2 GOG Mission Statement The Gynecologic Oncology Group is a non-profit organization with the purpose of promoting excellence in the quality and integrity of clinical and basic scientific re-r search in the field of Gynecologic malignancies. The Group is committed to maintainn- ing the highest standards in clinical trials development, execution, analysis and distrii- bution of results. 3 Learning Objectives The GOG has the objective to provide improved health care for women afflicted with gynecologic malignancies. For the educational objectives, we list the following: 1. To inform the participants at the semi-annual business meetings as to the most current state of clinical and basic research in gynecologic oncology, particularly, but not exclusively as it relates to clinical field trials by: Progress reports of recent completed GOG studies, cancer center activities, work of other groups and efforts of individual investigators. Status update of current trials, whether a group or non-group in origin. Projections of future clinical programs to diagnose, treat and/or prevent cancer. 2. To provide participants with peer review critique of progress (or lack of it) with the objective of self-improvement Identify strengths and weakness of individual investigators and their institutions. Counsel investigators in corrective procedures to obtain better research and better reported data. Set up detailed programs for quality and productivity from a basic foundation of education of the investigator in research objectives and procedures. 3. To provide an opportunity to learn research administration and financial management in a cooperative group setting. 4. To provide a lecture type forum of reports from experts from many fields that may relate to better research or patient management. 3
4 Hotel Information The Gynecologic Oncology Group (GOG) will be holding their Semi-Annual Meeting at the: Manchester Grand Hyatt San Diego One Market Place San Diego, CA Reservations: Direct Toll-Free Attendees are responsible for making their own reservations. Individual attendees may make reservations directly with the hotel at or by calling and identify themselves as attendees of the Gynecologic Oncology Group. A deposit equal to one (1) night s room and tax, or a major credit card number with expiration date, will be required to guarantee the reservations. Hotel room rates are $ for single and double occupancy. The cut-off date for accepting reservations into this room block is December 13, Reservations requests received after the cut-off date will be based on availability at the Hotel s prevailing rates. Hotel Location The Manchester Grand Hyatt is a waterfront hotel located downtown on San Diego Bay. The hotel is between the San Diego Convention Center and Seaport Village, with easy access to the San Diego Zoo, Sea World, historic Old Town, Balboa Park, golf, Tennis, and beaches. Airport and Ground Transportation The Manchester Grand Hyatt Hotel is about 3 ½ miles from the San Diego International Airport. Taxicabs - The Taxicab service from the San Diego International Airport to the Manchester Grand Hyatt Hotel is $10. Shuttle Bus Service Airport Shuttle Service is available through Cloud 9 Shuttle. No Reservation is required. The cost from the San Diego International Airport to the Manchester Grand Hyatt Hotel is $6 each way. For questions/reservations please contact Hotel Parking Hotel overnight parking is $16. Valet Parking is $22 Reminder - The cut-off date is Monday, December 13, Reservations requests received after the cut-off date will be based on availability at the Hotel s prevailing rates. The Deadline for Hotel Reservations is: Monday, December 13, 2004 The Manchester Grand Hyatt will sell out quickly so reservations should be made promptly by the cut-off date to secure rooms. IMPORTANT PLEASE READ CAREFULLY Please try to refrain from making hotel reservations and then canceling after the cut-off date. When this situation occurs, the hotel room block becomes full and GOG registrants who are more definite about attending the meeting may have problems reserving rooms. The hotel does not give the rooms back to GOG if you cancel after the cut-off date. Therefore, we lose the rooms and GOG registrants who definitely plan to attend the meeting are forced to other hotels. 4
5 Meeting Registration Information Registration Information Since reservations will be made directly with the hotel, the GOG Administrative Office will require pre-registration for all persons attending the GOG meeting. This registration will insure that meeting materials be available to you and your colleagues. Please complete the attached registration form. Print or Type the form clearly and provide in the appropriate spaces the information that is requested for all persons attending the meeting. Please register as early as possible becauseregistrations will not be accepted after theregistration deadline. Registration Forms received in the GOG Administrative Office after December 13, 2004, will be processed On-Site at the Registration Desk at the GOG meeting. PLEASE NOTE: There is now only ONE registration form for the GOG Semi-Annual Meeting, GOG Winter Symposium and Data Management Training Session. Online Registration Online Registration on the GOG website is now improved to include online payment of all registration fees. Attendees will now have the option of submitting payment securely online. Visa and MasterCard will only be accepted. A confirmation will be given immediately upon submitting payment. 6 Committee Meeting Registration The Gynecologic Oncology Group now requires that you register for each Committee Meeting that you plan to attend. Before completing your preregistration form please review the preliminary agenda for committee meetings and times. Please select all committee meetings in which you plan to attend on the registration form. The GOG has applied for Continuing Medical Education Credits (CME s) for this meeting. All meetings with approved CME Certificates that you are registered for will be provided in your meeting packet at the meeting. Registration for each committee meeting will insure that each attendee s names are preprinted on CME certificates and Sign-in sheets. Upon entry to each meeting please sign-in at the Sign-In Desk. CME Certificates must be validated at the GOG Validation Booth after the close of each committee meeting. Committee Evaluations will also be provided in your Meeting Packet. These evaluations must be completed before CME Certificates will be validated. All attendees registered for conflicting committee meetings that occur during the same time, will be issued only one CME Certificate for one of the meetings chosen.
