1 for Years Fa milies in Transition s Hope House Providing a temporary home and services to families in need.

2 The Hope House Families in Transition, (FIT) was invited to come to Wolfeboro by concerned community members in order to provide transitional housing to local families in need of assistance. The need in Wolfeboro is immmediate; there are families in the area who are homeless, with no place to go. The purchase of the building is through private investment dollars. The Hope House, the only short-term family housing in the southern Lakes Region of Carroll County, will be a temporary, safe and secure place for families, with 24/7, onsite staff and video surveillance. Currently, there are 126 homeless children in the local school district and several dozen families without a place to live in Wolfeboro. In addition to providing transitional housing to families experiencing homelessness, FIT will provide families with a number of essential services to help these families find permanent residence in their hometown, to break the cycle of homelessness. The goal is to assist families so that they can live, work and raise their children on their own. Details about the project: A home for 42 parents and 84 children per year. Comprehensive intake, assessment and referral designed to direct families to the appropriate homeless and housing resources in the community. Referral to medical care for parents and children. Connection to Department of Education homeless school liaisons to help ensure that school aged children remain in school through their homeless experience. Access to nutritious hot meals. Assistance in increasing income through employment or access to mainstream benefits. Assistance in accessing community services including treatment for mental health or substance use disorders.

3 Who We Are What We Do FIT provides: Emergency Shelter for Homeless Families Short & Long Term Housing for Homeless Families & Individuals Affordable Housing Case Management Support Groups Substance Use Programs Employment Training Skill-Building Workshops Therapeutic Services to Preschoolers Entrepreneurship Serving as a steady stream of revenue for the organization, Families in Transition owns and operates two thrift stores, OutFITters Thrift Store in Manchester, NH, and OutFITters Thrift Store Boutique in Concord, NH, which specialize in selling new and gently used clothing, housewares, books, and furniture. 100% of the proceeds from the thrift stores support Families in Transition housing and services for homeless families and individuals. For more information, visit Families in Transition (FIT) provides safe, affordable housing and social services to individuals and families who are homeless or at risk of becoming homeless, enabling them to gain self-sufficiency and respect. Founded as a nonprofit organization in 1991, Families in Transition was formed as response to the growing number of homeless individuals and families in Manchester and throughout the State of New Hampshire. Since its inception, FIT has been committed to providing only the most innovative and effective programming to help homeless individuals and families reach beyond the cycle of homelessness and lead healthy and successful lives. We believe that having a home is a basic human right and a fundamental component of becoming an engaged and contributing member of the community. Services We Provide Families in Transition provides social services that help fight the root causes of homelessness. With licensed clinical staff and supportive service specialists, Families in Transition helps participants overcome poverty, domestic violence, substance use, mental and/or physical illness, underemployment, addiction, isolation and loneliness. Families in Transition has 18 properties with over 200 housing units located in Manchester, Concord and Dover, NH. In 2015, FIT received nearly 5,000 requests for housing assistance.

4 Homelessness in NH On any given night in January 2016, there were a total of 262 unsheltered homeless people in New Hampshire. In 2015 Families in Transition (FIT) received nearly 5,000 requests for assistance. FIT provided housing to 616 people, 311 of whom were children. In Carroll County There are 126 homeless children in the Governor Wentworth Regional Cooperative (K-12). In Wolfeboro alone there are 26 homeless students and 17 homeless families. There is currently no family safe, short-term, transitional housing in Carroll County. 1 in every 4 homeless people in New Hampshire is a child.
