2 Mission & Areas Served Statements Mission Statement To increase cancer awareness through education and access to services among the underserved in the community. Background Statement The Coalition of Hispanic Women Against Cancer (CHWAC) started in 1996 as a volunteer led, grassroots organization. In 2000, the CHWAC became a 501(c)3. Since its inception the CHWAC has involved members of the community (cancer survivors, medical professionals and representatives from cancer related organizations) to raise awareness of early detection screening of cancer. Establishing a non-profit status served as a turning point for the CHWAC to evolve from only providing awareness and education to also provide screening and support programs. The CHWAC initially focused its efforts on breast cancer education. However, during the classes the women attending were also interested in learning more about cervical cancer. In responding to this need, the CHWAC began to provide education and screening services for cervical cancer as well. To date, the CHWAC looks at education, prevention, and support services as the organization's focus. They provide education by attending community health fairs, creating social media campaigns, and providing free community education classes led by Ley Health Educators trained by the CHWAC. They provide free or low cost cancer screening opportunities through mobile events, for both cervical and breast cancer. They also provide support through a Spanish speaking cancer support group (the only one in the metro) and patient navigation services to women who are detected with cancer. Through the years the CHWAC has become a local referral source for other community partners and health organizations. This is through the trust that the CHWAC has built with the community and its success in engaging and empowering Latina women to take better care of their health. Impact Statement The CHWAC was formed by a group of Latina women who recognized the need for education in Spanish about mammogram screenings and breast cancer. Since then it has grown to have focused programming with a primary goal of providing cancer education, prevention and support services to the Latin community. The organization has worked closely with partners to ensure its mission. The organization continuously examines the type of programming that is being provided, its quality and its appropriateness to the community. Through these evaluations the organization has realized that much work is still needed in the area of cancer awareness, education, and screening among minority populations. One of the challenges that we face in the organization is addressing all of these needs. We currently only have 1 full-time staff, therefore we rely heavily on the contributions of our members and volunteers. In a recent strategic planning retreat conducted by the board of directors, increasing staffing and funding resources were identified as the most important concerns to address. It is extremely important that the organization continues its work, but in order to do so we must create a strong foundation and sustainability with our programs and initiatives. Before entering as President of the organization I had been assisting as a volunteer for several years. During my tenure as President I have gained new perspective on ways to improve systems within our organization and programs for our community. I am committed to the advancement of the mission and vision of this organization and feel fortunate that I have been given an opportunity to contribute and be a part of something that is unique, growing, and much needed in the community. 4 Programs Programs Free mammography screening program Description Category Population Served Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success We partner with Early Detection Works (EDW), breast and cervical cancer prevention program funded by the state of Kansas, to provide free clinical breast exams and mammography screenings during four events we host in throughout the year. EDW acknowledges the trust and relationship that our organization has with this marginalized population and so we are responsible for recruiting and communicating with participants. Leavenworth Kansas City Imaging and Dr. Huet-Vaughn also collaborate on this program by providing the clinical services and equipment. To assist and connect women in Missouri to clinical breast exams and mammography screenings, we refer them to Show Me Healthy Women, the breast and cervical cancer prevention program funded by the state of Missouri. This is a "warm" referral, meaning we go through the entire referral process with each woman to ensure that they indeed receive the service. Health Care, General/Other Early Intervention & Prevention Females, Adults, Hispanic, Latino Heritage Eliminate barriers preventing women from seeking mammograms.provide access to free mammograms.increase knowledge of the 3 components of early detection screening of breast cancer.encourage change in behavior for women to begin practicing monthly breast self exams, annual clinical breast exams, mammograms. Increase utilization of early detection screening of breast cancer among Hispanic women ultimately reducing the cancer deaths due to late diagnosis and low survival rates. We will provide Clinical Breast Exam and mammograms and also we still giving information to our community and refer them to where they can have the service. This program records demographic data on all participants. A satisfaction survey is administered to all mammogram recipients for feedback of the event on registration, screening, communication, information, and result and follow-up process. Here are some comments on the surveys about the only mammogram mobile that we had last year. "Thank you for the mammogram, you have helped me a lot please continue with program.""having this service in my own language is more comfortable thank you so much.""the mammogram event is great, I am so thankful." 40 women participate in a total of two mammogram mobiles event last year. The majority of participants do not have medical insurance, are Spanish- speaking. Thanks to that success we will have four mammogram events this year. The Coalition tracks abnormal follow-up and the number of breast cancer cases found. 4

5 Lay Health Educators Addressing Breast Health Disparities Description Category Population Served We provide free community education classes using a curriculum entitled Conocerse es Vivir ( To Know Yourself is to Live ). The ultimate goal is to reduce disparities by equipping Latina women to become Lay Health Educators (LHEs), giving them the knowledge and skills to promote breast and cervical health. Interactive activities engage women in learning about basic female anatomy and critical components of breast and cervical cancer screening processes. The impact of the project is measured through pre and post tests about participants' knowledge and behaviors regarding cancer screenings. These now trained LHEs then provide education to other women within their existing social networks and communities. This is a proven model for the dissemination of breast and cervical cancer prevention messages and increases utilization of cancer screening resources. No other organization in the greater Kansas City area is targeting underserved Latina women with a LHE model and curriculum like ours. Health Care, General/Other Early Intervention & Prevention Adults, Females, Hispanic, Latino Heritage Short-Term Success Lay Health Educators did facilitate the "Por la Vida" session to 47 women from our community during period. Lay health educators and women will focus efforts on promoting breast and cervical cancer awareness, referring participants to Pap smear screening services and referring them to the mammograms resources too. Long- Term Success Program Success Monitored By Examples of Program Success The long-term goal of this project is to increase breast and cervical awareness and reduce breast and cervical cancer mortality by providing services in a way that is culturally appropriate and that the community is receptive to. Results showed that 95% of the participants had an increase in knowledge of breast and cervical cancer. The average increase among individuals was 21% from pre test to post test. 100% of the participants had report a high level of satisfaction with Por La Vida classes. During the grant period the lay health educators conducted educational sessions to 47 Latina women from their community. 6 Support Group for Cancer Survivors in Spanish Description Category Population Served Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success Receiving the news about a positive cancer diagnosis is life changing and the Latina women we aim to assist in our community already face many challenges and vulnerabilities. We know that by offering screenings and diagnostics we could uncover a new diagnosed group of women that would be facing a wave of barriers and will need support. For us, it is not enough to help women learn about cancer and receive screenings services. We also provide them support in a safe environment that meets their cultural and linguistic needs. We have contracted the professional services of a licensed Spanish-speaking therapist to facilitate a support group. This group of women currently meet once a month and is made up of cancer survivors and women currently in treatment. It is the only Spanish speaking support group in the metro area. With additional funding we will be able to host the group twice a month, a request that has been voiced by participants of the group. Human Services, General/Other Human Services, General/Other Females, Hispanic, Latino Heritage, Immigrant, Newcomers, Refugees Survivors women will feel that they are loved when they come to the Support group. This support group will help emotional and spiritual to our survivors women, they will feel that they are not alone, they will be heard and also they will express their own feelings. CHWAC's support group has 11 participants that are very grateful to have a place and group where they can be heard and express themselves, this support group really help them to reduce her emotional pain. Here there are some quotes from our support group's participants: "I am so happy that we have this support group, thank you.""i am not alone in this journey, I am thankful for this beautiful group.""i count the days until the next session, thank you." 6

7 Pap Smear screening mobiles Description Category We began our mobile Pap Smear screenings a few years ago at the request of the community we serve. Women who participated in our LHE community classes and Mammography screenings began asking for education and screening options for cervical cancer. We started to provide mobile cervical cancer screenings throughout the community. Participants are first screened for eligibility of the EDW and Show Me Healthy Women programs, which fund the lab cost associated with the Pap Smear. This allows us to leverage funds and help women who do not qualify for those programs afford their screening for just $ If the participant is unable to pay, we still provide her the service. Screenings are conducted by Dr. Huet-Vaughn, who is a Spanish speaking provider, throughout parts of the metro area. The mobility of the program helps to reduce barriers and meet participants where they are at in the community. Pap smear screening mobiles were implemented two years ago, we did have 8 mobiles during the last year, with a total of 197 participants. CHWAC, pap smear mobile , just have it # 6 pap smear screening mobile, so far we have 145 participants, that are really grateful for their pap smear test. Here are some quotes from our participants; "This is a great program, thank you so much for helping us.""i am so thankful that this organization has this service.""i understand the Dr. and she was really nice with me, thank you." CEO Comments The CHWAC was created to fill a need that had never been addressed in the Greater KC area. That focus was and continues to be to increase awareness and education of prevention and early detection cancer screening to Latina women and all underserved and underrepresented women in our communities. The organization has been successful at reaching this community and working with partners, particularly healthcare providers to ensure culturally appropriate screenings and cancer education. We have successfully started the first Spanish support group and expanded out services with our mobile pap smear project that has allowed us to provide screenings to over 200 women in the past year. We will continue our work in breast and cervical cancer education, prevention and support and welcome individuals looking to donate their experience, talents and time to our cause. 7

8 Leadership & Staff Executive Director/CEO Executive Director Ms. Erica Andrade Term Start July Experience Erica graduated from University of Kansas in 2001 with a BA in Latin American Studies. She has worked at El Centro for the last 13 years. As the Health Navigation Program Manager, it's her role to help individuals maneuver through the barriers of the health care system. Her life's work has become advocating for the clients she sees and speaking to the needs they have in accessing health care services in hope that someday health disparities in Wyandotte County will be eliminated. Ms. Andrade also currently sits on the Providence Medical Center Community Advisory Board and on the Board of Support Kansas City. Senior Staff Ms. Maribel Guzman Title Experience/Biography Bilingual Program Coordinator Ms. Bernal-Estudillo is a native of Mexico and fully bilingual (Spanish/English). She has is well versed in project coordination and management given her experience and education in Business Administration. She also has experience providing health-related services to the Hispanic community in Kansas City. She holds a Bachelors degree in Business Administration from the Universidad Veracruzana in Mexico. 9 Collaborations The CHWAC is always seeking partnerships with organizations that will enhance the health and well being of the community. Partnerships include safety net clinics, private health care providers, hospitals, surgeons, statefunded programs, and charitable care network. In addition to health care partnerships, the CHWAC has established strong collaborations with other community-based organizations, academic institutions, and community members. Many members already have active roles in the Latino community and thus bring many strengths to the CHWAC. During the past year, much of the Board's focus has been on improving systems and prioritizing programs. The CHWAC has been able to grow its programs over the past couple of years and diversify its funding. This has allowed the CHWAC to focus on better responding to the needs and asks of the community it serves. Our Board continues to strive in providing strategic planning, its time and talents to the CHWAC. 11

No Organization Comments The Coalition is a fairly small organization that began from the desire to reduce cancer disparity in a special population group. As any start-up organization the members dedicated their time and money to the success of the organization. In 2004, the Coalition received one of the most important grants that allowed the Coalition to provide a part-time contracted position and an official work site. With these two elements in place, the Coalition was able to move forward in its mission, funding and programs. Since then the Coalition has been able to grow into a budget of $100,000+. As an organization, we realize there is still much more room to grow. A fund development plan is a top priority.
