In the next two months, we will complete our selection of a Lead Practice in all fifty states that utilize the system to help patients. The Lead Practice will benefit significantly by providing its patients a numerical score of their carotenoid antioxidant levels that have been shown in more than 1,000 peer reviewed, published studies to reduce oxidative stress and the resultant periodontal disease. Your time investment is only a few minutes daily, usually to answer a communication from a colleague interested in the process. 2 Business Proposal Description: The Foundation of a Healthy America is seeking a business relationship with Great Hill Dental Partners, LLC in Project Dental America, as the Lead Practice in Massachusetts. The preventive dentistry initiative of Project Dental America is designed to reduce the frequency and severity of periodontal disease. The procedure, market analysis, and financial benefits of the project are included in this document. Executive Summary : The purpose of this proposal is to forge a strategic relationship between The Foundation and Great Hill Dental Partners, LLC to introduce a sixteen year old technology to general dental practices throughout the state.. Great Hill Dental Partners, LLC utilizes the S3 Antioxidant Scanner program: 30 second scan of the hand offered to daily patients Provides Numerical Value of antioxidant tissue density Automatic monthly shipment of product(s) to the customer Free Support from The Foundation of a Healthy America Support and commission payments from Nu Skin Enterprises, Inc., a publicly traded, multibillion dollar company operating in 54 markets There is a window of opportunity to become the Lead Practice in Massachusetts for Project Dental America to enhance the quality of care for the patients of Great Hill Dental Partners, LLC and develop a very significant source of continuing income.

3 Project Description : The project will be completed as follows: Phase 1 : Qualifying Purchase of Scan Cards, ipad & Product Display = $6,000 Submission of Lease Payment for $125 monthly Phase 2 : Staff Training Phase 3 : Office Operations Phase 4 : Building the Network by the Foundation - not the Lead Practice The Foundation of a Healthy America provides support at no charge ever for all phases of helping the practice(s) effectively utilize the antioxidant scanner. After two months of successful operations, the foundation will begin directly contacting every general dentist in the state to utilize the scanner in their individual practices. All dentists in the state so agreeable will be placed in the network of Great Hill Dental Partners, LLC which will receive lasting commission payments of 5-25% on all Nu Skin Enterprises, Inc. products sold. Sales and Marketing Strategy: Target market for the business is: Dentists in General Practice in Massachusetts Every one of these dentists may desire to participate in Project Dental America After 60 days of successful office operations of the scanner, the foundation will present the program monthly to general dentists in the state. This presentation will originate with a direct telephone call by us to the dentist and will include web links of scientific validity, videos, etc.

4 It is expected that a minimum of one dentist will adopt the scanner each month beginning with the third month following successful scanner operations in the office. This dentist will be placed in the network of Great Hill Dental Partners, LLC which will receive a monthly commission check from Nu Skin Enterprises, Inc. of 5-25% of the value of all product(s) sold in the practice and the network. The foundation will provide the same support for each of these dentists... Financial Summary Great Hill Dental Partners, LLC is expected to generate a very significant income from this endeavor. Income is from office and network operations, with the four assumptions noted below: Assumption One : Practices using the antioxidant scanner will charge $20 for each new scan and will perform an average of four scans daily for twenty-one days each month; Assumption Two : One fourth of persons scanned will begin a Guaranteed Product with an average cost of $80 monthly; Assumption Three : One new dental practice will be added to the network each month beginning in the third month of operations; Assumption Four : Nu Skin Enterprises, Inc. will continue its 33 years of paid commissions for products sold. Compensation to Great Hill Dental Partners, LLC Year One : $75,000 Year Two : $340,000 Year Three: $842,000
