2 Pride in Wellness State College of Florida s Employee Wellness Program VISION Pride in Wellness creates a culture within the workplace that promotes and supports healthy behaviors. MISSION Pride in Wellness educates, encourages, and empowers employees to continuously improve their quality of life by offering a variety of wellness programs and resources that consistently meet their needs and interests. WellCREDITS: Employee Wellness Incentive Program Pride in Wellness, State College of Florida s employee wellness program, is rewarding employees for making positive healthy choices. Employees who complete various activities listed in this program are eligible to obtain rewards ranging from $50 to $100. Employees must be in an active status to be eligible to receive incentive. Additional ways to be rewarded are also available (see below for details). 1 P age
3 Using the WellCREDITS Tracking Form, employees will record the completion of various programs and assign the credits associated with each activity completed. All wellness program activities are completely voluntary. A detailed description of the wellness program activities and the three reward levels are listed below. Program Guidelines: The current WellCREDITS Incentive Program is a 6- month program, effective from July 3 through December 29, The next 6- months will begin in January 2018 and an updated program booklet will be available early in the New Year. There are some activities that require you to accurately track your participation. Most wellness activities are on an honor system and require self- reporting. You are asked to complete the forms with integrity and honesty. It is your responsibility not to violate this system, as the purpose of this wellness incentive is to reward those who actively engage in healthy behaviors. There are three qualifying events listed below. Only those employees who complete these events will be eligible to receive their reward. If it is unreasonably difficult, due to a medical condition, for you to achieve any of the wellness activities, please contact the Wellness Coordinator in Human Resources ( ), to discuss alternative options for you to fulfill the criteria. Who is eligible to participate in the WellCREDITS program? All employees may participate in the Pride in Wellness activities and programs. However, only those employees who are benefit- eligible can earn WellCREDITS for a reward. How do I earn and track my WellCREDITS? As you complete various wellness activities listed in this program, you will be earning WellCREDITS. Each activity has a specific credit value associated with it. Use the WellCREDITS Tracking Form to track your credits. When do I submit my Tracking Form? Please submit your tracking form to the Employee Wellness Coordinator (HR, Building 7) no later than January 5, The next 6- months will begin early 2018 and an updated program booklet will be available in which you can start accruing new points. Rewards will be issued on a 6- month basis. One submission per 6 months; twice per year. 2 P age

YES! Additional rewards can be earned by: 1. Wellness Challenge (see page 6) 2. Suncoast Credit Union mpower Challenge (see page 10) 3. Fresh Picks at the Cafeterias (see page 12) 4. 5 What are the three qualifying events that must be completed before I can receive my reward(s)? There are three qualifying events that must be completed at some point during the 6- month period before you can submit your WellCREDITS for your reward: Preventive Screening *Exercise 2 days of exercise a week Wellness Challenge *Exemption form is available if you cannot meet this requirement. 4 P age
6 Qualifying Event #1: Preventive Screening (10 WellCREDITS per screening; 30 Maximum) Criteria for Preventive Screening credits: Complete at least ONE screening in a 6- month period. We encourage you to check with your physician with regards to what screenings are appropriate for you. General Flu shot Comprehensive Blood Screening Dental Check Up Vision Exam Colonoscopy (50+) Fecal Occult Blood Screening (50+) Skin Cancer Comprehensive Screening Female Pap Smear Clinical Breast Exam Mammogram (40+) Male Prostate Specific Antigen (50+) Digital Rectal Exam (50+) (FALL SEMESTER ONLY) Health Screening at the Benefits Fair, including the completion of a Personal Health Assessment (10 WellCREDITS maximum) Health Screening: At the Annual Benefits Fair, you have the opportunity to receive an onsite biometric testing for: Blood Glucose Blood Pressure Body Mass Index (BMI) Cholesterol Ratio Total Cholesterol High Density Lipoprotein (HDL) At this health- screening event, you will receive the added benefit of meeting with a health coach face- to- face immediately after you receive your results. You will answer a series of questions about your health and lifestyle choices either on paper or electronically. Paired up with the biometric screening results, you will be given an overall wellness score and pertinent information about health risks, lifestyle choices, and risk reduction strategies. 5 P age
7 Qualifying Event #2: Wellness Challenge (25 WellCREDITS per challenge; 50 Maximum) Criteria for Challenge credits: Complete at least ONE challenge in a 6- month period. Wellness challenges are offered for 3-4 week periods and focus on making healthier lifestyle choices. There will be two challenges offered per every 6- month period. WellCREDITS are awarded once the challenge is COMPLETED based on the requirements. Each challenge also is a way to earn additional rewards: Walking Challenge 3 weeks October 16 November 3 Holiday Challenge 3 weeks December 11 December 29 Walking Challenge Water bojle Holiday Challenge Lunch Visit and click on the Wellness Challenge tab for detailed information about the upcoming challenge. 6 P age
8 Qualifying Event #3: Exercise 2 Days a Week (5 WellCREDITS every 30 minutes; 150 Maximum) Criteria for Exercise credits: 26 weeks in 6- month period: It is a 12- week commitment to exercise at least twice a week (you may begin this 12- week period at any time; make sure you indicate the start and end date on the Tracking Form). The length of your exercise session must be at least 30 minutes per day that is continuous movement in nature. Your 12- week session of exercise sessions can be any of the activities listed below: Group Exercise Classes (i.e. SCF Fitness classes, fitness center, or outdoor programs, such as BootCamp training) Self- lead physical activity that is continuous movement in nature (i.e. walking, cycling, jogging, swimming, weight training, ballroom dancing, line dancing, racquet sports, active yard work, rowing, kayaking, Pilates, and cardio training on elliptical, stair climber, etc.) What if I miss an exercise session during this 12- week period? Can I make it up the following week? The idea is to maintain your exercise sessions at least twice a week. If you miss a session in a week, you are permitted to add on a session the following week to make up for a missed session. 9 Physical & Social Wellbeing Pack a Healthy Lunch (1 WellCREDIT per healthy lunch brought to work; 120 maximum) Pack a healthy lunch to bring to work (whole grains, vegetables, fruits, lean meats, less processed, low sodium) Take a Walk Break (1 WellCREDIT per 15 minute walk; 120 maximum) Take a 15- minute walk during the work- day (time of day does not matter; 15 minutes must be consecutive). Take 15 Mindfulness Minutes (1 WellCREDIT per 15- minute mindfulness break; 120 maximum) Complete 15 minutes of breathing and/or meditation during the work- day (time of day does not matter; 15 minutes must be consecutive). Guided Stress Reducing Activity (5 WellCREDITS per activity; 20 maximum) Attend a guided stress reducing activity for 30 or more minutes, including: Adult Coloring group, acupuncture, professional massage, Tai Chi, meditation, etc. Healthy Cooking Class (10 WellCREDITS per class; 20 maximum) Attend a cooking class (Apron s, Manatee Technical Institute, SCF Corporate and Community Development, Viking Culinary, etc.) Race Participation 5K and above (20 WellCREDITS per event; 40 maximum) Participate and complete a cross- obstacle, adventure race, or race over a 5K. Personal Training Session (10 WellCREDITS per session; 100 maximum) Participant in a one- on- one session with a personal trainer (at home and/or at a fitness center) for at least 30 minute sessions with a trainer. Recreational Wellness (10 WellCREDITS maximum) Participate in a recreational sport such as softball, basketball, volleyball, kickball, ultimate Frisbee, flag football, etc. Commitment to participate weekly (8- week season or longer) 8 P age
10 Wellness Education Wellness Seminars/Workshops (10 WellCREDITS per seminar; 30 maximum) Attend a Pride In Wellness onsite wellness seminar or workshop (in person or via LifeSize; or a seminar in the community at least 60 minutes in length). IF YOU CANNOT ATTEND IN PERSON, you can still earn points (see below). Breezing through the Holidays Thurs, November 9, 2017 Healthy Holiday Eating Tues, December 5, 2017 Visit and click on the Wellness Seminars tab for detailed information about the upcoming seminars. NEW! Monthly Employee Assistance Program (EAP) Webinars through Aetna Watch live or recorded webinars from the on- demand library, then submit a written summary of what you learned and submit with tracking form at end of 6- months. Visit Username: SCF, Password: SCF, then click on Webinars Monthly WELCOA Health Bulletin Quiz (5 WellCREDITS per quiz; 25 maximum) Read the Monthly WELCOA Health Bulletins and complete the quiz on the Pride in Wellness website (located under the Resources tab). Please submit completed quizzes WITH THE MONTH LABELED ON THE TOP with your tracking sheet. Please submit with your tracking sheet for points. Health & Wellness Coaching Onsite ONE- ON- ONE Wellness Coaching Sessions (10 WellCREDITS per session; 40 maximum) Participate one- on- one wellness coaching sessions with a Certified Health & Wellness Coach. Sessions are minutes in length. See Individual Coaching on website. 9 P age
11 Onsite GROUP Wellness Coaching Sessions (10 WellCREDITS per session; 40 maximum) Participate in the group wellness coaching sessions with a Certified Health & Wellness Coach. Sessions are minutes in length and held once a month. See Group Coaching on website. Financial Wellbeing NEW! Pride In Wellness will be working with Suncoast Credit Union to bring SCF employees ongoing financial wellness support. Individual Suncoast Trust & Investment Session (10 WellCREDITS per meeting; 30 maximum) Bradenton Campus Tuesday, September 19 8:30am to 3:30pm Venice Campus Thursday, October 26 8:30am to 3:30pm LWR Campus Tuesday, November 7 8:30am to 3:30pm Suncoast mpower Challenge (10 WellCREDITS per program; 30 maximum) Take the mpower Challenge (4 programs: The Basics, The Next Steps, The Watchful Eye, and The Essentials) to learn money management skills online at suncoastcreditunion.com/mpower and win prizes. Each month, an SCF employee will be selected as our mpower Champion by taking on the mpower Challenge. This opportunity is exclusive to just State College of Florida employees. September Employees from all campuses can complete the Essentials mpower course on & submit their certificate to Teresa Przetocki by September 29th to enter a raffle. Prize: $250 VISA Gift Card. 12 Community Wellbeing Volunteer for Habitat, Food Bank, Reading Pal, Soup Kitchen, Hospital, Animal Shelter, Beach Cleanup, etc. (10 WellCREDITS per activity; 30 maximum) Donate Blood (10 WellCREDITS per visit; 30 maximum) Attend an SCF diversity event, art exhibition, Inter- Faith Dialogue event, musical/ theatre performance, or an athletic event. Visit for upcoming calendar of events. (10 WellCREDITS per activity; 30 maximum) Go to the SCF Library and check out a resource (book, DVD, etc.) with your staff ID that supports your wellness. (5 WellCREDITS per resource; 30 maximum) To help support you, visit our website under the Resources tab. Wellness Leadership & Development Lead a wellness activity for the employees at SCF - exercise class, a seminar/ workshop, etc. (10 WellCREDITS per activity; 20 maximum) Receive a certification in CPR/First Aid/Personal Trainer/Group Fitness Instructor, etc. (10 WellCREDITS per activity; 20 maximum) 11 P age
13 Additional Wellness REWARDS In addition to the VISA Gift Cards (page 3) and prizes awarded for completing a challenge (page 6), you can also get rewarded for: Fresh Picks at the Cafeterias - $25.00 CREDIT Pick up a SCF Fresh Picks card (seen below) at the Cafeteria, and then when you purchase a Fresh Picks meal from the Cafeteria (as outlined on the Fresh Picks menu available in each Cafeteria), receive a stamp from the cashier. After 5 stamps are received, turn the completed card WITH YOUR NAME ON THE BACK to Human Resources (building 7) to be entered into a monthly drawing to earn a $25.00 credit with the Cafeteria. Available on ALL campuses. Florida Blue Rewards Up to $ in Gift Cards To help clear up confusion of the two employee wellness programs, several activities and programs have been removed from the Pride In Wellness WellCREDITS programs and can now be counted towards Blue Rewards.
