2 Mission & Areas Served Statements Mission Statement The mission of the American Diabetes Association is to prevent and cure diabetes and to improve the lives of all people affected by diabetes. Background Statement The American Diabetes Association (ADA) is the nation s leading nonprofit health organization providing diabetes research, information and advocacy for all types of diabetes: type 1, type 2, gestational and prediabetes. Founded in 1940, as a medical association for physicians, research scientists, and health care professionals it rapidly expanded to include public education via its magazine Diabetes Forecast and hosted its first camps for children with type 1 diabetes in Currently the American Diabetes Association conducts diabetes education programs in all 50 states and the District of Columbia, reaching more than 800 communities. Locally, the American Diabetes Association of Kansas City serves the entire Kansas City area and surrounding communities. The ADA office in Kansas City has continuously provided diabetes education programs and signature events in our community for more than 35 years. Impact Statement The American Diabetes Association, Kansas City area, provides support through programming and advocacy for those living with diabetes. Accomplishments for 2014: Safe at School- The American Diabetes Association s Safe at School campaign works to give a voice to students denied their rights because of diabetes. With expert staff support, volunteers in local communities throughout the United States who complete the Advanced School Advocacy Training Program (ASAT) program are able to enhance the Association s ability to expand its capacity to help parents/guardian's of children with diabetes across the country. In 2014, 20 volunteers completed the Advanced School Advocacy Training Program. ASAT empowers and prepares volunteers to educate parents/guardians and others about the rights of their children to receive diabetes care in the school setting, pro-active steps parents/guardians should take to make sure their children are treated fairly and how to assist parents confronting school-related challenges facing their children who have diabetes. Project Power- Project POWER is a faith-based program which targets the African American church community with diabetes awareness messages and healthy living tips into the life of the family and church. We are excited about the opportunity to offer input and expand out collaborative relationships as we believe this effort has the potential to have a great impact on the environment, health and well-being of the African American church community. In 2014 we worked with community churches to offer diabetes education and plan to expand our reach in Volunteer Development- In 2014 the American Diabetes Association, Kansas City area, continued to work to increase diabetes awareness and showcase fund-raising events through strategic volunteer recruitment. Implementing this plan has furthered our outreach efforts by creating new avenues to get key messaging distributed regarding diabetes education and fundraising events. Goals for 2015: Continue to give a voice to individuals denied their rights because of diabetes through our advocacy work. Improve outcomes for the residents of the Kansas City area living with diabetes and prediabetes by continuing to provide programming and educational materials. Build our capacity for success by continuing to expand our reach by diversifying our volunteer base. Needs Statement Camp Planet D-Funding for diabetes camp to assist our efforts to never turn a camper away due to their family s inability to pay. Project POWER-Financial support to expand Project Power into more Kansas City area churches. Safe at School-Sponsorship of Safe at School Kits for distribution to Kansas City area school administrators. Community Education-Volunteer and financial support to expand our reach for distributing crucial information related to diabetes care and prevention. 2
3 CEO/Executive Director/Board Chair Statement The American Diabetes Association leads the fight against the deadly consequences of all types of diabetes and fight for those affected by diabetes. Support from individuals, foundations, corporations and organizations throughout the Kansas City area are vital for the success of our programs and advocacy efforts. With over 320,000 individuals living with diabetes in the Kansas City area, it is essential that we continue to grow our volunteer and funding base to meet the needs of our community. Diabetes doesn't stop. It is 24/7, 365 days a year disease. We will continue to advocate and support those living with diabetes until our ultimate goal is met - to Stop Diabetes once and for all. 4 Programs Programs ADA Camp Planet D Description Category American Diabetes Association (ADA) ADA Camp Planet D is a residential camp held annually on the third week of July at the Tall Oaks Conference Center in Linwood, KS. We serve 135 campers ages 7-15 each summer. In addition to the traditional camp activities such as swimming, horseback riding, archery and challenge courses, campers also participate in diabetes education classes with members of our medical staff. Campers learn diabetes self-management skills and the importance of counting carbohydrates, managing their diet, and exercise. They also learn to be more independent in their diabetes management while at camp. Most importantly, they are able to interact with other children who relate to the challenges of living with diabetes. Our long term collaboration with Children Mercy Hospital (CMH) has lead to the program s continued success. CMH provides ADA Camp Planet D s medical camp staff including nurses, endocrinologists, pharmacists, social workers and dieticians. Health Care, General/Other Patient Education Population Served Children Only (5-14 years),, Short-Term Success Long- Term Success Campers will achieve acceptable "control" of diabetes. Control meansblood sugars are at a level that allows the camper to fully participate in camp activities without experiencing low blood sugars or ketoacidosis. Campers will model positive behaviors in diabetes self-management by participating in insulin dosing decisions, and demonstrating knowledge in regulating food and activity levels to maintain good blood sugar management. Campers will display an increased ability to determine the appropriate dosage of insulin needed in relationship to the number of carbohydrates eaten during a meal. Campers will display an increased ability to manage blood glucose levels through consistent monitoring. The ultimate goal of Camp Planet D is for children with diabetes to learn about living with diabetes and gain independence in managing their disease. Program Success Monitored By of camper diabetes skills is part of the camp application process and provides us with a skill baseline for each camper. Camp staff work riences. Camp staff utilizes group interactions, one-on-one instruction, teachable moments and observation to increase knowledge level p evaluations are completed by the medical staff, camper parents, and campers as a comparison measure against pre-camp skills and b Examples of Program Success Camp Planet D has served children living with diabetes in the Kansas City area for over 35 years. In 2015, 135 campers spent the week at Tall Oaks Conference Center learning about diabetes management. The generosity of volunteers with service and funding will enable 135 campers next summer to manage their diabetes in a safe and fun environment. 4
5 POWER Project Description Category Facing disparities in status and access to health care, the 4.9 million African Americans with diabetes experience higher rates of complications than the general population and comprise a significant number of the 7 million Americans unaware of having the disease. More than 25% of African American women over the age of 55 have diabetes. One out of every four African Americans between ages 65 and 74 has diabetes. Addressing the crisis, the ADA developed Project POWER, an educational outreach program to the African American Community through churches, providing education about how to prevent diabetes, improve management and prevent long-term complications associated with the disease. Project POWER Ambassadors are members of the church and are key to the success of Project POWER and its sustainability within the church. The Ambassadors work closely with the American Diabetes Association staff and diabetes educators to coordinate year-round activities for the Church Family. Health Care, General/Other Patient Education Population Served Blacks, African Heritage,, Short-Term Success Long- Term Success Increase awareness that diabetes is a serious disease and inform the congregation of its risk factors. Encourage church members at risk to get tested. Inform the congregation about the resources available through the American Diabetes Association. Create and increase awareness about diabetes, management and treatment of the disease. Integrate healthy messages and activities into the life of the family and church. Create awareness that regular physical activity and maintaining a healthy weight is important for managing diabetes. Integrate physical activity into the life of the family and church. Create awareness that healthy eating is important for the whole family, especially family members with diabetes. Integrate healthy eating into the family life style. Create awareness about the link between diabetes, heart disease and stroke. Create awareness about the ABC s of good diabetes care. Incorporate activities that promote a healthy heart into the life of the family and church. The long-term goal of Project POWER is to bring life saving educational information to the Kansas City area so that steps can be taken to prevent or control the disease and lower the risk of complications. Program Success Monitored By Evaluations are given to all participants, volunteers, church leaders, speakers, vendors and staff to collect feedback about the program s impact. Evaluations ask participants to list new items learned from each module. Also, pre and post tests are administered at each module to assess the educational impact of the materials. Tests results are compiled and analyzed at the national office. Statistical results are reported on an annual basis. 5
6 Examples of Program Success Outreach programs continue to reach more people with higher risk of developing diabetes. As Project Power continues to engage more church members in a variety of year-round activities that provide lessons which improve health and knowledge, public awareness of risk factors, management and prevention will continue to increase. ADA Community Education Programs Description Category American Diabetes Association community education programs are essential to reach Kansas City residents to create awareness, prevent diabetes among at-risk populations, and ensure that all people with diabetes get the best care, treatment, and information about how to manage their diabetes. Commuity education encompasses our Stop Diabetes@Work, Safe@Schools, Living with Type 2 Diabetes, patient seminar, health fair, and lunch and learn programming. These programs are offered year round and open to all of the community. Increase knowledge of risk factors and complications to higher risk populations. Increase public awareness that a healthy lifestyle and education are important for managing diabetes. Increase public awareness of services and education available to those living with diabetes. Increase awareness of the resources available to the community through the American Diabetes Association. Long- Term Success Program Success Monitored By Examples of Program Success The ultimate goal of community education programs is to educate the public about how to Stop Diabetes and support those living with the disease. Evaluations are given to participants after seminars to collect feedback about the programs impact and measure participant satisfaction. Evaluations are compiled and analyzed to determine what participants learned and ways to improve participant satisfaction for future events. For outreach education such has health fairs, data is collected and recorded in the national system to assess our collective impact both regionally and nationwide. Community education programs are essential to reach Kansas City residents about the importance of managing and preventing disease. In 2014, 43,000 residents were reached through education programs and events. Patient education seminars in 2014 received a 91% satisfaction rate and had an attendance rate of 40% for first time attendees. 6
