1 REQUEST FOR PROPOSAL FITNESS EQUIPMENT BRIGHTON AREA SCHOOLS BRIGHTON, MICHIGAN November 25, Brighton Area Schools is seeking bids for FITNESS EQUIPMENT at Brighton High School. 2. A pre-bid meeting will take place at 10:00am Monday, December 7, 2015, Brighton High School Main Conference Room at which time questions will be answered. This meet is not mandatory. 3. Sealed Proposals will be received not later than 12:00am Wednesday December 16, 2015, Brighton High School Main Conference Room at which time the proposals will be opened and recorded as received. 4. Description of the information and content required for your bid is attached. Please complete and add necessary information on additional sheets and return to: Brighton Area Schools Athletic Director 7878 Brighton Rd Brighton, MI Mark the return envelope: Fitness Equipment 6. Bidders must complete the required attached Familial Discloser Affidavit. 7. All responses must be printed and signed. The bidder assumes the risk of any delay in the mail or in the handling of the mail by employees of Brighton Area Schools. Weather sent by mail or by means of personal delivery, the bidder assumes responsibility for having his bid deposited on time at the place specified. 8. The contract will be awarded to the responsible bidder whose proposal is within the competitive range and determined to be the most advantageous to the Brighton Area Schools. Price, experience, references and other factors are considered. Brighton Area Schools reserves the right to make awards within sixty (60) days after the date of the bid opening during which period bids may not be withdrawn. 9. The Board of Education or its designee reserves the right to reject any or all bids, in part or in total, for any objective or subjective reason whatsoever. Late bids will not be opened. Page 1

2 10. Bids over $23, must be accompanied by a certified check or Treasury listed bid bond, as listed in U.S Dept. of Treasury Circular 570, in minimum amount of 5% of bid amount. Please include this. Bids not received without this guaranty will not be read or considered. 11. Successful bidders for work in excess of $50,000 will be required to provide Payment and Performance Bonds issued by sureties listed by the Treasury Department and licensed to do business in Michigan. Sureties must be acceptable by Brighton Area Schools 12. The Owner shall have the right to accept Mandatory and Voluntary Alternated in any order of combination, unless otherwise specifically provided in the Bidding Documents. INVITATION TO BID Notice is hereby given that sealed bids will be received by Brighton Area Schools Project: Location: Area: High School Fitness Equipment Brighton High School 7878 Brighton Road Brighton, MI Fitness Center Submittals: Bidder shall provide a picture, warranty information and specification sheet on each piece of equipment at the time of bid. Scope of Work Manufacturers: If listed, the following manufacturers' products have been used to establish minimum standards for materials, workmanship and function. Equal products of other manufacturers may be used and should be listed as voluntary alternates. If accepted, the substitution shall be included in an addendum and the decision by the Owner shall be final. Page 2

3 Prices: Training Delivery: Warranties: All prices shall be F.O.B. Destination (7878 Brighton Rd.) and shall include all cost for the delivery and installation of Fitness Equipment specified herein. No other costs will be permitted. The vendor beyond those stated in the bid. Bidder must hold prices firm for sixty (60) days. Bid should include the complete training of Brighton Area Public Schools Athletic and Physical Education staff in the proper use and maintenance of all equipment sold. Training is to be conducted once installation of all equipment has been completed. Equipment is to be delivered and set up in the Brighton High School Fitness Center, 7878 Brighton Rd Brighton, MI Equipment must be moved and secured in the fitness center space once received to Vendors shall coordinate delivery dates and times with the Director of Operations. The owner, Brighton Area Public Schools shall be provided with full manufacturer s warranty as a condition of the award. The warranty shall be stated on the bid form for each product specified. The successful bidder must provide the owner with any and all instructions, manuals, and directions for proper cleaning and maintenance of all items bid. All work (labor and materials) is to be warranted for one (1) year from the project completion date. Clean-Up: During the course of and the completion of the project the contractor agrees to clean up all debris and residue left from the project to the satisfaction of the owner at the expense of the contractor. Schedule: Contractor shall schedule work with the Director of Operations. Work in the building must be completed within 10 days from the project start date. The building is available from 8am to 5pm. The bid proposal will be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the owner or any employee of the bidder and any member of the Brighton Area School Board or the Brighton Area Schools Superintendent. The district will not accept a bid proposal that does not include this sworn and notarized disclosure statement.
