1 OBJECTIVES AND MEMBERSHIP GUIDELINES The University f Clrad Cancer Center (CU Cancer Center) has been designated a cmprehensive cancer center by the Natinal Cancer Institute f the Natinal Institutes f Health. The CU Cancer Center receives supprt frm the Natinal Cancer Institute under grant P30CA The CU Cancer Center s missin is t discver, develp and deliver breakthrughs in diagnsis, treatment and preventin t imprve cancer care lcally, natinally and glbally. These effrts frm the basis f a cmprehensive spectrum f translatinal preventin, diagnsis, treatment, survivrship and utreach. The Center s visin is t transfrm cancer research and practice in the state f Clrad by creating an interdisciplinary nexus f clinicians and scientists acrss ur statewide cnsrtium that can leverage, synergize, marshal and fcus resurces and expertise t discver new ways t prevent and treat cancer. The CU Cancer Center has nine institutinal members: University f Clrad Denver, University f Clrad Bulder, Clrad State University, University f Clrad Hspital, Children s Hspital Clrad, the Denver VA Medical Center, Denver Health, Natinal Jewish Health and the Research Institute at Kaiser Permanente Clrad. The Cancer Center encurages membership f researchers and clinicians interested in cllabratin with scientist and institutins that are pursuing cancer research in the areas f basic science, clinical care, cancer preventin and cntrl, and educatin. Cancer Center membership is available t individuals in the Rcky Muntain regin wh have a demnstrated, n-ging interest in cancer related research and/r cancer related clinical activities that cntribute t the missin f the University f Clrad Cancer Center. Membership is granted at ne f the fllwing levels: Full, Assciate, r Affiliate Member. OBJECTIVES OF the CU Cancer Center Fllwing are the majr bjectives f the University f Clrad Cancer Center. 1. The Cancer Center encurages and facilitates cperatin and cmmunicatin amng basic scientists, clinical investigatrs and scial, psychlgical and ther behaviral scientists, as well as between CU Cancer Center members and cancer researchers at ther institutins natin-wide. 2. New Research Prgram areas and services are develped by the Cancer Center t cmplement existing cancer research and clinical resurces, as well as t prmte areas f new research. 3. The Cancer Center s Shared Resurces exist fr the benefit f the cancer-relevant research cnducted by member investigatrs. The shared resurces are reviewed regularly fr ways t strengthen and imprve their service t members, as well as t expand int new services as technlgies advance. 4. The Cancer Center strives fr the highest quality f clinical care by prmting multidisciplinary diagnstic, treatment and preventin services; by demnstrating and evaluating new management techniques; and by fstering the rapid clinical applicatin f demnstrably useful methds f cancer management and preventin. 5. Teaching prgrams in pre-and pst-dctral fields related t cancer and in the allied health prfessins are encuraged and fstered in rder t train the prfessinal and technical persnnel necessary t attack the cancer prblem natinally. 6. Active participatin in Cancer Center Prgrams by practicing health prfessinals in Denver and thrughut Clrad and the surrunding regin is encuraged. Participatin fsters an integrated, cperative cmmunity f cancer research, treatment, preventin and cntrl, cmmunity engagement and educatin activities.
2 Sectin 1. Eligibility MEMBERSHIP GUIDELINES Cancer Center membership is available t individuals in the Rcky Muntain regin wh have a demnstrated, n-ging interest in cancer related research and/r cancer related clinical activities that cntribute t the missin f the University f Clrad Cancer Center. Applicatins are taken frm individuals, nt grups r institutins. Membership is granted at ne f the fllwing levels: Full Member, Assciate Member r Affiliate Member (Sectin 3). Sectin 2. Applicatin Prcess Applicatin fr membership is vluntary and cnntes agreement by the applicant with the Center's missin, bjectives and requirements. Candidates must submit a cmplete applicatin package t the Cancer Center s Administrative Office. A cmplete applicatin package cnsists f (1) the cmpleted Membership Applicatin Frm; (2) the Cancer Relevance Infrmatin Frm and Statement; and (3) a current cpy f the applicant s cmplete curriculum vitae. The statement f cancer research interest shuld cntain sufficient detail t allw reviewers t understand hw the applicant is invlved in cancer research (i.e. basic, clinical r bth), what types f cancer are impacted by the applicant s research r clinical activities, and whether the research has translatinal ptential. In additin, the applicant s statement shuld prvide the names f ther cancer center members with whm (s)he cllabrates r intends t establish cllabratins, and identify any shared resurces that (s)he intends t use in pursuit f the applicant s cancer research prgram. Applicatins are reviewed by the C-Leaders f the research prgram selected by the candidate. The Prgram C-Leaders are required t specify, as part f their review, hw the candidate will cntribute t and strengthen the prgram. The Prgram C-Leaders als recmmend the membership level. Prgram C-Leaders may elect t cntact an applicant directly t btain any necessary clarificatins and/r t discuss in-depth the applicant s interests and area(s) f cancer-relevant activities. Fllwing endrsement by the Prgram C-Leaders, the applicatin is directed t the Membership Cmmittee which is respnsible fr verifying eligibility accrding t the criteria utlined in Sectin 3 and recmmending either (a) apprval f the candidate and f the membership level; r (b) disapprval. A recmmendatin fr apprval f membership by the Membership Cmmittee must be unanimus. Decisins lacking unanimity will be addressed by the Membership Cmmittee Chair, wh will prvide a summary pinin t the Cancer Center Directr. The Prgram C-Leader and Membership Cmmittee recmmendatins are frwarded t the Cancer Center Directr fr final apprval. Applicatins frm exceptinal individuals wh are nt engaged in independent labratry, clinical, r ppulatin-based research, yet wh cntribute t the Cancer Center s missin may be cnsidered fr Affiliate membership status. These applicatins will be reviewed by the Membership Cmmittee, evaluated by the Cancer Center s Executive Cmmittee and apprved by the Cancer Center Directr. Sectin 3. Membership Categries There are three categries f membership in the Cancer Center: Full, Assciate and Affiliate. Sectin 3.1. Full membership will be cnsidered fr applicants wh: (a) are faculty at any f the Member Institutins f the Cancer Center 1 ; (b) perfrm basic, clinical, translatinal r ppulatin-based cancer research n a regular basis; and (c) meet all f the fllwing three criteria: 1 Member institutins include: University f Clrad Denver, University f Clrad Hspital, University f Clrad Bulder, Clrad State University, Natinal Jewish Health, Denver VA Medical Center, Children s Hspital Clrad, Denver Health & Hspitals, and the Research Institute f Kaiser Permanente Clrad.
3 1) Perfrm cancer research r cancer care as the primary fcus f their prfessinal career. 2) Have n-ging independent supprt fr their cancer related activities, fr example as a PI f a cancer-fcused, peer-reviewed grant 2 ; r as PI f an investigatr- initiated cancer clinical study. 3) Have at least five peer-reviewed papers published within the past five years that are significantly cancer-related (in-press publicatins can be cnsidered). Sectin 3.2. Assciate membership will be cnsidered fr thse individuals wh are faculty at any f the Member Institutins f the Cancer Center 1 and wh cntribute significantly t basic, clinical, translatinal r ppulatin-based cancer research, but d nt meet the criteria fr Full membership. Assciate members typically are junir faculty, established faculty changing career fcus, r established faculty wh d nt have cancer research as their main career fcus. Assciate members must meet the first tw f the fllwing five criteria and at least tw f the last three criteria: 1) Perfrm cancer research, cancer care and/r cancer-related educatinal activities as a significant cmpnent f their prfessinal career. 2) Have supprt fr their cancer related activities frm faculty start-up funds, spnsred grants and cntracts as PI r c-investigatr (including seed grants, pilt prjects, and institutinal grants). 3) Have at least three peer-reviewed papers with significant cancer fcus published within the past five years. 4) Hld membership in at least ne prfessinal scientific assciatin with a cancer fcus. 5) Have presented at a scientific meeting rganized by a cancer-related prfessinal rganizatin such as AACR, ASCO, ONS r the ACS within the last tw years. Sectin 3.3. Affiliate membership will be cnsidered fr thse individuals wh cntribute t basic, clinical, translatinal r ppulatin-based cancer research thrugh a supprting rle, r wh therwise cntribute t the missin f the Cancer Center. Examples f supprting rles include, but are nt limited t: (a) Cntributin t patient accrual in cancer-related clinical trials; (b) Rle as c-pi r c-investigatr in cancer related grants r cntracts; (c) Membership in prfessinal and scientific assciatins with cancer fcus; (d) Presentatins in the last tw years in scientific meetings rganized by cancer-related assciatins such as AACR, ASCO, ONS, ACS; (e) cntribute t furthering the Cancer Center s missin thrugh cmmunity engagement, educatin and/r philanthrpic activities. Sectin 3.4. Exceptinal individuals wh prvide significant cntributins t the Cancer Center missin may be apprved fr each f the membership categries at the discretin f the Cancer Center Directr. Sectin 4. Membership Requirements Sectin 4.1. Membership will be reviewed annually by the Membership Cmmittee and Prgram Leadership f the Cancer Center. All members will be reviewed t insure the cntinued relevance f the member s prfessinal activities t the Cancer Center s scientific prgrams, missin, and ther activities. Members may be asked t prvide infrmatin regarding their publicatins and spnsred research awards, as well as an updated statement f cancer research interest. Sectin 4.2. Cntinuus membership is cntingent upn n-ging adherence with the criteria specified abve fr the distinct membership categries. The annual membership review can result in decisins t change an individual s membership status. 2 Peer-reviewed as defined by the Natinal Cancer Institute
4 Sectin 5. Appeals Individuals may appeal decisins regarding eligibility r leading t a change in their membership status. Appeals shuld be submitted t the Cancer Center Membership Cmmittee and shuld include evidence f cmpliance with the listed criteria. The Cancer Center Directr will be the final arbiter in an appeal. Sectin 6. Benefits f Membership Members f the University f Clrad Cancer Center have numerus pprtunities t participate in research sympsia, research retreats, attend special presentatins, and engage in cmmunity engagement, educatin and training activities. Full and Assciate members will be integrated int fcus grups within their Research Prgram. The C-Prgram Leaders will rganize peridic sympsia and research meetings t fcus n specific cancer-relevant research questins, fstering the develpment f crss-disciplinary cllabratins and facilitating the advancement f translatinal science. Participatin by members, in turn, leads t the develpment f new fcus areas within existing Prgrams, f new Prgrams and f new shared resurce services t cmplement and prmte new areas f research. Full and Assciate members als receive the fllwing benefits: Access t the Cancer Center s shared resurces at subsidized rates fr supprt f their cancerrelated research activities 3 ; Access t intramural funding pprtunities prvided by the Cancer Center; Opprtunities t be cnsidered fr nminatin t ne f the many natinal cancer-specific funding pprtunities that limit applicatins t ne per institutin (e.g. the Damn Runyn Cancer Research Fundatin; Samuel Waxman Fundatin; Mary Kay Fundatin; and the V Fundatin). Opprtunities t serve as a mentr in varius training prgrams supprted by the Cancer Center; Opprtunities t be selected as a recipient f dnr directed philanthrpy; and Access t the Cancer Center s Administrative Office fr supprt f Develpment and submissin f large, multi-departmental cancer-specific grants; Planning, scheduling and rganizatin f Cancer Center endrsed activities benefiting a large cntingent f the membership; Annuncement f new research develpments and clinical breakthrughs by PR and media relatins prfessinals; and Particular philanthrpic activities supprting Cancer Center strategic gals, with dedicated develpment prfessinals t assist such fund-raising effrts. Sectin 7. Expectatins f Members The CU Cancer Center is an NCI-designated cmprehensive cancer center. Our ability t maintain this designatin is dependent upn members willingness t participate actively in the research and clinical care cmmunity fstered by the Cancer Center. T assist ur effrts t cntinue t build upn the vibrant cmmunity f physicians, scientists, staff and cmmunity members engaged in the Cancer Center, we ask the fllwing f ur members: 1. Participate in the Cancer Center spnsred sympsia, retreats, seminars and surveys. Yur participatin can impact decisins regarding the develpment f new fcus areas in ur Research Prgrams; new shared resurce services; and the develpment f new funding pprtunities. 3 Please cite the Cancer Center Supprt Grant P30CA and the name f the Shared Resurce used in all publicatins resulting frm wrk perfrmed in any part by the Cancer Center s shared resurces.

Prvide annual lay and scientific prgress reprts fr yur prjects that are funded by the Cancer Center; and cntinue t prvide us with infrmatin regarding publicatins, grant submissins and awards, and any ther research resurce yu develp as a result f the funding. This infrmatin is used t infrm bth extramural spnsrs and philanthrpic dnrs f the utcmes achieved as a result f their funding supprt. 3. Acknwledge the Cancer Center Supprt Grant (P30CA046934) and yur use f Cancer Center Shared Resurces in yur publicatins. Our NCI designatin is dependent upn this grant. Failure by members t cite the grant in publicatins will, ver time, undermine ur NCI supprt and affect ur designatin.
