3 Statements & Search Criteria Statements Mission Statement Bluegrass Ovarian Cancer Support, Inc. exists to help Central Kentucky women and their loved ones during diagnosis, treatment, and survival of ovarian and other gynecological cancers; and works to promote public awareness and education of these cancers. Background Statement When Suzi Shoemaker was diagnosed with ovarian cancer in 2006, she assumed there would be a support group for her in Lexington, Kentucky. After her first experience with a Women s Cancer Group where she was told, We only had one member with ovarian cancer, and she just died, Suzi decided there was a need for ovarian cancer survivors to share their unique stories. Through referrals from the University of Kentucky Gynecologic Oncology staff, Suzi met another survivor, Sharon Walsh. In March of 2008 the two women founded Bluegrass Ovarian Cancer Support Inc. at Ramsey s Restaurant with about eight other ovarian cancer survivors in attendance. Bluegrass Ovarian Cancer Support, Inc. (BOCS) is made up entirely of volunteers. Today, Bluegrass Ovarian Cancer Support, Inc. offers a monthly support group for women with ovarian and other gynecologic cancers. About women and family members are part of the group at any time. Guest speakers provide information to BOCS members, related to the diagnosis, treatment and survivorship of gynecologic cancers, at more than 25 % of the meetings each year. Bluegrass Ovarian Cancer Support participates in the Ovarian Cancer Research Fund Alliance s Survivors Teaching Students: Saving Women s Lives (STS ) program. Local volunteers give presentations on ovarian cancer for third year medical, physician assistant, and Sophomore and Junior nursing students at the University of Kentucky. BOCS members share information about ovarian and other gynecologic cancers at health fairs and other awareness events. They also provide women's "care bags", containing items useful during a hospital stay or during chemotherapy treatments, for women diagnosed with gynecologic cancers. Bluegrass Ovarian Cancer Support, Inc. has also been a Partner member of the Ovarian Cancer National Alliance since 2012, and now Community Partner with the Ovarian Cancer Research Fund Alliance. BOCS is also a member of the Kentucky nprofit Network. BOCS is proud to be awarded gold level status for nonprofits from Guidestar, and is a valued nonprofit partner with the Bluegrass Community Foundation. Bluegrass Ovarian Cancer Support, Inc. is dedicated to supporting women, and their caregivers, through survivorship of ovarian and other gynecological cancers. 3

4 Impact Statement In 2017, over 800 ovarian cancer symptoms cards were distributed to individuals and businesses. Sixty-six (66) women's "care bags" were distributed to new gynecologic cancer survivors, and 156 chemotherapy hats were distributed to UK Markey, Baptist Health and Lexington Kentucky One Health infusion centers. In September 2017, Bluegrass Ovarian Cancer Support, Inc. (BOCS) members created over 300 "Turn the Towns Teal" ribbon bows that were displayed in downtown Lexington, UK Markey Cancer Center, UK Colleges of Pharmacy and Agriculture, Baptist Health, St. Joseph East Hospital, the City of Versailles, KY, Azur & Brasabana restaurants, both Lexington locations of Newk's & City BBQ restaurants, and James B. Haggin Memorial Hospital - Harrodsburg, KY and Mercer County Health Department. In 2017, BOCS members presented the Ovarian Cancer Research Fund Alliance's "Survivors Teaching Students: Saving Women's Lives" program, to a total of 395 UK health care students, comprising third year medical students, UK physician assistant students, and UK Sophomore and Junior nursing students. In 2017, the third annual BOCS Survivor Luncheon, was held for gynecologic cancer survivors and guests. And, except for the month of September, BOCS monthly support group meetings were held for survivors/caregivers/families and friends. Guest speakers gave presentations to members at 7 out of 11 of those meetings. Needs Statement Current needs: Making initial contact with ovarian and other gynecologic cancer survivors. Increasing public awareness of ovarian and other gynecologic cancers through awareness events, and media. On-going funding for printed literature/supplies for awareness events, and other group projects, such as women's "care bags", and survivor luncheon. Volunteer support for Bluegrass Ovarian Cancer Support, Inc. activities, including: board members, officer positions, event planners, website/communications manager. Funding to sponsor cancer survivors, from Bluegrass Ovarian Cancer Support, Inc., to attend regional or national ovarian cancer conferences. 4

5 CEO/Executive Director Statement BOCS is an all volunteer non-profit organization with the following goals for 2017: Participate in 2 or more community activities promoting awareness of ovarian and other gynecologic cancers. Participate in at least one on-line fundraising campaign during the year, such as the Kentucky nprofit Network's "KY Gives Day" or the Blue Grass Community Foundation's "Good Giving Challenge" Have at least 60% board member attendance at our 2017 Board meetings. Add at least one more facilitator to the Ovarian Cancer Research Fund Alliance's "Survivors Teaching Students: Saving Women's Lives" program. Add at least one more presenter to the Ovarian Cancer Research Fund Alliance's "Survivors Teaching Students: Saving Women's Lives program. Offer, and provide as requested, the Ovarian Cancer Research Fund Alliance's "Survivors Teaching Students: Saving Women's Lives" program to: UK third year medical, UK Physician Assistant, and UK Nursing students. Establish one new healthcare program in Central or Eastern Kentucky to receive the Ovarian Cancer Research Fund Alliance's "Survivor's Teaching Student's: Saving Women's Lives " program. Consider a teleconference format if needed. Have guest speakers at a minimum of 25% of our 2017 monthly BOCS meetings. 5

6 Board Chair Statement My name is Sue Jacobs and I am blessed to be a 9 year survivor of ovarian cancer. Although my cancer was diagnosed early, sadly almost 90% of ovarian cancer patients receive their diagnosis at a late stage of disease, when treatment is less successful. This is due to lack of a definitive screening test for ovarian cancer, and many times women are unaware or ignore the symptoms of ovarian cancer. I have become passionate about making women aware of the signs and symptoms of ovarian and other gynecologic cancers, and supporting women and their families in their cancer journey. Shortly after my cancer diagnosis, I was thrilled to join an organization whose mission supports my passion: Bluegrass Ovarian Cancer Support, Inc. (BOCS) ( I became Secretary of BOCS in 2009, and along with my husband and daughters, have been involved with the group ever since. I was elected the 2014 President of Bluegrass Ovarian Cancer Support, Inc., and re-elected as President for I have been coordinator of the Ovarian Cancer Research Fund Alliance's "Survivor Teaching Students: Saving Women's Lives" (R) program since The Bluegrass Ovarian Cancer Support group board and officers are made up entirely of volunteers, many of them survivors of cancer. I am continually humbled and amazed by the dedication of many of our member volunteers who may be undergoing cancer treatment while being involved with many of our groups' activities. Despite the challenge of literally having enough healthy volunteers to complete our planned projects, I am extremely proud of the ways in which BOCS is trying to raise awareness of the symptoms and risk factors of ovarian and other gynecologic cancers, in the Bluegrass region. In 2014 and 2015, BOCS participated in 5 or more health fairs/awareness events, providing free information to participants. Some of our members made appearances on local television and radio programs, during Ovarian Cancer Awareness month, in September, to increase ovarian cancer awareness. Member volunteers provide education about the symptoms and risk factors for ovarian cancer, while sharing their personal journey of cancer diagnosis, treatment and survivorship, to University of Kentucky third year medical students, physician assistant students and nurse practitioner students through the Ovarian Cancer National Alliance "Survivors Teaching Student's: Saving Women's Lives (R) program. I feel Bluegrass Ovarian Cancer Support, Inc. continually strives to fulfill our mission to help women and their loved ones during diagnosis, treatment and survival of ovarian and other gynecological cancers, and to promote public awareness and education of these cancers. Train ovarian cancer survivors and their caregivers to present the Ovarian Cancer Research Fund Alliance's program: "Survivors Teaching Students: Saving Women's Lives Program (R). 5.) Advocate for additional research and funding for research for screening, diagnosis, and treatment of gynecologic cancers Strategies The following strategies will be employed to achieve the above goals: Supply women's "care bags", containing information about Bluegrass Ovarian Cancer Support, Inc., to the GYN-Oncology practices at the University of Kentucky Markey Cancer Center and Baptist Health Lexington. Advertise monthly support group meetings in the Herald Leader newspaper, Health & Wellness magazine, Bluegrass Ovarian Cancer Support, Inc.(BOCS) literature and on the BOCS website: Offer to staff a BOCS table of free information, about ovarian and other gynecologic cancers, at health fairs or community awareness events. Actively recruit gynecologic cancer survivors and their caregivers to complete the Ovarian Cancer Research Fund Alliance training for the "Survivors Teaching Students: Saving Women's Lives" (R) program, on an ongoing basis Contact representatives of professional healthcare training programs, such as Medical, Physician Assistant, Nursing, Nurse Practitioner and Osteopathy Schools in Central and Eastern Kentucky and offer the "Survivors Teaching Students: Saving Women's Lives" (R) program to their students. Capabilities BOCS will actively engage in fundraising, apply for grants and use available funds to achieve the aforementioned goals. In 2015 and 2016, Bluegrass Ovarian Cancer Support Inc., was honored to receive grants from the Lexington Medical Society Foundation to further our mission. Indicators BOCS will keep a yearly account of: all organization income and expenses, the number of ovarian cancer symptom cards distributed at community awareness events, the number of trained presenters and the number of students who receive the Ovarian Cancer Research Fund Alliance's "Survivors Teaching Students" Saving Women's Lives" (R) program, and all board meeting minutes. Progress In 2016, BOCS presented the Ovarian Cancer Research Fund Alliance's "Survivors Teaching Students: Saving Women's Lives" (R) program for the first time to University of Kentucky Sophomore nursing students, while continuing to present to UK third year medical students, physician students, and Junior nursing students. In addition to UK Markey Cancer Center and downtown Lexington, the "Turn the Towns Teal" program was expanded to other Lexington businesses and to the city of Versailles. BOCS received a $1400 grant from the Lexington Medical Society Foundation to promote ovarian and other gynecologic awareness in the community. BOCS realizes that all of our educational initiatives are dependent upon having active volunteers to participate in all program areas. Due to many of our volunteers being in treatment for their gynecologic cancers, having an adequate number of volunteers to sustain or grow our community awareness outreach can be challenging. In the future, BOCS would benefit from soliciting volunteer support from the community at large. 7

8 Programs Programs Monthly Support Group meetings Description Monthly support group meetings are held the second or third Wednesday of the month, in the evening, in the meeting room of the Joseph Beth Café. Meetings are open to cancer survivors and their family, caregivers and friends. Several meetings include a guest speaker. The whirlwind of emotions and feelings of being alone can be devastating for some patients. BOCS provides a foundation for those who need support through their or their loved one's diagnosis and care. Further, BOCS gives its members educational opportunities: not only do members educate others about gynecologic cancers, but they also educate themselves about these challenging cancers. She took on that responsibility because this group was so important to her. Eunice was always a very healthy person. Even when some "bug" that was going around would hit me and our children Eunice seldom caught it. Before her diagnosis with Stage 4 ovarian cancer she had symptoms for exactly 5 days. BOCS was important to Eunice for several reasons, however, two stand out as most important, the people / members and the mission. The people we met in and through BOCS were caring, supportive and understanding of the battle Eunice was fighting to beat almost unbeatable odds. Only 15% of Stage 4 Ovarian Cancer patients live 5 years. The average is 17 months. It was good for Eunice to spend some time each month with women who understood exactly what she was facing. The mission of Ovarian Cancer Awareness was equally important. Eunice was blindsided. She had her annual Gyn Physical exam just 2 1/2 months prior to being diagnosed Stage 4. Everything appeared normal in vember and then in early Feb. we heard the words, "We are looking at a cancer situation here." When possible Eunice participated in the BOCS awareness activities including, TURN THE TOWN'S TO TEAL and Survivors Teaching Students (STS) as well as attending Health Fairs and other opportunities to inform women of the importance of Ovarian Awareness. The group has a motto, "Until there is a test, awareness is best." Our group is not on the front lines of the important research to find first of all a reliable test for ovarian cancer and then a cure. However, our group is working hard to do the important work of supporting women and families battling this disease. Of equal or possibly even more importance is the work of helping raise the awareness of the disease with women in general and especially nursing students, physician assistant students, pharmacy students and medical school students at the University of Kentucky. Ovarian Cancer has been called, The Silent Killer because the symptoms can be so vague or nonexistent until the disease has progressed to Stage 3 or 4. Currently making women and health care providers more aware of the signs pointing to ovarian cancer is the best chance women have of being diagnosed in the early stages when a cure has a better chance of happening. This was important to Eunice so I am honoring her memory and battle by continuing my involvement with BOCS. 14
