2 A letter from the AOCMF North America Education Council Chairperson As Chairperson of the AO North America Craniomaxillofacial (CMF) Council, I would like to take this opportunity to share with you some thoughts on AO Education. Founded in Switzerland in 1958 under the name Arbeitsgemeinschaft für Osteosynthesefragen, the AO today is the world leader in the study of stable internal fixation and is known as the Association in Osteosynthesis. AO North America (AONA ) is a non-profit organization based on a network of surgeons committed to the study, practice, and teaching of AO principles and their advancement in the field of trauma and reconstructive surgery; working in the areas of research, development, education and quality assurance for the benefit of improving patient care. The focus is to develop and support graduate and post-graduate education. AONA is an accredited CME provider and is committed to providing the highest quality education for surgeons at all stages of their development. AONA offers a comprehensive spectrum of education activities to support surgeons who strive to be lifelong learners. AONA is dedicated to providing the highest quality educational experience for course participants and Faculty. This is evidenced by the AONA Craniomaxillofacial Systematic Education Approach as described below: Within the AONA CMF organization, the three primary specialties, oral and craniomaxillofacial surgery, otolaryngology/head and neck surgery, and plastic and reconstructive surgery meet on a constructive basis and develop a joint approach. Recently oculoplastic surgeons and neurosurgeons have joined the organization to further enhance the educational opportunities. AONA CMF courses are designed to teach principles and techniques at both the theoretical and practical levels. The aim is not to popularize the use of indiscriminate surgical fixation but instead to evaluate scientifically its place in the care of the sick or injured, and to refine surgical practice for optimized patient outcomes. These courses include the annual Advanced Symposium (Level III), Challenges and Advances courses (Level II) and the Principles courses (Level I). Principles Course Objectives (Level I) Describe the principles of stable internal fixation as outlined by the AO; Define the biological and mechanical aspects of fracture healing; Discuss the problems, complications and intraoperative difficulties that can result from internal fixation; Explain preoperative planning methods and outcome analysis; and Apply the psychomotor skills developed in the practical exercises into surgical practice.

3 These objectives are reached through lectures, hands-on exercises, panels and small group discussions. This course is designed for Junior Residents, Fellows and Practicing Surgeons looking to refresh their skills. The course presents the basic information regarding the AO Principles of Stable Internal Fixation: Anatomic Reduction of the fracture fragments, particularly joint fractures; Stable fixation to ensure proper healing of the fracture allowing surrounding tissue to move and strengthen; Atraumatic surgical technique to preserve blood supply to the bone fragments and soft tissue; and Early, pain free mobilization returning the patient to function as soon as possible Challenges and Advances Objectives (Level II) are determined by the programs. Examples of courses offered to date include: Cadaver Practical Workshops, Distraction Osteogenesis, Orthognathic, Navigation, Pre surgical Planning and Patient Specific Implants, Orbital Reconstruction, Management of Ballistic Trauma, and Pediatrics to name a few. These courses are conducted both didactically and with practical exercises (most with cadavers) and also provide opportunities for panels and small group discussions. These courses are designed for Practicing Surgeons, Fellows and Residents that have completed the Principles course. The annual Advanced Symposium (Level III) is conducted to address the most challenging issues facing surgeons today. The program is didactic and allows time for small group discussions to assist surgeons in understanding how best to address their concerns. This course is geared towards the Practicing Surgeons, Fellows and most senior Residents. 4 4 Selected Topics in Orthognathic Surgery March 24, 2018 CME Mission Statement The Continuing Medical Education (CME) mission of AO North America (AONA ) is to provide comprehensive multidisciplinary needs-based education to surgeons, fellows, and residents in the specialties of orthopedic, hand, craniomaxillofacial, spine, neurosurgery, and veterinary surgery in the areas of trauma (i.e., operative reduction and fixation), degenerative disorders, deformities, tumors, and reconstruction. Expected results of AONA s CME activities for surgeons, fellows, and residents are to: Increase their knowledge base and surgical skill level Improve competence by applying advances of knowledge in patient care in the areas of trauma, degenerative disorders, deformities, tumors, and reconstructive surgical techniques Address practice performance gaps by improving management of all aspects of musculoskeletal injuries and disorders (i.e., pre-operative planning to post-operative care)

5 Symposium Description Selected Topics in Orthognathic Surgery March 24, The evolution of Orthognathic principles and practices, combined with knowledge of recent advances, helps the clinician more effectively treat challenging clinical problems. Coordinated comprehensive treatment planning primarily between the orthodontist and the orthognathic surgeon will help improve patient outcomes and decrease complications. Continuing education benefits clinicians by exposing them to new knowledge in the field, which in turn, leads to improved patient outcomes. Target Audience This Symposium should be of special interest to attending physicians/surgeons/dentists and residents in Oral and Maxillofacial, Otolaryngology - Head and Neck, Plastic Surgery and Orthodontics.

6 6 Selected Topics in Orthognathic Surgery March 24, 2018 Learner Objectives Upon completion of the symposium, participants should be able to: Identify advances and techniques in orthognathic surgery practice that contribute to functional and aesthetic improvement for patients Describe prevailing approaches to both routine and complex orthognathic clinical care problems Recognize the factors involved in a calculated treatment plan for orthognathic patients to achieve a predictable outcome including a thorough evaluation, proper diagnosis and setting realistic expectations Utilize an interdisciplinary approach in the evaluation and treatment of patients with dentofacial deformities Accreditation AO North America is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. AONA has been resurveyed by the Accreditation Council for Continuing Medical Education (ACCME) and awarded Accreditation with Commendation. Continuing Education Dental Credit Statement As an Accreditation Council for Continuing Medical Education (ACCME) accredited provider, AO North America meets the definition of a constituent or component organization of the AMA and thereby meets most state dental board requirements of an approved sponsor of continuing education. This Symposium is focused on clinical issues in oral-maxillofacial surgery that are relevant to the treatment and care of dental patients. Most states accept AMA constituents as approved sponsors for continuing dental education credit. If you have questions, your state dental board can confirm eligibility of this symposium. Designation Statement AO North America designates this live educational activity for a maximum of 7.5 AMA PRA Category 1 Credits. Physicians should claim only the credit commensurate with the extent of their participation in the activity. 7 Selected Topics in Orthognathic Surgery March 24, Faculty The Faculty for this course includes surgeons distinguished in the fields of Oral and Maxillofacial Surgery, Otolaryngology Head and Neck Surgery, Plastic and Reconstructive Surgery and Orthodontists. Course Chairman Jeffrey C. Posnick, DMD, MD, FRCSC, FACS Educated in both dentistry (Harvard University) and medicine (Vanderbilt University), Dr. Posnick is American board certified in both Oral and Maxillofacial Surgery and Plastic Surgery. His professional appointments include Clinical Professor of Surgery and Pediatrics at Georgetown University in Washington, DC, Clinical Professor of Orthodontics at the University of Maryland, School of Dentistry, Baltimore, Maryland and Adjunct Professor of Oral and Maxillofacial Surgery, Howard University College of Dentistry, Washington, DC. Dr. Posnick has lectured around the world and authored more than 300 medical journal articles and book chapter. His earlier two volume textbook, published by W.B. Saunders, Craniofacial and Maxillofacial Surgery in Children and Young Adults, is considered an authoritative reference. His current comprehensive two volume textbook Orthognathic Surgery: Principles and Practice published by Elsevier, captures the latest thinking for the evaluation and treatment of all aspects of dentofacial deformities. Dr. Posnick conducts an active clinical practice and serves as director of the Posnick Center for Facial Plastic Surgery in Chevy Chase, Maryland. Course Co-Chairmen Damir Matic, MD, MSc, FRCSC Dr. Matic finished both his medical school and residency training in Plastic Surgery at the University of Toronto. He completed a fellowship in Craniofacial surgery at Johns Hopkins Hospital and at the University of Maryland. After working at St. Michael s Hospital in Toronto for a year he moved to London, Ontario in He is currently an Associate Professor at Western University, working at Victoria Hospital which is a tertiary care pediatric and trauma hospital. His primary clinical interests include cleft lip and palate, acute and secondary facial trauma reconstruction, facial nerve re-animation as well as all aspects of paediatric craniofacial surgery. He completed a Master s of Science degree in London at Western University in the Department of Medical Biophysics. His current research interests include facial growth, the effects of botulinum toxin on nerve regeneration, and long term analysis of cleft lip repair and orbicularis oris muscle motion. Travis T. Tollefson, MD, MPH, FACS Dr. Tollefson is Professor and Director of Facial Plastic & Reconstructive Surgery at the University of California Davis, where he specializes in cleft and pediatric craniofacial care, facial reconstruction and facial trauma care. His interest in the emerging field of Global Surgery and improving surgical access in low-resource countries led him to complete an MPH at the Harvard School of Public Health. He leads the CMF arm of the AO-Alliance.org, whose goal is to instill AO principles in facial injuries in low resource settings. A native Kansan, he attended medical school and residency in Otolaryngology Head and Neck Surgery at the University of Kansas. His fellowship through the American Academy of Facial Plastic and Reconstructive Surgery was completed at UC Davis. His current research is focuses on clinical outcomes of patients with cleft lip-palate, facial trauma education in Africa, and development of ipad games for adjuvant speech therapy, and developing artificial muscle for facial paralysis. He coedited, Complete Cleft Care, an evidence-based textbook on team management of cleft lip-palate. He serves as his department s Director of Mentoring, as a Faculty Educator for the AOCMF, on the Board of the American Academy of Facial Plastic Surgery, and the editorial board for JAMA Facial Plastic Surgery journal.

MacLeod, BDS, MB ChB, FDS RCS (ED&ENG), FRCS (ED), FFST RCS (ED), FACS Stephen MacLeod is the Chief of Oral & Maxillofacial Surgery and Dental Medicine Program Director of the Oral and Maxillofacial Surgery Residency Program at Loyola University Medical Center. Dr. MacLeod is a graduate of the University of Dundee School of Dentistry. His medical degree is from the University of Aberdeen in Scotland. Dr MacLeod completed his training in oral and maxillofacial surgery in Scotland and was an AO Fellow at the University of Louisville. Dr. MacLeod has been actively involved in research, postgraduate training and education. He has multiple publications and has presented his original research at both national and international meetings. This includes being the primary author on over twenty-five peer-reviewed scientific publications on topics. He has recently authored book chapters on treatment of mandibular fractures, frontal sinus fractures and the management of osteoradionecrosis. Dr. MacLeod practices full-scope oral and maxillofacial surgery. His clinical interests are reconstructive surgery, maxillofacial trauma, and benign bone. Invited Faculty Samir Mardini, MD Dr. Mardini is a Professor of Surgery and Consultant in the Division of Plastic Surgery at Mayo Clinic in Rochester, Minnesota. Dr. Mardini is Program Director, Division of Plastic Surgery; Director, Facial Paralysis and Reanimation Clinic; Co-Director Cleft and Craniofacial Clinic and Surgical Director for the Essam and Dalal Obaid Center for Reconstructive Transplant Surgery. Dr. Mardini completed his general surgery and plastic surgery residency at Georgetown University. He then completed a one-year Craniofacial Fellowship, a two-year Reconstructive Microsurgery Fellowship, and two years in an academic plastic surgery position before joining the staff at Mayo Clinic. Dr. Mardini s practice focuses on microsurgical reconstruction and craniofacial reconstruction. Dr. Mardini has published over 120 peer reviewed papers, 20 book chapters, and three books. Dr. Mardini is an editorial Board member and Guest Editor for Seminars in Plastic Surgery, Editorial Board Member for Microsurgery, and manuscript reviewer for Microsurgery, Surgery, Plastic & Reconstructive Surgery, and Seminars in Plastic Surgery. He is on the Council of the American Society of Reconstructive Microsurgery and the World Society of Reconstructive Microsurgery. Dr. Mardini is a member of the American Society of Plastic Surgery, a founding member of the American Society for Reconstructive Transplantation and the World Society of Reconstructive Microsurgery. Dr. Mardini received the Godina Traveling Fellow Award for 2011 and the Ahmad M. Obaid Honored Scholar in Reconstructive Surgery at Mayo Clinic. Dr. Mardini is also a volunteer surgeon for several medical mission trips each year. Timothy J. Tremont, DMD, MS Dr. Tremont received his Bachelor of Science and Dental Degree from the University of Pittsburgh. He completed an Orthodontic residency and a Master of Science at the University at Buffalo before entering private practice limited to Orthodontics in White Oak, Pennsylvania. Dr. Tremont is a Diplomate of The American Board of Orthodontics and Clinical Assistant Professor in the Department of Orthodontics at West Virginia University School of Dentistry. His major area of interest includes: developing The Four Faces of Orthognathic Surgery, a method and device for achieving accurate surgical results. He is an active teacher of the Andrews Six Elements of Orofacial Harmony. He has published scientific articles in The American Journal of Orthodontics and Dentofacial Orthopedics, Journal of Clinical Orthodontics, and The Andrews Journal.

9 Selected Topics in Orthognathic Surgery March 24, Faculty (continued) Invited Faculty (continued) Timothy Turvey, DDS After receiving his dental degree from Columbia University, Dr. Turvey trained in Oral and Maxillofacial Surgery at Parkland Memorial Hospital. Since 1994, he has served as Professor and Chairman of the Department of Oral and Maxillofacial Surgery at the University of North Carolina. He has published more than one hundred peer reviewed manuscripts, book chapters and co-edited two major text books including Orthognathic Surgery and another entitled Facial Clefts and Craniosynostosis. He has received numerous honors and awards including the William H. Geis award for excellence in Oral and Maxillofacial Surgery. Dr. Turvey is considered a world renowned Orthognathic Surgeon. John Wallace, DDS, MD John Wallace is a native Texan and spent most of his childhood growing up in Austin and Houston. He began his college education at 16 years old, at the University of North Texas through a state wide math and science magnet school program called The Texas Academy of Mathematics and Science. Upon completion of his high school requirements, Dr. Wallace transferred to The University of Texas at Austin where he majored in Biology. He received his dental degree from The University of Texas at San Antonio, where he graduated Summa Cum Laude, with honors in research. After earning his medical degree from The University of Texas Southwestern Medical Center, Dr. Wallace completed his general surgery and oral and maxillofacial surgery training at Parkland Memorial Hospital. He concluded his training with a fellowship in craniomaxillofacial and orthognathic surgery at the world-renowned Posnick Center and Georgetown University Hospital. Currently, he is in private practice in Dallas, Texas. Local Faculty George Obeid, DDS Dr. Obeid is the chairman of the Department of Oral and Maxillofacial Surgery at Medstar Washington Hospital Center in Washington DC. He received his training in oral and maxillofaxial surgery at the Queen Victoria Hospital, East Grinstead, England and the Washington Hospital Center, in Washington DC. Dr. Obeid served on the Advisory Committee for the American Board of Oral and Maxillofacial Surgery and was a board examiner. He is an attending surgeon at Children National Medical Center where he is also a member of the craniofacial team. Dr. Obeid s clinical practice covers the full scope of oral and maxillofacial surgery with special interest in orthognathic surgery. Steven Siegel, DMD Dr. Siegel earned his DMD degree from Tufts University School of Dental Medicine and received his certificate in orthodontics from the University of Medicine and Dentistry of New Jersey. Upon graduation from dental school, he served as a Commissioned Dental Officer in the United States Public Health Service. Dr. Siegel is a member of the American Association of Orthodontists, the Middle Atlantic Society of Orthodontists, the Maryland State Society of Orthodontists, and the American Dental Association. Over the past 25 years, he has been a member of the Johns Hopkins Cleft Palate and Craniofacial Rehabilitation Team. Dr. Siegel is an Assistant Clinical Professor at the University of Maryland School of Dentistry post-graduate Orthodontic Department. He is past-president of the Maryland State Society of Orthodontists and the Middle Atlantic Society of Orthodontists; currently he serves as trustee for the American Association of Orthodontists.
