3 ONLINE PREPARATORY TUTORIALS July 2018 LIVE INTERVENTION 8-9 September 2018 ONLINE IN-DEPTH TUTORIALS October 2018 Cardiometabolic disorders in clinical practice: a comprehensive update Overview Cardiometabolic disorders are the major cause of morbidity and mortality worldwide, and are intrinsically linked with diabetes, a devastating disease affecting more than 400 million people. Diabetes is the leading cause of cardiovascular disease, end-stage renal disease, blindness and non-traumatic lower-limb amputation, as well as negatively affecting pregnancy outcomes and the nervous system. However, diabetes and its complications can be avoided. A huge number of people suffer from initial alterations in glucose metabolism, known as "pre-diabetes". These people are at higher risk of developing overt diabetes in the coming years, but evidence shows the progression towards diabetes can be halted. Prompt intervention, with significant changes in lifestyle and/or implementation of appropriate pharmacotherapy, are effective strategies to prevent diabetes in at-risk people. To facilitate this, healthcare professionals should be updated with the most recent and compelling evidence about diabetes prevention. For those with overt diabetes, relentless effort should be expended to avoid the debilitating complications of the disease and to increase life expectancy. Knowledge and understanding of the complex pharmacological mechanisms and their clinical implementation will facilitate successful management of diabetes. Like diabetes, thyroid disorders are among the most prevalent endocrine diseases that also impact cardiovascular homeostasis. Unfortunately, a great number of patients with thyroid diseases are undiagnosed, contributing to increased morbidity and reduced quality of life. Therefore, there is a need for clear recommendations allowing for effective and prompt recognition of the most frequent thyroid disorders in order to adequately treat patients and avoid adverse clinical outcomes. Finally, hypertension is still the primary cause of death in the world and medicine is far from establishing adequate control of this cardiovascular risk factor. Recently, new aggressive targets for antihypertensive treatment were proposed, but it is not clear if scientific evidence justifies this therapeutic strategy. Additional discussion surrounds the issue of applying the results of controlled clinical trials to daily life. The role of combination treatment in hypertension and recent novelties in the availability of fixed dose formulations will also be discussed. Xxx Xxx An EXCEMED Learning Journey EXCEMED is involved in a comprehensive educational blended journey to increase knowledge in cardiometabolic diseases. This programme will address the most recent advancements of research in the areas of cardiometabolic medicine and will discuss the best approaches for clinical management of the complex cardiometabolic patient in daily practice. A multistep approach will be used, encompassing web-based and live activities. All activities in-person and online will involve the full interactivity of learners. This online educational tool is facilitated by the video-scribing technique and it enables to: Understand the importance of an effective diabetes screening Learn how to identify subjects at increased risk of diabetes LIBRARY The second exclusive tutorial helps illuminate the content of the live intervention, giving you the head start you need to be fully engaged during the in-person learning. You are also able to share your research and publications with the rest of the community using the library platform, and submit questions directly to faculty ahead of the live intervention. You can look forward to your question being addressed by faculty during theliv Q&A sessions. 11 George J. Kahaly Department of Medicine Gutenberg University Medical Center Mainz, Germany Ernesto Maddaloni University Campus Bio-Medico (UCBM) Rome, Italy George J. Kahaly, M.D., Ph.D. holds the rank of Professor of Medicine and Endocrinology / Metabolism and is chief physician of the endocrine outpatient clinic at the Johannes Gutenberg University (JGU) Medical Center, Mainz, Germany. Dr. Kahaly chairs the ORPHAN JGU expert center for Graves orbitopathy and polyglandular autoimmune syndromes. Dr. Kahaly directs the Molecular Thyroid Research Laboratory of JGU and has authored 247 original papers and reviews, covering clinical and experimental aspects of endocrine autoimmunity, immunogenetics of thyroid and polyglandular autoimmunity, as well as cardiovascular involvement of metabolic disorders. Original papers and reviews have been published in the New England Journal of Medicine, Lancet, Jama, Clinical Chemistry, the Journal of Autoimmunity, the Journal of Clinical Endocrinology and Metabolism, the Journal of Nuclear Medicine, Endocrinology, Thyroid, Endocrine Reviews, Nature Reviews and Autoimmunity Reviews. Ernesto Maddaloni is a consultant endocrinologist at his home institution, Campus Bio-Medico University of Rome, Italy, were he graduated in Medicine in 2010 and completed his clinical fellowship in Endocrinology and Metabolic Disorders in In 2014, he moved to Boston for a Post- Doctoral Research Fellowship at the Vascular Cell Biology Section of the Joslin Diabetes Center (Harvard Medical School) focusing on the study of aging adults with autoimmune diabetes and the interplay between vascular complications, bone health and autoimmunity. As a Mentee of the European Foundation for the Study of Diabetes (EFSD) Future Leaders Mentorship Programme, Dr Maddaloni has joined since 2017 the Diabetes Trial Unit of the University of Oxford, Oxford Centre for Diabetes Endocrinology and Metabolism (OCDEM) where he is actually pursuing his studies on clinical diabetes and cardiovascular outcomes. Dr Maddaloni has acted as study investigator and coordinator in several national and international clinical trials and has published as leading author in several top-ranked peerreviewed international medical journals. His scientific achievements have been recognized with several awards, including the "2015 Albert Renold fellowship of the European Foundation for the Study of Diabetes, the "2017 Young Investigator Award" of the Italian Society of Endocrinology (SIE) and the "2017 Early Investigator Award" of the Endocrine Society, USA

12 Giuseppe Mancia University of Milano-Bicocca Milan, Italy Mafauzy Mohamed University Sains Malaysia Health Campus Kelantan, Malaysia Giuseppe Mancia is Professor Emeritus of the University Milano-Bicocca. He is also President of the Italian Foundation on Ricerca e Innovazione su Ipertensione e Protezione Cardiovascolare and of the Foundation of the European Society of Hypertension(ESH) in Zurich. He serves since 2008 as Chairman of the Joint Board of the Italian Scientific Societies operating in the area of cardiovascular prevention. He has served as President of the International Society of Hypertension, President of the European Society of Clinical Investigation, Chairman of the Working Group on Hypertension and the Heart of the ESC, member of the Executive Scientific Committee of ESC, President of the Italian Society of Hypertension ( ) and President of ESH. He has been member ex-officio (only European) of the Executive Council of the American Society of Hypertension, Officer for the ISH Forum of National Societies, member of the Committee on Cardiovascular Drugs and Therapy of the World Heart Federation and member of the Writing Committee of the ESC guidelines on Prevention of Cardiovascular Diseases, the WHO/ISH Guidelines on Hypertension and the Guidelines on Hypertension in the Elderly of the American College of Cardiology. He has chaired the Committee on the ESH-ESC guidelines on Hypertension, the WHO/ISH Liaison Committee, and the Scientific and Organising Committees of various ESH Meetings. He is currently Chairman of the Educational Board of ESH. His research interests focus on epidemiology pathophysiology, diagnosis and treatment of hypertension, heart failure, and coronary disease, as well as on the clinical aspects of diabetes, obesity and other metabolic abnormalities. His expertise includes ambulatory blood pressure monitoring, neurohumoral control of the circulation, large artery mechanics, hemodynamic changes of cardiovascular drugs, cardiovascular regulation during stress and sleep as well as epidemiology and intervention trials in hypertension and diabetes. Mafauzy Mohamed is currently a Professor of Medicine and Senior Consultant Endocrinologist in University Sains Malaysia. He was formerly the Assistant Vice-Cancellor, Director of Campus, Director of Hospital and Dean of Medicine in the same university in Kelantan, Malaysia. He obtained his M.B.B.S. from the University of Adelaide, Australia in He was appointed as a Lecturer after he received his M.R.C.P (UK) in He also obtained his Masters of Medical Sciences from University of Sheffield in He was promoted to Associate Professor in In 1996, he was admitted as Fellow of Royal College of Physicians (Edinburgh) and in 2000 he was promoted as full Professor of Medicine. He also received an Honorary Doctor of Medicine degree from International Medical University in He is actively involved in the field of endocrinology and diabetes. He has also served as an examiner for the Royal College of Physicians, United Kingdom and has been appointed by several universities as External Examiner for Professional Examinations at both undergraduate and postgraduate levels and also as Masters and PhD theses examiner. Dr. Mafauzy is also actively involved in research and has to date obtained over 110 research grants mainly in the field of diabetes, dyslipidemia and thyroid disorders including many clinical trials, including CV and renal outcome trials, relating to diabetes and metabolism. He has supervised over 25 postgraduate students and has published over 110 papers in national and international Journals, presented over 100 papers in national and international meetings and has been invited to give over 500 lectures in national and international meetings. He has been Chief Editor of the Malaysian Journal of Medical Sciences, Journal of ASEAN Federation of Endocrine Societies and Journal of Endocrinology and Metabolism. He is also a Council Member of the Malaysian Medical Council and is actively involved in accreditation and recognition of medical programmes by the Malaysian Medical Council and Malaysian Qualifications Agency

13 Thy Khue Nguyen University of Medicine and Pharmacy Ho Chi Minh City, Vietnam Nemencio Almare Nicodemus Jr Department of Medicine University of the Philippines-College of Medicine Manila, Philippines Thy Khue Nguyen is Associate Professor Emeritus, Department of Endocrinology, University of Medicine and Pharmacy, Ho Chi Minh City. She is immediate Past President of the Vietnam Association of Diabetes and Endocrinology, President of HCMC Diabetes and Endocrinology Association, Vice President of HCMC Medical Association, Member of The Endocrine Society, United States, Senior Consultant at Hospital 115 HCMC and Head of Endocrinology Division at MEDIC HCMC. Her studies are mainly on Epidemiology of diabetes in South Vietnam diabetic nephropathy, lipid disorders in diabetes, hypoglycemia in DM, diabetic keto- acidosis, thyroid disorders in pregnancy. Nemencio Almare Nicodemus Jr obtained his degree of Doctor of Medicine at the University of the Philippines - College of Medicine. He subsequently did his residency training in Internal Medicine and fellowship training in Endocrinology, Diabetes and Metabolism at the Philippine General Hospital. Subsequently, he went to the Mayo Clinic in Rochester, Minnesota as part of the Visiting Clinician Program. He is a Professor at the University of the Philippines - College of Medicine, Department of Biochemistry & Molecular Biology, where he was awarded the Most Outstanding Medical Teacher in the Basic Sciences. He is also a Clinical Professor at the Department of Medicine of the Philippine General Hospital where he was recognized by the University of the Philippines Medical Alumni Society in America as Best Teacher. He is currently the Vice President of the Philippine Society of Endocrinology, Diabetes and Metabolism (PSEDM), a director of the Philippine Association For The Study of Overweight and Obesity, Asian Alliance for the Study of Neuroendocrine Tumors and Philippine Thyroid Association. He is the Associate Editor of the Journal of the ASEAN Federation of Endocrine Societies (JAFES)

14 Dang Van Phuoc Vietnam National University School of Medicine Ho Chi Minh City, Vietnam Paolo Pozzilli University Campus Bio-Medico (UCBM) Rome, Italy Dang Van Phuoc is currently Professor and Dean of the Faculty of Medicine Vietnam National University HCM City and Senior Consultant Cardiologist at the University Medical Center / University of Medicine & Pharmacy Ho Chi Minh City. He earned his medical degree from the Hanoi Medical University, completed his doctoral training in cardiovascular medicine in Germany in and a research fellowship in Germany in He went on to obtain a one-year fellowship in interventional cardiology at the Houston s Texas Medical Center, USA, in Professor Phuoc previously held the positions of Chancellor and Dean of the Faculty of Medicine of the University of Medicine and Pharmacy of HCM City (2006 to 2011) as well as Head of the Department of Cardiology, Vice Director of Cho Ray Hospital HCM City (2004 to 2011). Professor Phuoc is the President of the Heart Association of Ho Chi Minh City and Vice- President of the Viet Nam National Heart Association. He has served on the editorial board of several academic journals of Viet Nam and published widely in the local medical literature. Paolo Pozzilli, MD trained in Medicine and specialized in Endocrinology & Metabolic Diseases from Sapienza University of Rome, Italy, under the guidance of Professor Domenico Andreani. He worked mainly in the field of diabetes with particular focus to type 1 diabetes (T1D) pathogenesis and prevention in Rome and in the UK, and in collaboration with several investigators worldwide. Internationally he set up with others the EURODIAB study on the incidence of childhood T1D, that is the most comprehensive mapping of the incidence of T1D in Europe. He has been the regional P.I. for TRIGR, the longest trial for the primary prevention of T1D. He established a joint international PhD program in diabetes including 4 Universities (Barts & the London, Queen Mary, UK; University of Ulm, Germany; University of Barcelona, Spain and University Campus Bio- Medico, Rome, Italy). The most relevant international studies where he acted as Leader or regional P.I. include ENDIT, TRIGR, Action LADA and more recently EMPAREG, EXSCEL, CANVAS and IN-TANDEM2 trials. His collaborations with investigators in Europe, US and more recently China, have allowed him to implement joint research programs for the identification of T1D pathogenic mechanisms leading to beta cell destruction. He is the Director of the Department of Endocrinology & Diabetes, at University Campus Bio-Medico in Rome and of the Post-Graduate School of Endocrinology and Metabolism at the same University where he is also Coordinator of the PhD program in Integrated Biomedical Sciences and Bioethics. He was President of the Italian Society of Diabetes for Lazio and Rome region, founder and Chairman in the late 80 s of the IMDIAB group (immunotherapy of T1D) with participation of several diabetes hospital centers in Rome, leading to the conduction of 13 clinical trials in the specific field. Currently is the Coordinator of the Italian Society of Endocrinology for Lazio and Rome region. In the UK he has worked at the Centre of Diabetes, St. Bartholomew s and the London School of Medicine, Queen Mary, University of London, initially under the guidance of the late Professor Andrew Cudworth at St. Bartholomew s Hospital. He continued his activity at the Centre of Diabetes, Blizard Institute, Queen Mary on projects on Latent Autoimmune Diabetes in Adults (LADA) being Deputy leader of the EU funded programme ACTION LADA

15 Ho Huynh Quang Tri Heart Institute Ho Chi Minh City, Vietnam Kun-Ho Yoon Department of Endocrinology & Metabolism Seoul St. Mary's Hospital Seoul, Korea Ho Huynh Quang Tri is the Chief of Intensive Care Unit at the Heart Institute, Ho Chi Minh city. A 1988 graduate of the Faculty of Medicine, University of Odessa, Ukraine, Dr Ho obtained a fellowship in internal medicine at Nhan Dan Gia Dinh Hospital, Ho Chi Minh city, and a fellowship in intensive care and cardiology at the Heart Institute. He honed his skills in cardiac intensive care during his 1-year-long residency at Broussais Hospital in Paris, France. He was awarded the degree of PhD in Medicine and Cardiology in 2010 at the University of Medicine and Pharmacy, Ho Chi Minh city. Dr Ho holds the position of Vice-President and Secretary-General of the Society of Cardiology, Ho Chi Minh city. He is also a member of the editorial board for the society s official journal. He has published widely in the field of cardiology in Vietnamese and international journals including the Asian Cardiovascular & Thoracic Annals and the Journal of Heart Valve Disease. Dr Ho is a visiting senior lecturer at the Department of Internal Medicine, Ho Chi Minh city University of Medicine and Pharmacy. Kun-Ho Yoon is Professor Department of Endocrinology & Metabolism at Seoul St Mary s Hospital, and chief of Department of Medical Informatics, Catholic University Medical College, Korea. Prof Yoon graduated with his medical degree and also pursued his PhD program at the Catholic University Medical College. Prof Yoon also completed a 2- year stint as a visiting scholar with the Cell biology and islet transplantation section in Joslin Diabetes Center of Harvard Medical School, US at 1996~1998. Prof Yoon was the recipient of the KDA Scientific Award three times. He was honoured with health technology Award by minister of Ministry of Health and Welfare and Innovative ICT and healthcare award by president. Prof Yoon has published more than 200 articles in international peer-reviewed journals with recent publications in Lancet, JAMA, Cell metabolism, Diabetes Care, Diabetes, and so on. His research has been focused on the betacell biology and improved diabetes patients care using the mobile and internet platform. He also led the development of treatment guideline of diabetes by KDA and he served as a vice president of KDA. He had served as Executive Director, Bureau of HT R&D Planning and Budget Management of Korea Health Industry Development Institute for two years
