2 Mission Statement The Arizona Coyotes Foundation seeks to enhance the quality of life throughout Arizona communities by supporting non-profit organizations that promote healthcare, education and cultural arts programs for children and service men and women. We are dedicated to health and wellness programs that stimulate the growth of youth sports and financially supporting programs that encourage teamwork skills while building future leaders in our community. 3 Letter from CEO and Foundation Chairman Dear Coyotes Fans and Friends: As we enter our 20th season of NHL hockey in the State of Arizona, we understand that our organization has a unique opportunity to affect positive change within our community. We are very proud of the accomplishments of the Arizona Coyotes Foundation during our season. We donated $450,000 in cash to 30 different organizations in the state that work with children and service men and women in the areas of sports, health, arts and education. Anthony LeBlanc While we are proud of what the Arizona Coyotes Foundation has achieved, we know all too well that we are just getting started. We understand that everything we do can be done better together with other likeminded companies and individuals. With the continued support of our many stakeholders and contributors, including the incredible fans of the Arizona Coyotes Hockey Club, we are well positioned to multiply the magnitude of our impact. And the Arizona Coyotes annual score a hat trick with St. Mary s through their efforts punctuated by enthusiasm, dedication and concern. The Arizona Coyotes Foundation provides enough funds to allow St. Mary s to distribute more than 100,000 meals to the hungry of Arizona. The teams annual Holiday Food Drive provides thousands more meals in the form of non-perishable items donated by their loyal fans. And each year the Coyotes staff, players and wives donate their time packing boxes, distributing food directly to clients or raising awareness through player and mascot appearances. That is a true Hunger Hero! Jerry Brown, Director of Public Relations, St. Mary s Food Bank Alliance I want to thank you again on behalf of FYHA for the Grant from the AZ Coyotes Foundation. This grant was used to help families struggling to afford something their kids really wanted to participate in. We helped 5 families pay for hockey as well as ran a coaching clinic throughout the season (5 sessions) that allowed us to bring educated coaches up from the Valley to coach our coaches. We are trying to retain coaches and help them learn how to coach all ages and levels. We helped up upwards of 30 coaches, as well as giving close to 60 kids, (ranging in age from 6-13), free clinics with emphasis on coaching. This has helped us tremendously! We are very grateful to the AZ coyotes Foundation. Kristi Smith, Flagstaff Youth Hockey Association Breakfast with Santa, a sports day at Riverview Park, a field day on the ASU campus with the Alpha Chi Omega Sorority girls, a special evening greeting guests for A New Leaf s Camaraderie Gala, and painting pottery at As You Wish these are trips the children from the La Mesita After School Program have been able to take already this year, thanks to the generous grant from the Coyotes Foundation. The $30,000 grant was used to purchase a new passenger van for La Mesita for educational and enriching field trips in the community. The La Mesita After School Program provides safe and supportive programming for about 30 children from homeless and low-income families each year. Children, in kindergarten through 8th grade, are provided healthy snacks, assistance with completing homework, tutoring, art activities, indoor and outdoor games, and have access to computers for homework and educational activities. 5 Fundr aising Golf Tournament A beautiful day at the golf course was shared by fans and sponsors as our annual Golf Classic took place on Monday, October 13, 2014 at Whirlwind by Troon at Wild Horse Pass and raised over $106,000. Celebrity Waiters Gala This highly anticipated and sold-out event took place on March 10, 2015 and raised over $161,000. Our fans and sponsors had the opportunity to be waited on by a member of the team and enjoyed a beautiful meal at the Country Club at DC Ranch. Auctions Our silent auctions generated $85,000 for the Arizona Coyotes Foundation. Highlights included our Military Jersey auction, team-themed wood carvings and team signed jerseys. 50/50 Raffle This highly popular raffle raised $337,000 this year. We were proud to host 41 non-profit organizations who raised monies for their organizations while promoting their mission. License Plate Fans can show their pack pride by purchasing an official Arizona Coyotes license plate. The plates can be ordered at servicearizona.com or at any Arizona Motor Vehicle Division or Authorized Third Party office. Fans making a first-time purchase for a non-personalized plate will be charged $25. A fee of $50 will be assessed for personalized plates (6 character limit). These fees will be charged at the time of each registration renewal. The Foundation will receive $17 from all license plate fees collected. Scoreboard Messages/Zamboni Rides Fans are able to donate to the Arizona Coyotes Foundation by purchasing a scoreboard message or a Zamboni ride during any home game. These two fan favorites raised over $8,000. 3
