If you are not already a member of the John L. Berrey Fitness Center you will receive a FREE 90 day membership upon registration!!~ There will also be 2 additional contests in conjunction with the Warrior Weight Loss Challenge Attached you will find the rules, regulations and registration form. 3 Warrior Weight Loss Challenge Rules & Regulations ~~If you are not already a member of the John L. Berrey Fitness Center you will receive a FREE 90 day membership upon registration!!!!~~ RULES 1. The competition will have a men s division and women s division. 2. The Warrior Weight Loss Challenge will begin on Tuesday January 17, 2017 through Friday March 17, Initial registration will be Thursday January 12, 2017 and Friday January 13, 2017 during normal business hours. Initial weigh in will be January 17, 2017 and Final weigh in will be March 17, The weigh in will be recorded by a staff member of the John L. Berrey Fitness Center. If you are unable to make it to the weigh in on either of these dates, you must make arrangements with Sherrie Nida, Director John L. Berrey Fitness Center. 3. To weigh in: ~Must empty pockets ~Must remove shoes ~Cannot wear hoodie, sweat shirts, jackets, coats or jeans 4. Winners for the Warrior Weight Loss will be based on a percentage of weight lost. 5. There will not be a fee to participate. This is a free registration.

4 6. Prizes: Men s Division 1st Place: TBA Women s Division 1 st Place: TBA 2 nd Place: TBA 2 nd Place: TBA 3 rd Place: TBA 3 rd Place: TBA 7. As a contest participant, you are encouraged to participate in the monthly health screenings sponsored by the Quapaw Tribe CHR Program. 8. We will also have 2 (two) companion contest going on at the same time. You may participate in all 3 (three) contests. This is not a requirement. Contest #2 is Most Time Visiting the fitness center. Contest #3 is Most Time Spent Working out in Fitness Center. Daily and Weekly sign in sheets will be required. We will also be giving 1 st, 2 nd, and 3 rd place overall prizes in both companion Contests. 9. This contest and the 2 companion contests are open to everyone with the exception of the employees/staff of the Quapaw Tribe CHR Program and the John L. Berrey Fitness Center. Restrictions 1. If you have had a weight loss surgery in the last year, you are not eligible to participate in the challenge.

5 2. If you are currently or begin taking any diet medication, prescription or over the counter, you are not eligible to participate in the challenge. 3. Anyone that is pregnant or becomes pregnant during the challenge is not eligible to participate in the challenge. CONTESTS #1 Warrior Weight Loss Challenge #2 Most Times Visiting Fitness Center #3 Most Hours Spent in Fitness Center The contests are sponsored in part by the Quapaw Tribe CHR Program and the Southern Plains Tribal Health Board. The Quapaw Tribe CHR program has been awarded a 2017 Good Health and Wellness grant by the SPTHB and the Oklahoma Area Tribal Epidemiology Center in Edmond, Ok.
