4 Highlights of 2011 Community Benefits In the spirit of our core values of community and commitment to those who are poor, Holy Cross provides programs and resources that improve our community s health. These programs make a huge difference in the lives of the people in our community. We work in partnership with local organizations, community groups and health care professionals for a better understanding of local health needs and to facilitate the development of cooperative prevention strategies and improved outcomes. In 2011, Holy Cross contributed: $17.5 million in care for the disadvantaged, community benefits and education $3.6 million in net cost of hospital patient care services, including traditional charity care, public programs and other programs

6 Growing Healthy Kids The Growing Healthy Kids program features on-site produce gardens; student, teacher and food service worker nutrition and wellness trainings; and a Pilates for Girls program which focuses on fitness, good eating habits and self-esteem. In 2011, Growing Healthy Kids served: 30 parochial schools 9,323 students 177 school-based produce gardens 737 nutrition lessons and activities taught to students 105 training sessions for 102 teachers and 41 food service workers 18 nutrition trainings to 300 parents 12 community gardens involving 300 community members 22 students through the Pilates for Girls program

8 Parish Nurse Helps Seniors The Parish Nurse program (and Parish Nurse Starquest program) helps seniors maintain their independence, integrity, social connections and optimal physical, mental and emotional health. Assistance at parish locations includes education, counseling, care navigation, maintenance of senior independence, and chronic disease management. The Starquest program provides parishioners with free transportation to medical appointments and social events through a partnership with St. Vincent de Paul, North District Council. In 2011, the program served/provided: 5,084 seniors (2,988 at the Parish) 49 health education classes for 1,021 individuals 72 support groups for 582 individuals 174 home visits 270 hospital visits 142 nursing home visits 877 senior riders 375 transports for medical and social intervention

10 promoting healthy habits Holy Cross Hospital s Community Outreach program served 82 percent more homeless individuals in 2011 than the previous year. Some of the efforts to help the homeless population in our community included a children s summer reading book drive; a puppet show attended by more than 100 of our community s sheltered homeless families which taught children and parents about the importance of good nutrition and active lifestyles and weekend food backpacks for HIV-positive and homeless people to provide access to food when soup kitchens are closed.

12 Mobile Health Comes to you The Holy Cross Mobile Health program was created in 2010 to bring health care to communities that lack access to it. The 33-foot vehicle is outfitted with two patient interview and screening rooms, one exam room, onboard laboratory services and a client waiting area with a monitor to deliver educational messages. The program provides physical exams, women s clinical breast exams, health screenings (blood pressure, sugar and cholesterol), rapid HIV and hepatitis C testing, and back-to-school well-child physicals and immunizations. In 2011, the Mobile Health program served 1,650 individuals.

14 A Goodstart for every age In line with the Sisters of Mercy s commitment to the care of the poor, women and children, the Goodstart Program was created in 1992 to increase knowledge within our community and instill good health behaviors. The program features 194 preschool age health classes taught on site at child-care centers; eight health classes and a Book Club for youth at the Jan Moran Collier City Learning Library; 10 four-hour child-focused workshops for day care providers; and nine in-service programs supporting intergenerational programming for seniors and preschool children. In 2011, Goodstart served: 2,584 preschool children 404 school-age children 1,650 adults 190 seniors

16 health program in the classroom Through Holy Cross Hospital s Community Outreach program, the School Health Program aims to improve health by providing health education classes, on-site screenings and immunizations. The School Health Program provides these services to all Broward County elementary parochial schools (kindergarten through eighth grade). In 2011, the School Health Program served or provided: 5,283 school-age children and youth 201 adults 6,916 screenings (8 percent referred for follow-up care) 1,200 immunization compliance reviews 904 immunizations to 361 children 98 classes to 1,575 students 12 health and CPR classes to 282 school staff members 46 parent/teacher conferences 36 classroom observations 26 classes to female youth at the Broward Regional Juvenile Detention Center

20 Partners in better Breast Health Established in 2011 through the support of organizations such as the Florida Breast Cancer Foundation, Komen for the Cure Miami/Fort Lauderdale and the Club to Benefit Cancer, the Partners in Breast Health program aims to reduce health disparities and improve early cancer diagnosis and access to treatment adopted from an evidence-based model of care. In 2011, Holy Cross Hospital s Partners in Breast Health Program provided 252 free screening mammograms to the community in an effort to reduce advanced diagnoses and cancer mortality in Broward County. Patients who were diagnosed with breast cancer were ensured follow-up care through the completion of their care.

22 filling pantries eases hunger Holy Cross Hospital continues to assist in the collection, distribution, donation, facilitation and/or provision of food to small food pantries in Broward County. Every day, our Associates contribute food items, canned goods and change from our hospital s cafeteria and Heart Healthy Café. One in six children in Broward County goes to bed hungry every night, and one in four is food-insecure. Our Associates are committed to helping with our hunger initiatives, which resulted in 1.2 million pounds of food being collected and donated to the community in 2011.

24 giving the gift of knowledge Through a Florida Department of Health, Health and Human Services contract award, Holy Cross Hospital s Community Outreach program offers a free 20-minute Rapid HIV Test. In 2011, of the 8,871 individuals who had contact with this program, 3,633 underwent HIV testing, and 25 percent of them were homeless. There was a 1.03 percent positivity rate. The program also provided 1,646 health screenings for blood glucose, blood pressure and total cholesterol. Our Community Outreach program is also one of six programs nationwide to implement a pilot Rapid Hepatitis C Testing project.
