2 COURSE DESCRIPTION The Ontario Medical Association in collaboration with the Champlain LHIN and local primary care leaders is pleased to offer a primary care course on Caring for the Elderly in Champlain: The Primary Care Perspective. This interdisciplinary event will provide an opportunity for physicians and other primary care providers through the Champlain LHIN to connect, exchange information, tools and innovations in primary care, leading the way to healthier communities and better care for our elder population. PLEASE NOTE THAT ALL SESSIONS ARE IN ENGLISH LEARNING OBJECTIVES By participating in this course, participants will be able to: Develop an understanding of a population perspective for care of the elderly in your practice who are my elderly patients, what do they need?
