1 Hospice Wellington: Grief and Bereavement Adult Grief Group: Winter 2018 Description: Hospice Wellington offers grief and bereavement supports to individuals who have lost a loved one. Our services are provided as support groups or one-to-one companionship. What the group/activity aims to support: Grief support groups offer companionship and understanding from others who have experienced a similar loss, and are experiencing the similar challenges that living with grief brings Emotional support in a safe and non-judgemental environment Coping skills to help you through the most difficult days of your grief journey Opportunities to discover new traditions and ideas to keep loved ones present in your hearts and in your memories Group Format: 8 week closed group program. Three to six months following the loss of a loved one, family members and caregivers may feel ready to join one of our Support Groups. The Adult Grief Group runs for an 8 week period for two hours weekly. The program does include a reunion session, four weeks after the last session. Grief Support Groups are available in the afternoon and during the evening. 2 Hospice Wellington: Art Therapy Program These Workshops seek to support the whole person, including: Tuesday Workshop Series: Creative Expressions Description: These workshops are designed to support the individual through the exploration of creative materials to promote creative expression and community connectivity. Gentle group discussion will conclude each workshop component to explore personal life experiences connected to the grief process and memory work towards increased self- awareness and overall well-being. This cycle invites its participants to explore the use of creative materials and narrative therapies to gain personal insight and developing deeper understanding of personal perspectives towards increased resiliency. Relationships, past and present: Current support systems Environment and surroundings Traditions, routines and daily activities Emotional expression Self-care strategies Group Format: Individual closed group workshop program. As this program exists in a community format, individual and group discussion will take place throughout the workshop to support developing thoughts and an inclusive atmosphere. Individual creative projects will be completed during the workshop session, all materials supplied. Registration: Please note each workshop session operates individually and workshops may be attended at the interest of the individual based on personal interest. 3 Hospice Wellington: Art Therapy Program Tuesday Workshop Series Schedule Tuesday, January 9 th Workshop: Pebbles and Stones: Exploring Our Stories through Nature This workshop will gently invite its participants to use simple nature materials to create intuitive representations of memories and experiences. A short creative writing process will be invited as a component of this gentle creative activity. Tuesday, January 23 rd Workshop: Finding Doorways: The Other Side of Grief Please join us for this unique creative process exploring the use of guided meditation in the creation of a unique art-making project, engaging the imagination and the playfulness of paint and found objects. A short creative writing process will be invited as a component of this gentle creative activity. Tuesday, February 6th Workshop: The Golden Thread: Exploring Loss through the Art of Kintsugi Please join us for this unique creative process exploring the use of guided meditation in the creation of a unique art-making project, engaging the imagination and the playfulness of paint and found objects. A short creative writing process will be invited as a component of this gentle creative activity. Tuesday, February 13th Workshop: Finding Our Voices: Spoken Word Workshop We are very excited to introduce an introductory session to exploring the use of creative writing and spoken word to this season s workshop agenda. Please join us for an evening exploring expressive storytelling through several exercises designed to engage and inspire the use of spoken word as a platform to share thoughts and feelings. Tuesday, March 6th Workshop: Exploring the SoulCollage Process This evening workshop will explore the use of collage materials to support connections related to our personal life experiences and support systems. This program runs as a ten-week course in our Spring 2018 Community Calendar, a wonderful introduction! Tuesday, March 20 th Workshop: Exploring Watercolour This workshop will gently invite its participants to explore watercolour materials to explore emotions and personal experiences. A co-creative art piece will also be engaged with all participants involved to support collaborative art-making processing.

4 Hospice Wellington: Art Therapy Program Open Studio Practice: Winter Cycle 2018 Description: To provide and promote a creative therapeutic environment for all Hospice Wellington clients, supporting the continuation of the individual journey through art-making and narrative practices. This program is open to all current Hospice Wellington clients interested in exploring creative expression on their healing journey and to support consistent self-care practices. The semi-open Open Studio program seeks to support the whole person, providing space to explore: A variety of visual materials Discuss and expand on thoughts and creative ideas Work independently and self-directed Engage in dialogue with resident Art Therapist and trained Hospice Wellington volunteers in support of the Open Studio Program Open Studio Format: 8 Week Semi-Open group program. The Open Studio concept encourages freedom of thoughts and exploratory use of creative materials to support self-expression. This program operates with a semi-open atmosphere and does require participants to attend each week of a given Open Studio cycle. The Open Studio Program does, however, require a moderate degree of self-direction and selfinitiative as it is not structured as a closed group process with regulated engagements with the lead facilitator or Art Therapist. 5 Hospice Wellington: Caregiver Support Groups Caregiver Support Group: Spring 2018 Description: This 10 week caregiver support group will offer space to acknowledge an individual s personal caregiving journey. The group will provide emotional validation as well as exploration of self care to relieve caregiver burnout. Together we will engage in a different theme each week giving the caregiver permission to explore their values by offering supportive discussion. What the group/activity aims to support: The Caregiver support group offers companionship and understanding from others who have experiences as a primary caregiver currently supporting someone with a life-limiting illness Emotional support in a safe and non-judgemental environment Coping skills to help you through the most difficult days of a caregiver s journey Opportunities to discover self-care strategies and participate in open group discussions facilitated by a professional clinician Group Format: 10 week closed group program. Hospice Wellington's Caregiver Support Group focuses on the caregiver s anticipatory grief, often expressed as feelings of sadness and anger, prior to the death of a loved one. Over a 10 week period trained Hospice Volunteer Facilitators provide a safe and comfortable environment to share information, provide resources and offer caregiver support. Registration: Registration is required to participate in this group program, including intake and communication with Hospice Wellington. 6 Hospice Wellington: Day Away Program Description: This program is offered once a week to those living with a life-limiting illness. The Day Away program is a break from the routine, providing a welcoming and friendly environment that gives clients an opportunity to share with others going through similar life experiences. The client must be able to get around unassisted or with the aid of a walker/wheelchair. No transportation is provided at this time. What the group/activity aims to support: To provide social and emotional support Focus is on affirming life and living it to the fullest Clients will find rest and relaxation Supportive relationships, sharing stories and laughter Respite for caregivers Weekly Format: Weekly program offered in Guelph for individuals living with a life-limiting illness. This day program starts with a coffee hour and then a hot, family-style lunch is served to conclude the morning. In the afternoon, a variety of activities will be offered such as Therapeutic Touch, crafts, card games, or just the simple opportunity to relax and socialize. 7 Hospice Wellington Integrated Wellness: Massage Program Student Massage Sessions: Winter Cycle 2018 Description: Hospice Wellington in partnership with the Canadian College of Massage & Hydrotherapy (CCMH) in Cambridge provides individually booked massage appointments. Treatments are provided by experienced massage therapy students under the supervision of a Registered Massage Therapist (RMT) to support clients with a life-limiting illness, caregivers, & bereavement. Massage treatments are available in effort to support primarily physical manifestations, and to provide an opportunity for self-care. The following services are available upon request: Deep Tissue Massage Relaxation Swedish Massage Homecare Recommendations Appointment Format: At the first appointment, clients will be required to fill out a health history form and sign in agreement to the program policies. Massage students will meet clients in the reception area at the time of their scheduled appointment and direct them to private treatment rooms. Clients will have the opportunity to discuss any requests for treatment with the student to ensure it will meet the client s individual needs. The student will also review the client s health history form to plan a safe and effective treatment. 8 Hospice Wellington: One-to-One Support Program Description: Hospice Wellington offers One-to-One Volunteer Companionship with a Hospice Wellington trained volunteer. Volunteer visits can take place at Hospice Wellington, in the community, or at the client s home. Clients can have conversations with the volunteer to express their thoughts and feelings, in a non-judgemental environment. What the activity aims to support: Companionship and support for clients living with a palliative diagnosis Companionship and support for clients who are caring for a loved one with a palliative diagnosis Companionship and support for clients who are bereaved Companionship at the bedside of clients who are at the end of their life Format: Clients who would like to be supported on an individual basis will be matched with a trained Hospice Wellington volunteer. These companion sessions will reflect the preference of the client in the location and frequency of the meetings. 9 Hospice Wellington Integrated Wellness: Bereavement Walking Group Walking Group: Winter Cycle 2018 Description: This walking group is offered to support clients in bereavement through weekly participation in gentle physical exercise. Each group will begin with a warm up, followed by indoor track walking for minutes (outdoor walking available upon request, or group consensus), & end with gentle stretching. Bereavement Walking Group will be offered in a semi-open format, through an extension of Hospice Wellington s Integrated Wellness programming. We hope you experience: Gentle, low-impact exercise A compassionate atmosphere supported by others who have also been impacted by grief Program Format: This program will be held at an offsite facility, in order to accommodate both indoor and outdoor walking opportunities, please come prepared to enjoy indoor walking most often. Please meet each week at the reception area of the Royal Distributing Athletic Performance Centre (RDAPC), and consider bringing the following supplies each week, weather dependent: Bottled water, snacks (if needed), locker lock (key or combination), comfortable walking shoes (please note: changing into clean indoor walking shoes is required by the facility when the indoor walking track is used). 10 Hospice Wellington Integrated Wellness: Gentle Yoga Program Gentle Yoga: Winter Cycle 2018 Description: This program offers gentle yoga practices to promote a sense of balance & well-being in the whole person. Yoga practices are thought to be over 10,000 years old, incorporating breath control, meditation, and holding specific body postures. Gentle Yoga will be offered in a semi-open format, through an extension of Hospice Wellington s Integrated Wellness programming. We hope you experience: Relaxation Improved flexibility Tranquil atmosphere Refreshed connection between mind, body & spirit Program Format: Gentle Yoga will be presented in a group format, and is open to all ages and physical abilities. Please join us in exploring the benefits of not only the physical movements, but the internal reflections as well.
