Annual salary (SCHCADS Level 5 dependent on qualifications and experience) plus 9.5% Employer superannuation, attractive salary packaging offered headspace Frankston Manager Nil Senior Youth Workers are integral in supporting access and engagement of young people to the services and programs offered at headspace Frankston and partner agencies. The role provides in-centre support to young people and their families/carers seeking information, resources and help. Outreach and community engagement activities, include health promotion, brief interventions, psychoeducation, harm minimisation and promotion of help seeking behaviours are facilitated by hsf Youth Workers in collaboration with the broader hsf Youth Access and clinical teams. A Youth Duty function provides support to the Youth Access Team s responsiveness, initial contact, screening and warm referral processes for young people within scope of practice. Senior Youth Workers hold a specialty portfolio. 1

2 Key Responsibilities Youth Access & Engagement To assume a portfolio for improving access for a targeted demographic or diversity group (LGBTIQA+ Young People). Facilitate QSA youth groups in conjunction with the Program Coordinator, hsf workers and partner agency workers. Actively monitor a small case load of young people in partnership with key workers, as guided by portfolio scope Support follow up on non-attendance to appointments and group programs (as required) To assist in or actively assess the needs of a young person and inform an active and targeted intervention within the headspace Frankston model Record and actively contribute to client screening, assessment, care planning, review and closure processes within a collaborative multidisciplinary team as required Support implementation of a young person s care planning goals Facilitate the provision of information, resources and brief interventions relating to mental health, drug and alcohol or other youth health issues to individuals, families/carers requesting guidance and support. Develop and monitor work plan KPIs for designated portfolio (HEY Grant work plan) Plan, deliver and evaluate QSA Unders and Overs groups term based program in consultation with Program Coordinator Effective and timely screening, warm referral and early interventions provided to young people and their families Attendance of weekly multidisciplinary clinical case review (when requested) Support and Advocacy Assist LGBTIQA+ young people to access a range of services that ensure coordinated care for the individual s identified needs Existing school QSA groups visited twice yearly Facilitate support to school QSA groups in Frankston LGA Facilitate the establishment of new QSA groups within schools Advocate on behalf of the young person with workers, schools, employers and other services as required Facilitate interventions to individuals or small groups of young people who do not have access to other youth services. This may include assertive outreach or as a prelude to joining the hsf service or group programs 2 new QSA groups in Frankston LGA annually Young people supported in linkages to hsf services, group programs of other community services and programs. 2

3 Health Promotion & Community Engagement Continuous Improvement Facilitate Boost Your Mood brief intervention and peer supported small group programs to young people Facilitate and evaluate in-school SSAIGD safety and inclusion training for students Participate in health promotion program development, implementation and evaluation Facilitate support and provision of information, resources to enable Frankston LGA schools to undertake IDAHOT day activities. Contribute to continuous quality improvement in relation to service delivery. Developing linkages with referral services. Apply the Young People s Charter of Practice Principles in all aspects of role. Participation in evaluation and ongoing monitoring of the program. Include clients in the evaluation of the service where appropriate. Collaborate with other support services and community based activities to integrate support and provide optimal service provision, including HEY partners, LGBTIQ+ Actions group and other relevant meetings and committees as delegated Deliver 8 x The People Project year level talks or school assembly presentations annually Inform and disseminate health promoting information & resources within schools IDAHOT Day activities supported, promoted & evaluated Inform and support the facilitation of hsf community engagement activity planning and delivery Deliver 1 x Boost Your Mood brief intervention session per term to each hsf QSA group. Ensure all work complies of the relevant legislation, regulations, YSAS policies and procedures Ensure confidentiality of documentation is maintained Ensure the diversity of LGBTIQA+ lived experience, intersectionality, social and other indicators that impact on an individual s mental health and general wellbeing are understood. 3

On every occasion YSAS is represented ethically and professionally. Good working relationships with stakeholders Organisational needs understood Support the induction, peer support and ongoing professional development of youth worker students and new graduates Undertake proactive identification and on-going management of stakeholders, including suppliers and auditors Work collaboratively with staff across the organisation to ensure organisational needs are understood, recognised and responded to, as well as staff are supported in their services Position Requirements Personal qualities A team player, able to work in a collaborative, conscientious and respectful manner with all stakeholders Commitment to the principles of youth participation and shared decision making. 5 Key Selection Criteria Education Experience Knowledge and Skills Tertiary qualifications in Youth Work, Social Work, Family Therapy, Psychology or other related health/welfare related qualifications Lived experience within the LGBTIQA+ community (desired but not essential) Demonstrated commitment to the values of human rights and social justice principles Demonstrated experience (min 5 years) in youth work practice and youth participation, Demonstrated experience and understanding of engagement issues related to LGBTIQA+ youth Experience and understanding of mental health, drug and alcohol, other social and health issues facing LGBTIQA+ young people. Experience in delivery of brief interventions or health promoting programs, educational materials or training packages to individuals, within a group and community engagement environment A comprehensive knowledge of youth services, programs and initiatives relevant for LGBTIQA+ young people within the community Demonstrated understanding of advocacy and activism and how they do or do not apply to the functions of the role Well-developed written and verbal communication skills, specifically public speaking and group facilitation Ability to work with confidential and sensitive information Ability to work independently and part of a multidisciplinary team I accept the PD as acknowledged above and understand that the PD will be reviewed as required.
