In my four short years on the Washington state delegation, I have had the opportunity to have excellent mentorship and support in order to perform well in this leadership position. I have held many leadership positions, starting as a young junior high student being involved in ASB leadership, throughout college, PT school and in my career. I want to continue to serve the physical therapy profession and APTA in the best way that I can. These great leaders lit a flame in me for the excitement and need to be involved in the growth and unity of our profession. Serving others makes me happy. I feel with my experience at the APTA House of Delegates and on the PTWA Board of Directors, I can represent the PTAs for the State of Washington with an open and objective mind to better the profession.

7 Nominating Committee Amanda Scharen, PT, DPT, OCS, CSCS Creighton University, DPT Gonzaga University, BS, Exercise Science Therapeutic Associates Ballard Physical Therapy, Staff Physical Therapist APTA Member Since 2009 PTWA Delegate, APTA House of Delegates, 2015-present PTWA Reimbursement Committee, 2015-present PT Pub Night, Seattle-area Host, 2014-present Greyhound Pets, 2015 to present American Academy of Orthopaedic Manual Physical Therapists, 2016 to present I would like to thank the PTWA Nominating Committee for this opportunity to run. I believe that involvement in our state professional association is an integral part of moving our profession forward and promoting physical therapy to the public. I remember what it was like to want to get involved in PTWA and the APTA, but having no idea where to start. I would like to serve on the PTWA Nominating Committee to help find those people who want to be involved. Having hosted PT Pub Night in the Seattle area for over two years, I have widened my network of wonderful PTs, PTAs and SPTs and have had many conversations with my colleagues across different practice settings about what involvement in the APTA and PTWA can look like. Encouraging and empowering others to get involved in our profession is an exciting responsibility to me. Thank you for your vote!

8 Susan Chalcraft, PT, DPT Evidence in Motion, tdpt Evidence in Motion, Certification in Practice Management University of Washington, MS, Rehabilitation Medicine University of Washington, BS, Physical Therapy Rehabilitation Services Manager, Providence Mount Carmel and Providence St. Josephs Hospitals APTA Member Since 1981 PTWA Delegate, APTA House of Delegates, 2007-present PTWA Nominating Committee Chair, 2017 PTWA Nominating Committee, 2016-present WPT-PAC Secretary, 2011-present APTA Advocacy Awards Committee, 2016-present APTA HPA Management Course Committee, Chair, 2016-present APTA Rural Health Care Forum Group, 2017-present PTWA Legislative Committee, PTWA Delegate to the APTA House of Delegates, PTWA Dry Needling Workgroup PTWA Mentoring Task Force APTA Graham Sessions Planning Committee PTWA President, PTWA President Elect, PTWA Vice President, PTWA Nominating Committee Chair, 1996 PTWA Nominating Committee, PTWA Legislative Committee Chair, PTWA Public School Guidelines Task Force, , 1987, 1991 PTWA Public School Liaison, APTA PT Journal Editor-in-Chief Search Committee, APTA Models of Service Delivery Task Force, APTA LAMP Administration and Management Sub-Committee Susan Chalcraft continued on next page

9 Susan Chalcraft Continued Colville Rotary, 2017-present Honors and Awards Washington Society of Association Executives Volunteer in Association Service Award, 2012 PTWA Donna El Din Distinguished Service Award, 2012 APTA State Legislative Leadership Award, 2005 PTWA Physical Therapist of the Year,1999 I will continue to be effective in the position of delegate because of my passion and experience in association governance, because I represent a practice area (rural health care) that is under-represented in the APTA House of Delegates, and because of my ability to mentor newer delegates and leaders. We are entering into exciting times as a profession. The APTA board has been charged with updating our mission as an organization. The house is reviewing and consolidating house policies. In addition we are working to define our scope and who we are as a profession and will be anticipating and responding to changes in health care. As a delegation we will have multiple new delegates and a new chief delegate. It is important to have delegates who are open to organizational change, but also to have some delegates who also have a history in APTA governance. It is also important to have delegates that represent a diversity of practice settings so that we can consider impact of changes on all settings, not just on the most common ones. Finally it is important to have seasoned delegates who will be able to assist our new chief delegate in their new leadership role and who is able to mentor newer delegates who are new to the process of APTA governance. I have been able to successfully perform all of these roles in the past and would like the opportunity to continue in these roles in the future. I thank you for your vote.

The last three years I also served on the APTA Public Policy and Advocacy Committee providing strategic advice, counsel and options to the APTA Board of Directors regarding APTA s response to current and emerging public policy issues impacting the profession, societal needs, and health care while advancement of the profession within public policy arenas. I am a new professional entering the field with a diversity of experience. As a student, I was heavily involved in representing the needs of fellow students, both in Washington and across the country, including assisting at the 2016 APTA House of Delegates. Prior to my re-entry to academia, I spent nearly six years in various roles within the public affairs and governance of a HMO. That work was centered on supporting state and federal legislative and regulatory work. I also have extensive customer service and administrative skills, specifically in organization and communication on behalf of executives and board members. I believe my unique set of skills would be beneficial to the role of Delegate, as well as allow me to continue to engage with this vibrant member association!

18 Daphne Ryan Continued The position of delegate represents you, the member, at the highest level of governance within APTA. I thank you for your confidence to serve you in the past. The APTA House of Delegates takes action to direct the future of our profession. This coming year, we may be discussing if sections have a vote in the house. I would like to be involved in that discussion. Our delegation is known for its diligence and thoughtfulness. My position as a direct access PT within the medical clinic at Naval Hospital Oak Harbor provides a unique flavor to our conversations. Implementing PT services within 24 hours for our military, where pain is relieved without opioids or surgery, is invigorating. I want to bring this unique perspective to the discussion table. My history of PTWA and section service gives me discernment regarding PTWA s cultural needs at the grassroots, state, and federal levels. I look forward to supporting Jenny Jordan as she follows the extraordinary leadership of Jennifer Lesko as Chief. Having walked Jenny s journey as an emerging leader myself, I look forward to supporting her at the helm. I humbly ask for your vote.

19 Amanda Scharen, PT, DPT, OCS, CSCS Creighton University, DPT Gonzaga University, BS, Exercise Science Therapeutic Associates Ballard Physical Therapy, Staff Physical Therapist APTA Member Since 2009 PTWA Delegate to the APTA House of Delegates, 2015 to present PTWA Reimbursement Committee, 2015 to present APTA Orthopaedic Section Member PT Pub Night, Seattle-area Host, 2014 to 2017 Greyhound Pets, 2015 to present American Academy of Orthopaedic Manual Physical Therapists, 2016 to present Thank you to the PTWA Nominating Committee for considering me. As a new delegate, I was greeted by a steep learning curve and am ready to employ my past experience to serve for another term. One of the most exciting moments of my career as a physical therapist was arriving at the APTA House of Delegates for my first term in Nashville. It was amazing to see how this body drives the APTA and our profession forward, and I hope to continue to serve as an advocate for how our profession advances in the future. Our health care environment is ever changing, and through sitting down with other PTs and PTAs at PT Pub Nights, I have learned about the issues and concerns that our profession has in a wide range of practice settings. By communicating with people who work across the lifespan and in different practice settings, I feel I am an excellent candidate to represent PTWA to the House of Delegates. I would be honored to serve as a PTWA Delegate again. Thank you for your consideration.
