Meningococcal immunization is often overlooked, as it is a recommended rather than required immunization. However, this disease can have devastating and deadly results. Death can result in less than 24 hours from onset of symptoms; survivors often suffer from debilitating sequelae such as disfigurement and loss of limbs. The funerals were due largely to diabetes and other conditions associated with poor diet. In response to this, Reverend Joyner started a small community garden tended to by the young church members. The youth led garden effort grew quickly, producing healthy and quality food for their families and community. Currently, 60 students tend to the 25 acres of vegetables with 130 bee hives during after-school and summer programs. The produce is sold to local restaurants, schools, and other organizations with proceeds being reinvested in the community. This successful community development initiative has resulted in local economic and leadership development and improved health in the community. Pain is multidimensional, with a high prevalence of psychiatric disorders in chronic pain. In this session, participants will hear about the key elements of integrated healthcare and how adopting these practices can improve outcomes for chronic pain patients. Participants will also hear how to identify chronic pain patients with substance dependency and how to intervene when chemical coping is identified. By the conclusion of this session, 1. Identify the need for evidence based interventions for substance dependence; 2. Identify pharmacological interventions for substance abuse in a clinical practice; and 3. There is currently no vaccination to prevent Hepatitis C and treatment can be challenging as patients will sometimes have little or no symptoms. In this session, participants will hear how Hepatitis C is affecting North Carolina and the best way to prevent the transmission of Hepatitis C once someone is diagnosed. By conclusion of this session, participants will be able to: 1. Identify the areas of North Carolina most affected by acute Hepatitis C; 2. Describe factors influencing the increase in Hepatitis C cases reported in NC; and 3. Identify prevention methods that can help reduce hepatitis C. Research suggests that patients are more knowledgeable and open to the care they receive when this model is utilized. Additionally, a description of the SDM Toolkit will be discussed and an overview of the ADAPT-NC and ACE Studies will be shared, providing participants the various components of SDM as it relates to asthma. Utilize an online toolkit for asthma shared decision making. Self-monitoring of blood glucose (SMBG) in patients with type 2 diabetes that do not use insulin is debatable. In this session, presenters will debate the pros and cons of SMBG testing.
