1 Barnett Wood Lane A welcoming home, offering transitional support for adults with mild to moderate learning disabilities.
2 Welcome to our home A warm welcome to Barnett Wood Lane, a lovely detached Victorian house in Ashtead, Surrey, close to local amenities and with great local transport links nearby. Barnett Wood Lane is an ideal home environment for people who are thinking of moving on or where transitional care would benefit. At Barnett Wood Lane we support people who wish to live as independently as possible, irrespective of the level of care and support they require. We provide a home where people can live comfortably and safely, enjoying learning new skills and building confidence. Barnett Wood Lane team exemplify the values of our charity - valuing the individual, working together, honesty, energy and creativity. Clare Pelham, CEO Award winning..we are the 2014 winners of the Leonard Cheshire Disability Excellent Care Award and The Chief Executive s Award.
3 Transitional support Our support packages are based around each individuals need; encompassing all aspects of daily living. Including personal care, help with cleaning and cooking, medical appointments, medication, financial assistance, road safety awareness, gaining employment developing skills through goal plans, building social networks and orientation to name a few. Providing support and gradually reducing that support as individual confidence is increased and living skills are strengthened. The aim of the home is to provide an environment with individually tailored support, where the next step is either supported living or moving into the local community. We aim to support our residents to be independent and get the most out of life, in preparation for supported living or moving into the local community.
4 The right support On arrival you are supported to create your own person-centred, support and goal plans. These are regularly monitored and reviewed to ensure goals are successful and achievable. We aim to support people at Barnett Wood Lane to become more confident about directing their own choices. The support we offer at Barnett Wood Lane is based on what our residents tell us they want. We will always ask what you would like to do or achieve. Our friendly team are fully trained to provide 24 hour care and support. Its a pleasure to work with everyone in a happy house. Pepe, member of staff
