Every dollar given to Cancer Action stays right here in our hometown helping our family, friends and neighbors through one of the most difficult life experiences they may ever face. There are many ways a person can support the work and mission of Cancer Action. Contact any office to get more information about the items we can accept. Cancer Action has a very active volunteer program. Our volunteers do a variety of different services to help us 1

2 2 meet the needs of those with cancer. Some opportunities include making comfort items, driving cancer patients to their treatment appointments, reception work at our front desk or other office tasks, help with special events, staff a health fair, do courier work and handyman. 3 Mission & Areas Served Statements Mission Statement As a caring community of professionals and volunteers, Cancer Action is dedicated to reducing distress and improving the quality of life for those living with, through and beyond cancer; and promote education, prevention and early detection to reduce the impact of cancer in the greater Kansas City area. Background Statement Over 57 years of compassionate care is the legacy of Cancer Action. The organization has been the place in Kansas City where cancer patients and their families turn to get the support, hope and assistance they need during this difficult and uncertain time. Cancer Action was founded in 1959 as a local, community- based organization providing the vital care to cancer patients and their care givers. We are neighbor helping neighbor, assisting thousands of cancer patients each year. Cancer Action has always been a United Way participating agency. The heart of the organization has been its thousands of volunteers- many who have given thousands of hours each year to help improve the lives of those with cancer in our community. In 2016, Cancer Action volunteers gave over 50,000 hours of service. While Cancer Action still relies on the amazing work of its volunteers, the organization's programs and services have grown to meet the tremendous change in cancer care. Today, Cancer Action provides a vast array of programs and services for those living with, through and beyond cancer in Kansas City. Our caring professionally trained case managers help those living with cancer navigate through the many challenges cancer brings, walking with them through every step of their journey. Our services include practical and direct support such as medical equipment, transportation, wigs, nutritional supplements, medical supplies and comfort items; financial assistance programs; education and emotional support; and wellness and survivorship programs. The agency also has a community education program to educate the public on ways to reduce their risk of cancer. In 2001, the agency was presented with its first national award from the National Cancer Institute's Cancer Information Service, "Partner in Patient Support" for our dedication to increasing the quality of life of cancer patients and the dedication to providing support to the underserved populations in the community. Cancer Action was honored with the 2008 Heart of America Ethics in Business Award, the first given to a non-profit in Kansas City, for the dedication to high ethical standards and integrity in our business practices. Cancer Action continues to be a place of support, encouragement, hope and comfort to thousands of families throughout the Kansas City area. We go the extra mile to ensure those living with cancer in our community have the support they need to cope with the challenges they face. 3

4 Impact Statement Cancer Action believes that no one should face the challenge of cancer alone. When a person hears the frightening words you have cancer it begins them on a very difficult journey. They need someone to come alongside to help navigate their way through this challenging time. Cancer Action is the place cancer patients turn to for help and hope. We offer a comprehensive array of programs and services addressing the physical, social, emotional, financial and spiritual needs of persons with cancer. Our accomplishments for 2016 included: 1. Cancer Action began a new dental program, providing dentures to head & neck cancer patients who have lost their teeth from their disease and/or treatment. This program is in partnership with the HNC Living Foundation. 2. Cancer Action s Champions for Survivorship Breakfast was the best one yet! We had over 350 guests attend. Cynthia Newsome, Mid-Day Anchor for 41 Action News was the guest speaker. 3. Cancer Action is one of the only organizations providing rides to life-saving treatment providing 800 rides to cancer patients during Provided life-sustaining care filling over 2,200 prescriptions an providing over 80,000 servings of nutritional supplements for cancer patients in Kansas City! Goals for 2017 include: 1. Continue to increase the support to underserved cancer patients providing financial assistance and practical services that address their physical, emotional and social needs during this critical time of their life, giving them the best chance for recovery. The needs of those living with cancer continue to grow. 2. Continue to grow our Champions for Survivorship Breakfast to raise additional funds to support the mission and work of the organization. 3. Grow new revenue streams to help meet the increasing financial demands of providing the vital, lifesustaining services to cancer patients in our community. Needs Statement Cancer Action is the only organization that provides many of the life-saving services that cancer patients rely on to get them through this difficult time. The agency had significant increases in the cost for providing these vital services during the past several years. Cancer Action's most pressing needs include: 1. Assisting those with limited financial resources and/or without insurance in obtaining their life-saving and/or life-improving medications. 2. Meeting the growing need for transportation services to treatment appointments which continues to increase so that cancer patients receive their treatment in a timely manner. 3. Meeting the increase in help with nutritional support for cancer patients going through the rigors of cancer treatment serving over 800 cancer patients each year. Cancer Action is the only non-profit offering this vital service. 4.Providing medical equipment and supplies for those with advanced illness and limited resources. 5. Financial support for operational expenses and occupancy costs to assist in maintaining our office locations so that those who need our life-sustaining care can have access to us throughout the Kansas City area. 4

5 CEO/Executive Director/Board Chair Statement For more than 57 years, Cancer Action has provided support, strength and hope for cancer survivors and their caregivers throughout the Kansas City area. Cancer Action is the only cancer-focused nonprofit that exclusively serves area residents. All donations are used solely to meet the growing needs of cancer patients in our community. The fact that Cancer Action is not affiliated with any national organization and is completely focused on serving the Kansas City region continues to be one of the agency s greatest strengths and one of our greatest challenges. While we work closely with the doctors, nurses and other health care providers who serve cancer patients, the agency struggles to make itself more well known within the general community. That has become an even greater challenge in recent years, as the demand for our services has grown faster than our financial support. In many cases, Cancer Action is the only agency that provides services that are critical to our clients health. Cancer Action has become a safety net for cancer patients in Kansas City. To meet this challenge Cancer Action relies heavily on an incredibly dedicated group of volunteers to provide as many services as possible. Many of our volunteers have been with the agency for several decades. These incredible people ensure that Cancer Action never loses the personal touch that is so important to our clients. Cancer Action also tries to partner with other organizations whenever possible to ensure that every dollar donated has the maximum impact possible. I have been involved with cancer organizations for more than two decades. My involvement with Cancer Action is especially rewarding because I know first-hand how important the services we provide are to our clients and their caregivers. The impact is best summed up by a client that received life-sustaining nutritional support. "My doctor cured my cancer, but Cancer Action saved my life." It's hard to make a greater impact on a person than providing hope for the future. Cancer Action is committed to being their when our friends and neighbors receive a cancer diagnosis, to ensure they have the support they desperately need. We are committed to going the extra mile, being there every step of the journey for those with cancer in our community. We do however, serve some cancer patients living outside of our five county service area with donated medical supplies, wigs and other donated items as available through our office locations. 6 Programs Programs Patient Services Program Description The mission of the Patient Services Program is to reduce distress and improve the quality of life for those living with, through and beyond cancer. Helping cancer patients navigate this difficult life experience has been at the core of Cancer Action's work for fifty-seven years. This program provides services addressing the physical, social, emotional, financial and spiritual needs of cancer patients that are not only lifeimproving, but life-saving. During 2016, Cancer Action assisted 2,600 families facing the challenges that cancer brings to their lives. The Patient Services Program provides the following support: nutritional supplements, medical equipment & supplies, transportation to treatment appointments, wigs/turbans/hats, mastectomy supplies, and comfort items; Financial Assistance for cancer-related medications, breast prostheses or lymphedema sleeves; Education and Emotional support, case management, educational programs and resource libraries; Survivorship programs. Category Population Served Short-Term Success Long- Term Success Health Care, General/Other Patient & Family Support People/Families of People with Cancer, People/Families of People with Cancer, Poor,Economically Disadvantaged,Indigent Short term success is measured by the timeliness of service delivery and the immediate impact on reducing distress. Often the person needs and receives assistance the very day they contact the agency. Over 97% of clients repond that services were delivered in a timely manner and helped ease their distress by meeting a physical, emotional and/or financial need. The agency measures the level of distress at time of intake (which is often high) and then again within a month. The professional staff follow-up to determine if the client's immediate needs were met, and if the assistance helped alleviate their current distress. Results show that 60% of clients reporting high levels at time of intake, report a decrease in level of distress with the assistance of Cancer Action. Those with higher distress are usually reporting distress due to treatment side effects. case managers continue to support the patient and offer further intervention as needed. Cancer Action goal is to decrease the distress and improve the quality of life for cancer patients and their families through every step of a person s journey. Many of the services provided by this program are critical to a person receiving the care they need giving them the best opportunity for recovery. These programs and services are particularly vital to the underserved populations in the Kansas City area. Cancer Action wants to ensure that the services offered are meeting the crucial needs of those with cancer in our community. Evaluations are completed by all our clients at least once per year. Over 94% of the respondents indicate that Cancer Action reduced their distress: helped them physically, emotionally, financially and/or educationally; and improved their quality of life during their time of illness, treatment and recovery. Evaluation results are reviewed by our program committee of the Board of Directors and utilized in strategic planning for the agency. 6

7 Program Success Monitored By Success is measured through in many ways. In 2016 we added a new outcome measurement tool. We are utilizing a Distress Assessment Tool at time of initial intake and than again in our monthly follow-up to measure the immediate impact of our work and to identify the needs and prioritize our case managers work. We maintain accurate data, compiled monthly to monitor the amount and type of services delivered, number of new clients registered, and the cost of each program. Case manager monthly logs and notes help assess the number of contacts with clients throughout the month and impact of services delivered. Client evaluation is utilized to determine if the services received helped reduce distress and improve quality of life for the client and/or their families. Evaluations are mailed once a quarter to new clients. Cancer Action registers about 100 new clients each month. These surveys are reviewed by the Program Committee and utilized in the strategic planning process of the agency. Examples of Program Success Summary of client surveys consistently show that over 95 % of clients report that the services and support received by Cancer Action helped reduce stress associated with the disease and its treatment; improved the quality of life for the client and/or family member; helped keep the cancer patient home throughout the course of serious illness (for those with advanced stages of illness); helped physically, emotionally, financially, educationally as they coped with the impact of cancer on their lives. Through the results of comments on surveys that the accessibility of Cancer Action services was difficult in some areas of the community, the Board of Directors made improving accessibility a priority in the strategic planning for the organization, including increasing availability of services without the need to come to the Cancer Action office. This expansion has led to dramatic increases in the number of persons helped throughout the community. 7

8 Community Education Program Description This program is designed to provide information on prevention and early detection of cancer to the community. Special emphasis is on reaching the underserved populations in the city including minority and low income individuals. This accomplished through participation in health fairs, presentations, distribution of education materials and in the Cancer Action newsletter. Cancer Action is active in the Kansas Cancer Partnership, a statewide group that develops and implements the Cancer Plan for the state, as well as other community efforts which focus on prevention efforts. Participants of educational programs at health fairs are given surveys to complete. The goal is that participants will indicate their intention to follow recommended guidelines regarding cancer prevention and early detection. Also, to increase awareness of opportunities within the community to receive free and/or low cost cancer screenings. The overall goal of this program is that Kansas City residents will receive cancer health information on reducing their risk of cancer and be informed on methods of early detection. The emphasis on this program is to reach underserved persons within the Kansas City metropolitan area with lifesaving prevention information. The success of this program is measured by the number of health fairs in which the agency participates and the target population to be reached, amount of literature that is distributed, number of educational programs done and the number of newsletters distributed by the agency. During 2016, Cancer Action provided several educational presentations in the community; participated in many health fairs primarily targeting underserved and minorities; and dispensed over 10,000 pieces of cancerrelated prevention literature. Due to the nature of this program, measuring long-term success is difficult. Program Success Monitored By Examples of Program Success The agency maintains accurate records of the number of health fairs in which it participates and the target population of the health fair; amount of literature distributed at each event and total for the year; the number of educational programs presented throughout the year; number of Cancer Action newsletters distributed that contains prevention and early detection information. Cancer Action has reached thousands of people in underserved areas of the community through the participation in health fairs and presentations. The agency is diligent to provide health information throughout the community to thousands of residents each year as demonstrated by the number of health fairs the agency participates, presentations offered throughout the community, and the amount of literature, referrals and information distributed throughout the year. 8

Just imagine if you were in the fight for your life and could not afford your medications, or had no way to get to your lifesaving treatment- these are the kind of needs that Cancer Action meets every day. Many of the services and support provided by the agency are critical to giving patients the best chance for recovery, and to improving the quality of life for patients and their families throughout their cancer experience. Our goal is to make our services as accessible as possible throughout the metropolitan area, with strategically placed offices, and well coordinated service delivery so patients often obtain the care they need in the most efficient and timely manner possible. The cancer care providers in our community rely on Cancer Action to be the vital partner in care, offering the tangible, financial and community support their patients need to make it through this stressful, lifeimpacting challenge. The agency's commitment to provide prompt and compassionate care is evidenced throughout the organizationfrom the board of directors, staff and volunteers. Our board has representation from heath care providers within the community to help us focus our efforts and resources to meet the most pressing needs of cancer patients in the Kansas City area. She worked at the University of Kansas Cancer Center as a clinical social worker for 9 years. During this time she served on the Board of Directors for Cancer Action, as well as the advisory board for Clinicare Hospice. She served as a member of the multidisciplinary cancer care committee for KU Hospital. She was active in providing cancer support via telemedicine and presented a poster presentation at the national telemedicine conference in She became the director of Cancer Action in August, Her expertise in working with those with cancer spans over two decades. She has expanded the reach and breadth of Cancer Action s programs and services. During her tenure with the agency she has served on numerous community task force and coalitions to improve the access to healthcare services and cancer care and screening in the community. She has provided leadership to the Kansas City Cancer Coalition and established the Integrative Cancer Care conference committee which helped train professionals in an integrative, holistic approach to cancer care. We rely on our community to support our efforts to provide the life-sustaining services and care to thousands of cancer patients in our community each year. Cancer Action's investment fund has provided an additional source of operational funding for the agency during difficult economic times. Cancer Action strives to meet the significant increase demand for services provided to those with cancer. Without these services, many of the underserved cancer patients in Kansas City would go without vital and many times life-saving care needs being met. Cancer Action leadership team continues to explore avenues to generate additional revenues to meet the increased need for services while sustaining the financial stability of the organization. This is a continuous challenge for our organization, as we work to keep pace with the increasing needs, and the increasing costs for the services we provide.
