Annual influenza immunizatin is recmmended, but the ability t munt an antibdy respnse may be limited by calcineurin inhibitrs which are key cmpnents f lung transplant immunsuppressin regimens. We hypthesized that a differential effect between tacrlimus and cyclsprine n influenza vaccine respnse rates wuld exist. The research study was apprved by the institutinal review bard, and all participants gave their written infrmed cnsent. Methds: Lung transplant patients had bld drawn prir t receiving the influenza vaccine and fur weeks later ver tw seasns. Influenza antibdy cncentratins were measured by hemagglutinatin inhibitin assay. Vaccine respnse rates (antibdy cncentratin > 40 hemagglutinatin units (HAU) and at least 4 fld increase in antibdy cncentratin t at least ne vaccine virus) were cmpared using chi square tests. The influence f tacrlimus and cyclsprine n vaccine respnse was cmpared. With a sample size f 65, the pwer t detect a 50% difference in vaccine respnse rate was at least 80%. Results: A ttal f 64 lung transplant patients participated. Frty-ne patients taking tacrlimus were fllwed fr median f 37 mnths and 23 patients taking cyclsprine were fllwed fr median 51 mnths (p>0.6; Mann-Whitney U) Amng the lung transplant patients, n difference in influenza vaccine respnse rates in patients taking tacrlimus r cyclsprine culd be fund. 5 CASE STUDY PROTOTYPE Cancer + TB + Cyanide = 4 Saved Lives Denise Tripp Martin, RN; Julia Landn, RN, CPTC, Carlina Dnr Services, Durham, NC Full name, credentials listed Intrductin and brief definitin Cncise explanatin f situatin Detailed explanatin f flw f events Three distinct sectins Patient Prfile: This 27y male was admitted t ED fr suicide ideatins stemming frm a histry f depressin. While in the ED, the patient had a syncpal episde leaving the restrm and sn was fund t be pulseless and cyantic. CPR, intubatin and ACLS drugs were administered. Discussin: Patient was nted t be in severe lactic acidsis, ph <6.8, lactate In additin, a white substance was fund n the patient, wh wrked as a chemist. Therefre, the hspital suspected cyanide pisning. Patient received sdium thisulfate, a cyanide antidte. This significantly imprved the lactic acidsis, thereby supprting cyanide txicity later cnfirmed by txiclgy. The patient was declared brain dead and was evaluated as a ptential rgan dnr due t family interest. The Pisn Cntrl Center was immediately cntacted. They reprted that rgans had previusly been transplanted fllwing cyanide pisnings. Past medical histry included, a diagnses at age 18 f Stage IV testicular cancer with metastasis t the abdminal lymph ndes and right lung. He received chemtherapy, radiatin, a left rchiectmy, and right lbectmy. Fllwing tw years f treatment he had been in remissin fr 7 years. At age 16, the patient underwent treatment fr ne year fr a psitive TB test. Written cnsent was btained and evaluatin f rgan functin ensued. Initial creatinine and liver functin tests were elevated but trending dwnward. Trpnin and cags remained within nrmal limits. An echcardigram revealed a structurally nrmal heart with an EF f 60-65%. CXR was clear. Brnchscpy revealed n purulent secretins; nrmal anatmy except absence f the right lwer lbe. PaO2 was >520 n 100% FIO2. Urinary utput averaged 3cc/kg/hr. The heart, liver, and kidneys were successfully transplanted. Summary: OPOs and hspitals shuld nt assume that patients are unsuitable dnrs when the patient presents with a significant past medical histry r a death frm a txic substance. This case is a clear example f why we shuld attempt t place rgans even when suitability is questinable. Because f the OPO s cmmitment t pursue every dnr, every rgan, every time, fur peple received life saving transplants. (N cnfidential infrmatin has been disclsed in this case study.
