For further information about the benefits of becoming a CANSW member please visit our website Our Conference Theme: Demographic and economic trends are increasingly at odds. The population is aging. Diabetes is on the rise. Economies are shrinking. Political attitudes are evolving so that we hear less about the social safety net, and more about how government services cannot be all things to all people. It is becoming routine that we are providing pre-dialysis care and renal replacement therapy for people who have dementia. Dementia makes providing services more complex. Dementia is not well understood among laypeople, policy-makers, administrators and frontline healthcare workers. The tsunami is coming. Governments and institutions are preparing by revising policies and programs to stretch resources to meet more needs, but also to protect budgets. How are social workers to respond to these trends? Knowledge is power, and any social worker with a good grasp of these emerging trends will be well positioned to better assist his or her clients and employer to cope with the rising tides. 5 Keynote Speaker Dr. Barbara Liu, MD, FRCP(C) Geriatric Giants: Dementia, Delirium, Falls & Polypharmacy (What s a Renal Social Worker to Do?) Dr. Barbara Liu is the Executive Director of the Regional Geriatric Program of Toronto and an Associate Professor of Medicine at the University of Toronto. Her areas of interest include the appropriateness of drug therapy in the older patient, fall prevention and geriatric service evaluation. She has a clinical practice at Sunnybrook Health Sciences Centre. Keynote Speaker Dr. Vanita Jassal, MSc, BChir, MB, MD Dialyzing the Elderly: Too Soon? Too Late? To What End? Dr. Vanita Jassal is currently a Staff Nephrologist at the University Health Network and Associate Professor at the University of Toronto. She has training in clinical nephrology, geriatric medicine, and clinical epidemiology. Her current academic interests include: changes in functional status with ageing and renal disease; falls and fall-related morbidity in dialysis patients; and the management of seniors with chronic kidney disease. She has an active interest in all aspects of care of seniors with advanced kidney disease and those who are established on renal replacement therapy. She is currently also the Director of a specialized rehabilitation program, which targets disability in seniors on dialysis, in Toronto. 5
6 Conference Speaker: Sandra Loucks-Campbell, PhD Social Workers Making a Difference for Older Adults An expert in the field of eldercare, Loucks-Campbell has provided training, research and service for elders, their families and their organizations over more than 20 years (advising government, developing programs and professional teams, individual and family mediation and counselling, evaluation & more). She has written book chapters, published papers and developed post secondary and graduate courses, keynote addresses and interactive presentations (online, face-to-face) on aging related themes. The detailed results of her research add richness to her work and to what she is able to offer the people she serves. Sandra is known as a compelling speaker and workshop leader sought by interested community groups, professionals and undergraduate and graduate students alike. Participants in her courses and workshops enjoy her interactive and challenging teaching style and her contagious love of learning. Commonpoint provides the venue which allows her to transform her strengthbased vision in programming for older persons and families into action. In today s social and health care systems this new vision of aging offers refreshing, invigorating care options for professionals who assist Canada s elders. Dr. Campbell received her MSW degree from Wilfrid Laurier University (1985) and more recently her PhD (WLU 2002). sandra@commonpoint.ca Her presentation will be presented at a graduate level but accessible to all through the use of an interactive, challenging and strengths-based approach. Providing care for older adults within the health system can be one of the most challenging roles faced by professional social workers. How can we help older adults engage with health care providers while holding fast to their life s meaning and value? This interactive event provides an opportunity to examine elder care within its multidisciplinary context. Why has reaching advanced age become a societal problem when one might expect it to be a success story? Participants will learn why and discover methods they can use in their practice to rewrite this story. Participants will leave the workshop feeling invigorated by the facts and with a fistful of useful strategies ready to make the system better for older adults. What better challenge can there be than to help someone navigate through challenging waters to find a life they want to live, one that enriches the last decades of their life? 7 Conference Speaker: Gale Elliot, BASc, MA Montessori Methods for Dementia: The Rules of Engagement Behavioural issues associated with dementia are often associated with boredom and loneliness. When individuals are engaged in life and living, at their own unique level of ability, these "responsive" behaviours disappear (assuming other medical issues have been ruled out first). When needs, interests, skills and abilities are taken into account in dementia programming there is great potential for maintaining, and even improving, function. This session will explore the "Rules for Engagement" using the Montessori Methods for Dementia as a framework for supporting both the person and environment with the purpose of enhancing quality of life. Gale Elliot is a Gerontologist and Dementia Specialist with DementiAbility Enterprises Inc. CANSW Member Speaker: Lyle Stockwell, MSW, RSW Durham Region, ON KDQOL36 Snap-shot of a Large Pre-dialysis Clinic At Lakeridge Health, the KDQOL36 quality of life survey is being administered on approximately 600 pre-dialysis patients. The survey is designed to assess well-being, and the resulting data can be stratified to identify those patients at higher risk of hospitalization or death. This helps social work target higher levels of intervention to the caseload portion identified as most in need. Presentation of the results will provide a good snap-shot of the caseload. Lyle Stockwell has been a renal social worker at Lakeridge Health for 14 years following 5 years as Patient Services Coordinator for The Kidney Foundation of Canada, Central Ontario Branch. He has previously held the position of Treasurer for CANSW, is a previous recipient of CANSW s prestigious Jane Dicks Award and is currently the editor of CANSW s newsletter, the VOICE. 7
8 CANSW Member Speaker: Gary Petingola, MSW, RSW Sudbury, ON Out of the Grey - Exploring the Impact of a Relaxation Therapy and Mindfulness Meditation Workshop for Nephrology Social Workers on Professional Practice Relaxation therapy techniques and mindfulness meditation are clinical intervention tools that have demonstrated benefits associated with reduction of physical and emotional suffering. These practices are well supported in the literature and are considered to be evidence-based interventions. Many mind-body therapeutic training workshops are offered in a one-day format; however, there is a gap in the literature examining the direct impact of learning relaxation therapy and mindfulness meditation specific to nephrology social work practitioners, and how, if at all, learning these techniques might assist a practitioner in his/her work. This study examined the influence of a one-day educational workshop on relaxation therapy and mindfulness meditation on nephrology social worker professional practice. Eight members of the Canadian Association of Nephrology Social Workers with limited previous exposure to mindbody therapeutic approaches took part in the project and were interviewed. The key findings from this study suggest that (1) the workshop had an impact on professional practice, (2) the workshop had an impact on self care, and (3) amid obstacles to implementation, nephrology social workers wanted to expand their current scope of practice to include relaxation therapy and mindfulness meditation with their clients. Gary Petingola holds the position of Nephrology Social Worker at the Hôpital régional de Sudbury Regional Hospital in Sudbury, ON. He has made many valuable contributions to the field of Psychosocial Nephrology recently completed his Masters Degree in Social Work at Laurentian University in Sudbury, ON. Gary has done extensive presentations on relaxation to hospital colleagues including at the Laurentian University School of Social Work and at the American National Kidney Foundation. He has numerous prestigious awards, has been involved in many projects, initiatives and organizations and is currently the Canadian representative on the Kidney Community Emergency Response Coalition USA overseeing the Provision of Dialysis in Pandemic and Disaster. He authored an article entitled Mindfulness Awareness and Its Influence on the Health Care Professional in a Nephrology Setting in the Journal of Nephrology Social Work. Gary is also a Previous CANSW President and Vice President, a previous Jane Dicks Award winner and is currently CANSW s web manager. 8
9 CANSW Member Speakers: Corinne MacNab, MSW, RSW Orillia, ON Bryn Milner, BSW, RSW Huntsville, ON Corinne MacNab Bryn Milner Truth Telling: The Social Work Role in Informing the Elderly about Conservative Care It has become more apparent over the years that a lot of what we do with our patients is palliative care, preferably explained as supportive care of a person living with a life threatening illness. As anyone involved in palliative care can tell you, it becomes a passion. As a Regional Social Work Group, it has become a team goal to continually improve our Advance Care Planning resources and skills. One strategy is with social work involvement committees within the hospital trying to advocate for a palliative care unit at Soldiers Memorial, and also with community forums with our Palliative Care Network to promote Advance Care Planning in the community. The focus of this presentation is truth-telling and making sure patients and families know all of their options for care, including no treatment. If managed well in Kidney Care Clinics, these patients may enjoy a better quality of life than on dialysis. How comfortable are we in talking to older seniors about conservative care, informing them of all their options and all of the potential effects of treatment? Corrine MacNab has worked in renal social work for almost 14 years at Soldiers Memorial Hospital in Orillia. Her focus has been mainly with hemodialysis patients, in-centre and in satellite, and more recently with Kidney Care Patients. She is a previous Jane Dicks Award winner. Bryn Milner has worked, part-time, as a Nephrology Social Worker for the last 15 years at Huntsville Hospital. Her unit is a satellite of Orillia and she states that she has been lucky enough to work with this unit since the beginning. It has been quite the journey watching this unit grow, change and develop! Over the last 6 years her role in the hospital has expanded to cover other areas (med/surg; OB; palliative care) and covering ER and ICU as needed. This expanded role has allowed her to follow the dialysis clients whenever they are in hospital. Five years earlier, she joined the Algonquin Family Health Team 2 days a week working with the Palliative Care Team (Hope Huntsville). She visits clients in their home (be that a house, apartment, RH or LTC facility); hospital or residential hospice. Palliative care has always been a passion for me and she considers herself very fortunate to work with such a team in this community. She finds that the 2 jobs blend beautifully and permit her to provide Social Work support across the continuum of the client s journey. She tries to balance this out by spending time with her family; kayaking, reading and creating as time allows. 9

Marie, ON Getting There isn t Half the Battle, It IS the Battle: A Social Worker s tale of Advocacy in Transportation for Transplantation Prior to 2009, all Northern Ontario deceased donor kidney transplant recipients were transported to the transplant centres via the Ministry of Health due to distance and time factors. A reinterpretation of policies brought about a sudden change in practice, complicating an already complex, anxiety filled race to receive the gift of life, frustrating the health care professionals involved, challenging those patients awaiting a kidney and leaving those patients who were most vulnerable, (those under the low income cutoff), in a precarious position. Hear about the persistent advocacy efforts of two Northern Ontario renal programs who refused to take no as the answer. Who continue to challenge the status quo and hope to close a newly developed gap as the province moves towards a one kidney transplant list. And maybe by the time this conference occurs, there will be momentum towards equal access for transplants for all. Shirley has worked for 22 years at the Algoma Regional Renal Program, with dialysis, clinic and transplant patients. Involved with CANSW in many capacities, including Past President during her reign of terror; with the Kidney Foundation of Canada Greater Ontario Branch as a Patient Services Committee member, as the Regional Representative from the Kidney Foundation for LHIN 13 on the Ontario Renal Network s (ORN s) advisory committee and most recently as a Director at Large for the Ontario Branch of the Kidney Foundation of Canada. She also has continued her commitment with a local Accessibility Committee involved in transportation for the disabled in the community. She believes that to truly make a difference, you have to step out of your comfort zone and get involved. Be the change you wish to see. And don t wait for someone else to do something about it. 10
