1 Calling all CDE s Diabetes Queensland is expanding its service delivery Three full time or part time, fixed term positions available This is an outstanding opportunity for an experienced Credentialled Diabetes Educator to work for Diabetes Queensland to improve the health and wellbeing of Queenslanders. We are seeking a skilled and passionate Credentialled Diabetes Educator to take our Health Services Team to a dynamic new level. As a member of a team of dedicated professionals, the successful candidate will take a leading role in providing individualised clinical diabetes self-management education to clients in the general practice setting. They will also provide evidence based diabetes information and education for consumers, health professionals and the general community. To enjoy real success in this role, you will have: Demonstrated ability to apply knowledge, skills and expertise in diabetes education. To deliver diabetes education in clinical occasions of service/appointments to clients in the general practice setting. The ability to support general practice staff with their diabetes education upskilling requests Demonstrated high-level written and verbal communication skills, including reporting, public speaking and presentations. Active involvement in bringing community health services together to increase service collaboration for people living with diabetes. Excellent documentation and reporting skills. The ability to be organised and flexible and meet timelines. The availability and flexibility to travel to multiple locations within Queensland. Role requirements: Postgraduate tertiary qualifications in Diabetes Education and completed credentialing with the Australian Diabetes Educator Association. C class motor vehicle licence. Current updated vaccine-preventable diseases immunisation record. (eg. is vaccinated against or not susceptible to Hepatitis B, Measles, Mumps and Rubella, Pertussis and Varicella). Pre-employment screening, including criminal history and discipline history checks, will be undertaken on the candidate recommended for employment. Purpose of Position The purpose of the Credentailled Diabetes Educator (CDE) is to provide individualised clinical diabetes self-management education to clients in the general practice setting. They will also provide evidence based diabetes information and education for consumers, health professionals and the general community. This position will support the Western Queensland PHN diabetes project which aims to improve diabetes management in the general practice setting. Providing upskilling of health professionals with diabetes management strategies to support their patients living with diabetes. It will involve face-to-face individual clinical consultations. The role will also involve group education for people living with diabetes as well as formal and informal education and upskilling in best practice diabetes management for general practice staff. The CDE, when in Brisbane will be expected to deliver support through Telehealth to their patients seen in the general practice setting. The CDE will have a room provided by the general practice to see their patients who are referred and booked by the GPs and staff within the practice. When there is opportunity, they will also sit with practice nurses and GPs who are with patients and provide clinical advice relating to diabetes self-management education. The CDE will also be expected to provide ad hoc and formal education on diabetes self-management to health professionals within the general practice setting. This position will also support the work of Diabetes Queensland. The CDE will be expected to provide diabetes expertise in the prevention and management of diabetes for consumers, health professionals and the general community. To assist in the delivery of health professional and consumer programs working closely with all health team members.

3 This position will also support meeting the needs of stakeholders and assist in the delivery of programs and initiatives in line with the organisational structure and identified strategies. Duties and Responsibilities Education: Provide high quality evidence-based information and education to people living with diabetes referred by the GP in clinical appointments and via telehealth. The education and information is tailored to the individuals health care needs and circumstances as per the ADEA guidelines and standards on diabetes education. Coordinate patient care with GP s, general practice staff and local primary health care services related to an individual s diabetes care. Work with general practice linking into continuous improvement, monitoring and evaluation activities that will ultimately improve diabetes management. Provide health professional education in the general practice setting in the form of ad hoc, opportunistic, case study and formal education sessions. The delivery of diabetes education programs and health promotion activities (state based or local) as required. Provide expertise and advice regarding Diabetes Queensland resources and publications, including the ongoing review of factsheets, web content and newsletter articles developed to increase awareness of diabetes and support people in their understanding and management of diabetes. Conduct duties with high standards of ethical conduct and behaviour and a culture of service, performance and teamwork. Network Work with general practice health professional networks and local community health networks assist in providing links to general practice, Aboriginal Community Controlled Health services and other health and community services. Identify opportunities and strategies to improve health and support networks within the community, include these in daily reporting activity to support the quarterly reporting framework. Reporting Daily reporting activities are essential to support invoicing activity and build the quarterly reporting framework required to meet contractual obligations. Report on identified opportunities, challenges and ideas that would improve and support diabetes management. The CDE will be expected to provide a consult report letter back to the referring general practice for each patient relating to the advice supplied and suggestions for the individuals diabetes self-management and next appointments required. Travel This position will be expected to travel to areas in Western Queensland for up to five days maximum on a monthly rotational roster. Travel will include flights and car travel (refer to licence requirements) to practices supported within the WQ PHN project. Travel will also consist of ad hoc travel to support diabetes education services for consumers and health professionals when not on WQ PHN project. Postgraduate tertiary qualifications in Diabetes Education and completed credentialling with the Australian Diabetes Educator Association with a Health Professional background. 2. Demonstrated excellent organisational and time management skills, communication skills, and the ability to work with multiple reporting systems. 3. Demonstrated ability to apply knowledge, skill and expertise in diabetes education, to evaluate and review delivery of education tailoring to the individual needs of the person living with diabetes. 4. Demonstrated ability to work independently within a multidisciplinary team under supervision of a Manager, to collaborate and advocate for the person living with diabetes. 5. Demonstrated high level of written and verbal communication skills, including reporting, public speaking and presentations. Highly desirable 1. Experience in working with general practice with a familiarity of how general practice works. 2. Familiarity with MBS occasions of service. 3. Experience as a Credentailled Diabetes Educator in clinical practice.

5 Additional Factors The incumbent to the position will be required to: Occasionally work non-standard hours Travel intrastate. Possess a C class Queensland drivers licence. Have up to date vaccine-preventable diseases immunisation status. Undergo a criminal history check, when recommended for employment.
