2 In response to global aging population trends and increases in life expectancy, health professionals in many nations have recognized the importance of developing care practices focused on maintaining and promoting the health of older adults. Components of health including quality of life, personal growth, and healthy relationships each contribute to mental health or mental wellness. Mental wellness in older adults can be fostered by promoting patient empowerment and resilience through communication, education, engagement and coordinated assessment, treatment and transitions of care. Learn how you can foster mental wellness in older adults through your role as a geriatric health/behavioral health provider. learning objectives By the end of this conference participants will be able to: Describe how the paradigm of resilience facilitates support of mental wellness in older adults. Cite the least intrusive legal means to provide a substitute judgment for an incapacitated adult. Identify patterns of substance abuse and treatment options for older adults. Describe strategies to promote improved communication and effective interaction between older adults, their family caregivers, and geriatric health/behavioral health providers. Review assessment of and interprofessional/ interagency responses to hoarding in older adults. Identify diagnostic and assessment criteria for common mental health conditions in older adults according to the DSM-V. Who Should Attend: Physicians, Mid-Level Practitioners (PA, NPs), Social Workers, RNs/LPNs, Therapists, Case Managers, LTC/AL Facility Administrators and educators and students would benefit from attending this conference.

3 CONTINUING EDUCATION CREDIT Application has been made requesting 6 credit hours to all the disciplines below: Physicians This conference has been submitted for 6 Prescribed credits to the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity. AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 Credit toward the AMA Physician s Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1. Nurses Application has been made to The University of Arizona Center on Aging, approved provider of the California Board of Registered Nursing for 6 contact hours. Social Workers Application has been made for 6 social work continuing education credits to the Arizona Chapter of the National Association of Social Workers. Case Managers Application has been made to the Commission for Case Manager Certification to provide 6 continuing education credits to Certified Case Managers. LTC/ALF Administrators/Directors Application has been made to the Board of Examiners of Nursing Care Institution Administrators and Assisted Living Facility Managers for 6 continuing education credits for managers and administrators of long term care and assisted living facilities.
