How Do I Earn My Wellness Points?? Florida Blue members may earn up to 200 points ($200) from July 1, 2016 to April 28, 2017. Your rewards will be paid out to you on June 30, 2017.

2 How Do I Earn My Wellness Points?? Florida Blue members may earn up to 200 points ($200) from July 1, 2016 to April 28, Your rewards will be paid out to you on June 30, (For those in the HSA High Deductible Insurance Plan 5180/5181, you will receive a deposit into your HSA account. For those of you in Plan 5771, you will receive a premium credit in your paycheck.) The following slides will show you where to log-in to view your current points, a list of available activities and their corresponding point values. Once logged in, your current point balance will be displayed at the top of the page. (Note: please allow up to 6-8 weeks for your points to be updated.)

Step 1: Log-in to (If you have not registered before, you will need your Florida Blue Member ID number) Step 2: Click on Health and Wellness then Discounts and Rewards Step 3: Click on Learn More to view available activities and your current point balance

4 WHAT CAN I DO TO EARN MY POINTS? Annual cancer screening (mammogram, papsmear, skin cancer, colonoscopy, etc), 10 points Exercise 12 or more times a month, receive 15 points! Dental Cleanings, 10 points Attend a Health Fair at your work location and complete the Personal Health Assessment, receive 50 points!!

5 Complete List of Available Activities and Points This will be automatically uploaded: o Personal Health Assessment Attend a Health Fair (50 points) o National Diabetes Prevention Program- 100% attendance required (50 points) o Annual Physical from your Gynecologist (25 points) o Annual Physical from your Primary Care Physician (25 points) o Florida Blue Assistance Program- Participate in the Healthy Addition Program or Next Steps Program (20 points) o Health Lecture (15 points) o Blood Donation Up to 5 times per year (must allow 56 days in between) (10 points) Must provide Explanation of Benefits (EOB) or statement from your Doctor or pony to the Wellness Manager: o Cancer Screening- Receive cancer screening(s) - prostate, skin, breast, colon, pap smear, etc. (10 points) o Counseling Sessions- Up to two times per year through Employee Assistance Program (10 points) o Dental Cleaning- Up to two times per year (10 points) o Vision/Glaucoma Screening- One time per year (10 points) Must or pony to the Wellness Manager: o Florida Blue Assistance Program Participate in a Condition Management or Care Coordination Program (20 points) o Exercise at least 12 times per month for at least 30 minutes at a time. Must complete a monthly activity log or hand in gym attendance (15 points) o Participate in a 5k/10k/Triathlon/Half Marathon/Marathon Must provide a copy of your registration to Stacy Donnelly (Up to 4 times per year) (15 points) You must self-report this by clicking the I did this box on the Florida Blue website or phone app: o Access the Florida Blue website or Florida Blue App (10 points) o Immunizations- Receive vaccinations(s) - flu, pneumonia, shingles (5 points)

6 Once logged into your rewards page on the Florida Blue website, click on more after each activity for details on how to report each of them. (FYI annual physicals with your primary care doctor are automatically reported) If you see I did this next to an activity that you have completed, then simply click on the I did this button and your points for that activity will download automatically. For exercising, you will need to turn in a completed activity form to Stacy at the end of each month, or ask your gym to print out your attendance sheet.
