2 Get Started on Your Journey 2018 WELLNESS INCENTIVE PROGRAM 2018 Biometric Screening and Online Wellness Assessment completed before August 31, 2018 Assess Make a Wellness Plan Activate Accelerate Achieve TOTALS (New hire prorated) Complete 2018 Biometric Screening and Online Wellness Assessment Plan $ = 30 $150 $350 $750 (New hire prorated) This unlocks the ability to earn incentives! Plan $ = 30 $150 $350 $750 CREDIT ACTIVITIES 1 Point Per Activity 3 Points Per Activity 6 Points Per Activity Asset Health online courses Wellness Fair Blood donation (once per year) Immunizations (once per year) Group classes/lunch and learns Community volunteer hours (4 hours minimum/twice per year) Additional employee information gatherings as scheduled, such as Bring Benefits to You or Jump Start Your Day with Wellness For a full list of activities, visit gcbewellbesafe.com Preventive screenings Meet with Empower Retirement representative Two telephonic coaching sessions (at least 30 days apart) Achieve your goals, like: Steps Strength training Cardio Stretching Water intake Servings of fruits and vegetables Asset Health sponsored challenges Tobacco cessation programs Weight Watchers/other weight management participation Serve as a Wellness Champion for a minimum of six months Two Onsite Wellness Coaching sessions (at least 30 days apart) SET GOALS TRACK PROGRESS EARN POINTS

3 Get Started on Your Journey Biometric Screening Your biometric screening gives you a snapshot of your health to help you make the best decisions for your well-being. Wellness Center Screening: Call to schedule an appointment. Onsite Screening Events: Schedule an onsite opportunity at gcbewellbesafe.com during the year. Your Primary Care Doctor: Take the Physician Screening Form, available at to your primary care doctor. Send it back to Asset Health to be uploaded to your account. Online Wellness Assessment Take stock of your health and fitness practices by taking the Online Wellness Assessment at gcbewellbesafe.com. Complete these two steps by August 31, 2018! (New hires must complete within 60 days of benefit elegibility.) Plan Your Journey Make a Wellness Plan Your plan is developed with a wellness coach using your 2017 or 2018 biometric screening and online wellness assessment results to set your health and wellness goals. How do I make a plan? Schedule an appointment to build your plan with one of the following: Wellness Center Clinician: Call to schedule your appointment. Onsite Wellness Advocate: Make an appointment online at gcbewellbesafe.com under the Events and Classes tab. Telephonic Coaching: Log into gcbewellbesafe.com to view your coach s contact information to schedule an appointment.

4 Track Your Journey Set Goals On gcbewellbesafe.com, you can set and track multiple fitness and nutrition goals in order to receive points. In addition to the goals you ve set with your coach, you can set your own goals to help you earn points in different areas. You can set goals for activities, like steps, strength training, cardio, sleep, and more. Track Progress Once you ve set your goals, you can begin tracking, or journaling, your progress. Some activities will require documentation, which can be uploaded directly to gcbewellbesafe.com. Earn Points As you complete your goals, you will earn points towards Activate, Accelerate, and Achieve incentives. How can I track my progress? Website Journaling After setting your goal, you will track your progress for four consecutive weeks. If you complete the goal, you will earn the points. You have a 10-day grace period to log your activity. Once you complete a goal, you can challenge yourself to complete the same goal again or set a new goal. The site is also mobile friendly, so you can track your progress on the go! Fitness Trackers To make tracking as easy as possible, you can link certain fitness trackers to Asset Health to have your data automatically loaded into the system for goals like steps, cardio, or sleep. For continuous tracking, reset your goal in gcbewellbesafe.com every four weeks. Compatible trackers include Fitbit, Jawbone UP, Polar, Adidas, and Under Armour. Visit gcbewellbesafe.com to view a full list of trackers and instructions on how to link your device to Asset Health.
