretty intense, she says. Participants will learn intervention and assessment skills. Contact the Center for Nonviolent Community at The number of participants is limited. So far they have trained almost 50 people and they are trying to train as many as possible of those who work with seniors. Katryna Hunt, Personal Care They are taking new clients and adding staff. Melody Roberson, Sonora Regional Home Health Their office has been busy. Kim Moss now working with them in Tuolumne County. Linda Happel, Senior Peer Counselor Coordinator, Behavioral Health They have been getting additional peer counseling referrals with the closure of Adult Day Health. Peter Carrillo, Practical Dreamer He and his partner/wife Joan facilitate Family Caregiver Education and Support Groups sponsored by the Area 12 Agency on Aging. They currently have a wait list of about 20 for the groups. They hope funding will continue to be available next fiscal year. Groups will be eight instead of 12 sessions next fiscal year to allow more caregivers to participate. They are also doing trainings for professional caregivers in local facilities. Christine Miller, District Attorney Victims Services Unit They are staying really busy if anyone needs help, call them at Cathy Peacock, Interfaith Cathy asked that each member of the Senior Network provide a one-page description of services, to be included in the Interfaith resource binder. Interfaith is seeing a swelling number of seniors who have worked all their lives, but have been devastated by the downturn in the economy. She is concerned with the despondence and melancholy among this population. We re constantly trying to refer these people.
3 The County has put together a task force do free debris removal from this year s winter and spring storms. The program is set up to assist those age 60 or older. Call Area 12 Agency on Aging at or Catholic Charities at Ira Uslander, Seniority Lifecare at Home They provide professional caregivers to keep people safely in their homes. They recently certified their first four hospice care aides. Seniority is about to launch a non-medical mental exercise program. It s a twice-a-week program to improves cognitive ability in early to moderate stages of dementia. It is non-invasive but can be used along side medications. The cost is approximately $100 week for cost of facilitator and materials. They are willing to partner with Medicare providers. For more information, call them at Judy Finley, Senior Resource Service, Senior Resource Services is 100 percent volunteers and they have more volunteers than ever before, however, they also have more requests for service than ever before. They currently have a wait list. Tickets are now on sale for the SRS annual fundraiser. Tickets are for eight chances at dinner for two at Black Oak Casino. For information, call Judy at Gracelyn Evans, Senior Peer Counseling She s been busy and is still doing in-home assessments of program clients. Kristin Millhoff, Area 12 Agency on Aging They are gearing up for the end of their fiscal year. There are rumors that the state budget may push out into November this year rather than October. The MSSP Program is running, but they have had cuts. The Family Caregiver Program is getting more and more calls for respite, equipment and caregiver groups. The Agency is sponsoring tai chi classes which are filling up fast. The recent A12AA needs assessment surveys have had about a 25 percent return rate, which is very high. 7 Substance Abuse Services Judicial /Perinatal Services Adult Drug Court, Prop 36, DEJ- ( Collaboration between court system probation and substance abuse counselors) offers the offenders the opportunity to enter drug court treatment system instead of jail. Dependency Drug Court ( Collaboration between Child Welfare services, the court system and substance abuse counselors) offers families alcohol and drug treatment programs to reunify with their dependent children. Perinatal Treatment Program: Volunteered based specialized alcohol and drug treatment for pregnant and/or parenting women
8 Adult Services Dual Diagnosis Treatment: Counseling for clients who have alcohol and/or drug problems in addition to a mental health disorder. Impulse Control Disorder Groups These are 16 week gender specific groups designed for individuals who have not been able to resist a behavior which may have been harmful to either self or others. These behaviors or actions may have been violent, sexual, gambling, fire starting, stealing or self-abusive behaviors.
9 Crisis Assessment Intervention Program Call or Walk in Clinic Offers a team of clinicians and recovery counselors that respond to crisis prevention or emergency support and referral services 24 hours/7 Days a week: Telephone and face-to-face intervention for support or crisis intervention Onsite evaluations at Sonora Regional Medical Center Help in connecting to community resources Follow-up appointments Assessment and arrangements for psychiatric hospitalization if needed On-site evaluations and services at Tuolumne County Jail Services available to the community
10 DAVID LAMBERT DROP - IN CENTER In Operation since 2000 Volunteers and TCBHD staff coordinate a peer recovery program that serves an average of 50 mentally ill individuals on a daily basis. Provides expanded services to the: Severely Mentally ill population Homeless Elderly Offers food/showers/blankets/support groups PRIDE Group Peer Recovery: Independent Development and Empowerment. This group fosters leadership development, extended recovery and skills to live independently with a high quality of life, and supported by clinical staff as needed
11 Senior Peer Support Program Senior Peer Counseling Program, a volunteer based program, offers services to anyone 55 or older who lives in Tuolumne County and is troubled by: Loneliness Depression Grief Loss of any Type Isolation Coping with Illness At Risk for Alcohol Abuse At Risk for Chemical Dependency At Risk for Medication Management For help encouarge Seniors to call A Helping Hand In A Time Of Need
12 Services For Children Collaboration with ICES provides Nurturing Parenting education classes and counseling for parents at risk Tuolumne County Behavioral Health Individual and family therapy targeting children and Transitional Age Youth Group counseling Dialectical behavior therapy. This therapy helps a person learn emotional regulation skills. Both boys and girls from ages 14-17y/o attending. Art therapy groups provide for therapeutic and artistic expressions of the client s behavioral health issues as well as positive socialization. Juvenile probation drug and alcohol, and anger management groups
13 Services For Children AB3632 program- Offers Behavioral Health services to children who qualify for Special Education services and have emotional problems Children s Med Services - The client s are provided a psychiatric evaluation from the psychiatrist and a treatment recommendation. Nursing services are provided to monitor the medications that may be described or side effects, compliance to the prescription, and effectiveness of the medication. Parent Cognitive/Behavioral skills class and counseling in-home counseling and behavioral intervention Immediate assessment for foster care crisis home placements

15 Community Services and Support Full Service Partnerships (FSP): TCBHD staff provides support and wrap around case management services for up to 24 individuals in our county. The target populations includes all ages who have a serious mental illness or severe emotional disorder. Catholic Charities Contracted Services for the last three years providing outreach and support to the older adult population based on highest level of needs specific to services. Tuolumne Me-Wuk Indian Health Clinic A contract services providing outreach and support to Native American youth and their families specific to mental health and substance abuse problems. Benefit Development: TCBHD staff provides part time services to assist mentally ill residents in applying for and obtaining public benefits such as Medi-Cal and SSI.
16 Prevention and Early Intervention (PEI) Center for Non Violent Community Funds used to provide suicide prevention education And Bullying prevention programs Infant Children Enrichment Services (ICES) Funds Provide nurturing parenting classes and positive parenting classes. ATCAA- Challenge Days/Friday Night Live Provides funds to implement bullying and violence prevention program Lilliput Children s Services Provides Foster Care /Adoptive Parent Support and Education classes Tuolumne County Office of Education Funds provided help support School Special Education Friendship Program

18 Supportive Housing TCBHD has received funding to purchase and build two permanent supportive houses for severely mentally ill residents includes up to four bedrooms each, with handicap accessibility. These two houses will be close to Sonora, as well as close to public transportation. The houses should be built and ready for occupancy by Summer, 2011.
19 INNOVATION Purpose - Interagency Task Force to improve & strengthen coordination among agencies and community representatives who are involved with long term out of county residential placements for Tuolumne county severely mentally ill residents. STAFF - Three Case management staff will work to reduce the number of first time and repeat placements. Case managers will assess clients during home visits identify symptoms, implement early support and avoid hospitalization. Funds may be available to help subsidize housing and transportation costs.
20 Capital Facilities & Technology Capital Facilities and Technology - Funds were available to remodel TCBHD s Walk in Crisis Center, as well as other mental health facilities. Technology funds are available to provide the necessary technology to support mental health services.
