2 Mission & Areas Served Statements Mission Statement The mission of Miles of Smiles, Inc. is to provide charitable, comprehensive, portable dental care, in a community-based setting, to the needy children of Clay and Platte Counties. Background Statement Miles of Smiles was founded in 2002 as a partnership between Northland Health Care Access and Swope Health Services in response to Vision North 2000, a study that determined one of the gaps in Northland services was dental care for children. At the time, it was estimated that 15,000 children in the Northland had no access to dental care. Our services were portable from the beginning, as the need indicated at the time. In 2004, the Northland Health Care Access Board of Directors determined the portable dental program would be best operated as an independent agency with its own office space. In December 2005, Miles of Smiles became an independent agency, designed specifically to meet the unique needs of low-income children without dental care through portable service. Since our inception, more than 20,000 low-income children have received comprehensive dental care through Miles of Smiles. Our history and experience provides practice-based evidence that our program works and, through our existence, low-income children have increased access to consistent oral health care. Impact Statement Since 2002, Miles of Smiles has provided free dental care to more than 20,000 low-income children living in Clay and Platte Counties in Missouri. Without Miles of Smiles, many of these children would not have received either the preventative or restorative dental care they so desperately needed. We ensure accessibility of our program by offering treatment in schools, social service agencies, and in collaboration with other community based agencies, as well as at our home office. Top accomplishments from 2016 include the following: 1.The number of new patients served by Miles of Smiles increased by 5%. 2. Participation by our patients in recalls (six-month check up appointments) increased by 15%. We are the dental home for our patients. Tracking this outcome is important to demonstrate ongoing oral healthcare. 3. Revenue generated by our agency through special events increased by 14%. We continue to seek diversification in our funding sources to ensure long-term sustainability. Special events are excellent opportunities to introduce Miles of Smiles to potential new donors. Miles of Smiles treated 3,764 children in % of patients had reduced cavities and 79% of patients completed treatment plans. Most of the patients we see have not been to the dentist before and require restorative treatment. This is prescribed by our staff in the form of a treatment plan, and its completion is critical to their improved oral health. We track completion to make sure we're doing all we can to improve our patients' health. We also documented a 10% increase in the number of patients enrolled in Medicaid. Most of our patients are eligible for Medicaid but not all are enrolled, for a variety of reasons. We do all we can to eliminate those barriers, because enrollment in Medicaid will open up other health care options for our target populations. Our goals for the coming year include moving to a larger office to have three permanent operatories (up from two in our current location); adding a mobile unit so we can treat more children in accessible locations; and increasing the number of children we see at Title I schools by 5%. 2

3 Needs Statement Miles of Smiles' top needs at this time include: 1. Funding to support the purchase of our mobile unit ($100,000). We will rent a mobile unit, fully equipped as a dental office, in the fall of 2017 with the intent to purchase the vehicle in the spring of While our two dental teams are portable, they set up temporary dental offices inside school buildings and other communitybased locations for up to two weeks at a time. We believe the addition of a mobile unit to our fleet - a vehicle that will be permanently equipped inside as a dental office with operatories and all needed equipment - will help us to treat more children at each partner location, and add new community based partners as well. We see great opportunity to take the mobile unit to service agencies, churches, and apartment complexes as time and scheduling allow to bring affordable dental care to more children. 2. Funding to install plumbed fixed nitrous service in our new office location ($20,000). We currently use rolling, portable nitrous tanks which take up a lot of storage space in the operatories and are costly to operate. When we move to our new office location, we plan to install permanent nitrous access to improve efficiency and decrease ongoing cost. 3. Funding for advanced dentistry training / continuing education for our licensed dentists and dental assistants ($2,000). 4.Funding for a new cargo van for hauling equipment ($40,000). 5.Funding for medical storage lockers in our new office ($6,000). Board Chair Statement Miles of Smiles treated 3,764 patients in 2016! The year 2016 marked our first full year in our new office at 5536 NE Antioch Road. The new office space and an additional dentist on staff, allowed us to see more emergency visits, complete more treatment plans, and overall be more available to our patients to serve as their dental home. The relationship that Miles of Smiles dental staff have with these children, in the school setting and at our office, builds trust and a desire to improve their oral health. Last year, Dr. Sparks treated a kindergarten student named Sam who had never before been to the dentist. The student was very anxious and did not want Dr. Sparks to check his teeth he even tried to hide under a table! Dr. Sparks counted his teeth during the first visit, then took pictures at the next, cleaned them at the next and then performed a few fillings. With each visit, Sam received hugs and high fives from the dental team, so even when his treatment was complete he came by the dental clinic in the school for a morning hug. The hugs, the oral health education, the free dental treatment are the immediate things that Sam got. The long-term benefits of the dental care he received include self-confidence and high self-esteem, a beautiful smile and improved overall health! At Miles of Smiles we strive not just to meet immediate dental needs, but also to lay a foundation of a positive, dental care experience that leads to a lifetime of happy smiles and overall health. 4 Programs Programs Free, Portable, Safety Net Dental Care for Low-Income School-Age Children Description Category Population Served Short-Term Success Dental care is the most common unmet treatment need in children due largely to lack of insurance or inability to pay. One in three children do not have dental insurance, translating to an estimated 17,000 children in the Northland who are uninsured or underinsured for dental care. Our portable dental program offers comprehensive dental care to children who live in Clay or Platte Counties. Services are offered free of charge to children who meet the 200% poverty guidelines. We use cargo vans to deploy two dental teams and equipment to schools and other communitybased settings, such as social service organizations and churches. Each team consists of one dentist and two dental assistants. Staff are full time employees of Miles of Smiles not volunteers and work five days a week, twelve months a year. Miles of Smiles is the dental home to the low income children of the Northland. We are available to our patients not only in community settings, but also at our clinical office and clinic at Synergy s teen center. Health Care, General/Other Dental Health Care Children and Youth (0-19 years), Poor,Economically Disadvantaged,Indigent, Miles of Smiles is committed to achieving the following results during the coming year: 90% of patients will complete a comprehensive treatment plan. Completing comprehensive treatment plans eliminates the need for further dental work and prevents pain, discomfort, and missed school. Staff follow up with parents on available appointment times, to check in on patients following procedures, and to assist families in completing comprehensive treatment plans. Treatment plans are prescribed for those defined as "most clinically-needy" by dental teams. These children need restorative work done to at least 4 teeth. 50% of patients will have fewer cavities in subsequent appointments than in their first appointment. We monitor a decrease in cavities as an immediate indicator in behavioral change. At every appointment children are taught the importance of good nutrition and oral hygiene. A decrease in cavities verifies that lessons are heard and applied in daily lifestyle. Long- Term Success The vision of Miles of Smiles is to accomplish the following for the youth served: Improved dental hygiene practices Improved nutritional status Improved overall health Improved self-esteem Increased awareness of the importance Improved community health status of dental health 4

5 Program Success Monitored By Examples of Program Success Miles of Smiles uses professional software developed for dental practices that allows tracking of each patient, treatment plans, procedures performed, scheduling and follow-up. Individual medical information and emergency information are available in the database for each patient. The program compiles and reports the number of patients treated within a given time frame, types of procedures, locations of treatment, and Medicaid status along with other information Results: 3,764 children treated 81% of patients had reduced cavities 79% of patients completed their prescribed treatment plan 10% increase in number of patients enrolled in Medicaid 5% increase in number of patients seen CEO Comments Miles of Smiles is experiencing growing pains we moved to a new office in mid The increased patient care space in our fixed office allowed for increased patient volume of 36%. The patient volume is primarily from increased appointments in our new two-operatory office. Now, two years later we are moving to another larger office to keep up with the needs of our growing office appointments, need for more storage and sterilization space as well as patient care space. We anticipate our new three operatory office will be open by late August The primary challenge we face is getting children to enroll for service at the schools. Miles of Smiles is a school-based/community-based dental program and we want our patients to be seen at the schools. We have full time dentists, credentialed and highly qualified, who use state of the art equipment to provide comprehensive dental care inside the schools for easy access and convenience for families. The number of children enrolling in our school-based programs is not growing as we would like. We have added an online registration process to ease the burden of enrolling by paper form and are seeking additional ways to increase enrollment. 5

6 Leadership & Staff Executive Director/CEO Executive Director Mrs. Christy May Term Start Dec Experience Before coming to Miles of Smiles, Christy May was the practice administrator of a large private neurology practice and simultaneously managed the MidAmerica Neuroscience Research Foundation from (10/96-1/2000). From there, she held a position as Director of Community Development at the University of Health Sciences (1/2000-1/2002). Since January of 2002, Christy has managed the Miles of Smiles portable dental program as it evolved from a program with Northland Health Care Access, to its now independent nonprofit status as Miles of Smiles, Inc. Christy has a bachelor's degree from the University of Tulsa in Psychology and Organizational Communication and a master's degree from UMKC in Health Care Administration.
